This method involves taking two or three large *Haemopis* (leeches). First, place a bamboo segment approximately two cun high with the bottom removed onto the sore. Pour a small amount of water into the bamboo, introduce the leeches, and allow them to suck the blood from the sore until they become distended and die. This removes the bad blood from the sore, allowing the condition to gradually lighten and heal, after which medication is applied. It is said that using leeches for bloodletting is applicable only to mild and minor conditions; if toxins are accumulated in the zang-fu organs, merely exhausting blood externally is of no benefit. (I believe that even when using leeches, one must still administer medication according to the syndrome pattern; thus, treating both internally and externally can lead to a cure. To claim that merely exhausting blood is of no benefit is an overly pessimistic view. Toxins are formed by accumulated blood; therefore, sucking out the bad blood is also one way to drive out the toxin.)
Grind *Fuzi* (Prepared Aconite Root) into powder, mix with saliva to form a cake about three qian thick, and place it on the sore. Moxibustion is performed using moxa cones: use larger cones for larger sinuses and smaller cones for smaller ones. The area should be moxibusted until slightly warm, but not painful. Replace the cake when it dries. If swallowing becomes difficult, stop. Repeat the procedure the following day as before, until the flesh levels out, which indicates efficacy. Then, apply a plaster made from the aforementioned tonifying herbs.
The *Douchi* (Fermented Soybean) cake is specifically used to treat back carbuncles (*Fabei*), whether ulcerated or unulcerated. Use light-colored fermented soybeans (*Dan Douchi*) from Jiangxi, grind them into powder, mix with saliva to form a cake, and place it on the affected area for moxibustion. If the cake dries, re-moisten it with saliva. For larger sores, mix with water and pound into a hard paste, forming cakes according to the size of the sore, about three fen thick. If there is already an opening, do not cover it; instead, arrange the *Douchi* cakes around the opening and place moxa cones on top for moxibustion. Keep the area slightly warm without breaking the skin. If heat pain occurs, replace immediately. Perform moxibustion three times daily. In cases with pre-existing openings, healing occurs when sweat discharges from the opening.
Some individuals cannot tolerate fatigue or cold foods, experience abdominal distension after eating, have loose stools, suffer from frequent oral and lingual ulcers, find that symptoms worsen with cooling medicines, experience persistent night sweats, frequent urination, weakness in the waist and legs, feel cold air entering the abdomen when swallowing saliva, have damp and itchy scrotums, cold hands and feet, pale complexion, dark complexion, aversion to cold, or shortness of breath. All the above syndromes belong to Kidney Deficiency (*Shen Xu*) and can only be treated with *Fuzi* (Prepared Aconite Root).
