When the fetus is unstable, accompanied by pain in the lower back and abdomen, and even a sensation of bearing down at its worst, this may not necessarily be due to deficiency of Qi and Blood, failing to nourish the fetus. The fetus in the womb is like fruit on a branch; if the branch withers, the fruit falls, which is a natural principle. If women indulge their desires excessively, causing internal Fire to flare up, it can also lead to fetal instability and miscarriage. Generally, the causes fall into two categories: deficiency or Fire. Treatment involving clearing Heat and nourishing Blood covers these aspects. Additionally, there are two other causes for disturbing the fetus that must be known: one is when the mother's illness disturbs the fetus; treating the mother's illness will stabilize the fetus naturally. The other is when external trauma disturbs the fetus; taking two or three doses of fetal-calming herbs will naturally stabilize the fetus.
1. Fetal pain caused by insufficient Blood: Use Siwu Tang (Four Substances Decoction) added with Cyperi Rhizoma (Xiangfu) processed with child's urine, ground into powder. Take 3 qian each time, taken down with Perillae Folium (Zisu) decoction.
2. For pain caused by injury or disturbance: Use Xionggui Tang (Ligusticum and Angelica Decoction) and Dusheng San (Single Sage Powder), which are highly effective.
3. Spontaneous fetal movement caused by excessive Fire: Use Siwu Tang (Four Substances Decoction) added with Scutellariae Radix (Tiaoqin). If there is rebellious Qi rising causing asthma/panting, add Cyperi Rhizoma (Xiangfu) and decoct together.
4. For fetal instability and abdominal pain with bleeding: It is suitable to take Jiyan Fang (Collected Effective Formula), specifically Sangjisheng San (Mulberry Mistletoe Powder) or Oyster Shell (Muli) preparations.
5. Fetal movement termed "fetal swelling" is mostly due to Dampness. Use Gardeniae Fructus (Shanzhi) ground into powder, taken down with rice soup, or made into pills. Baizhu San (Atractylodes Macrocephala Powder) is also excellent.
6. For any stage of pregnancy, if one feels fetal instability, slight lower back and abdominal pain, and loss of appetite, one can take Antai Yin (Fetal-Calming Drink) or Baotai Wan (Fetal-Protecting Pills).
7. During the seventh, eighth, or ninth month of pregnancy, if the fetus cries in the womb, it is because the pregnant woman reached high to fetch objects or stretched her arms to comb her hair, causing the umbilical cord knot near the fetus's mouth/nose to slip out, producing sound. Have the pregnant woman bend over to pick up an object from the ground, allowing the fetus to re-swallow the cord, which stops the crying. Another method is to hold a piece of soil from a rat hole in an old empty house in the pregnant woman's mouth to stop it. Alternatively, decoct Coptidis Rhizoma (Huanglian) and let the pregnant woman inhale the fumes, which also stops it.
Antai Yin (Fetal-Calming Drink): Treats fetal instability during pregnancy, slight lower back and abdominal pain, and reduced intake of food.
Baizhu (Atractylodis Macrocephalae Rhizoma), Baishaoyao (Paeoniae Radix Alba), Shudihuang (Rehmanniae Radix Praeparata), Danggui (Angelicae Sinensis Radix) (each 1 qian); Renshen (Ginseng Radix et Rhizoma), Chuanxiong (Chuanxiong Rhizoma), Tiaoqin (Scutellariae Radix), Chenpi (Citri Reticulatae Pericarpium) (each 0.5 qian); Gancao (Glycyrrhizae Radix et Rhizoma), Sharen (Amomi Fructus), Zisu (Perillae Folium) (each 0.3 qian).
Add 1.5 cups of water, decoct to 0.8 cup, take at any time.
For bleeding, add Ejiao (Colla Corii Asini); for severe cases, add Aye (Folium Artemisiae Argyi); for severe abdominal pain, double the dosage of Shaoyao (Paeoniae Radix).
Gutai Yin (Fetal-Securing Drink): Treats deficiency of both Qi and Blood during pregnancy, habitual miscarriage within three months. Use this formula first to secure the fetus and prevent miscarriage.

Chuanxiong (Chuanxiong Rhizoma) (not roasted, amount unrestricted, ground into fine powder).
Take one cun bi (square-inch spoonful) mixed with Mugwort (Ai Ye) decoction on an empty stomach. If movement is felt in the abdomen, there is a fetus; if movement is below the navel, it is a blood mass (Xue Jia); if no movement, it is blood stagnation, indicating disease.
Xionggui Tang (Ligusticum and Angelica Decoction): Treats all types of fetal instability.
Chuanxiong (Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix) (washed with wine).
Cut into equal parts, take 5 qian each time. Add one cup of good wine, decoct until nearly dry, add one cup of water, boil for five to seven boils, strain, and take warm.
Dusheng San (Single Sage Powder): Treats pregnancy disturbed by injury/trauma, agitating the fetal origin, causing abdominal pain and bleeding. Highly effective.
Sharen (Amomi Fructus) (amount unrestricted, stir-fry with peel, do not burn black, remove peel, take the kernel, grind into powder). Mix with ripe wine to take. For those who do not drink alcohol, mix with rice soup.
This single herb is indeed essential for pregnant women, as it has the functions of calming the fetus, facilitating childbirth, and guiding stagnation.
Sisheng San (Four Sages Powder): Treats leaking fetus with bleeding.
Tiaoqin (Scutellariae Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Sharen (Amomi Fructus), Ejiao (Colla Corii Asini) (equal parts).

Add 2 zhong of water, decoct to 1 zhong, take warm.
Zetai Yin (Fetal-Covering Drink): Treats pregnancy complicated by acne/rash; taking this prevents damage to the fetus. If fetal instability occurs, use Dusheng San (Single Sage Powder) or Antai Yin (Fetal-Calming Drink).
Chifuling (Poria Cum Ligno Rubrum), Baizhu (Atractylodis Macrocephalae Rhizoma), Danggui (Angelicae Sinensis Radix), Baishaoyao (Paeoniae Radix Alba), Chishaoyao (Paeoniae Radix Rubra), Tiaoqin (Scutellariae Radix), Gange (Puerariae Lobatae Radix), Renshen (Ginseng Radix et Rhizoma), Jiegeng (Platycodonis Radix), Chaihu (Bupleuri Radix), Fangfeng (Saposhnikoviae Radix), Chenpi (Citri Reticulatae Pericarpium), Jingjie (Schizonepetae Spica), Zhike (Aurantii Fructus Immaturus), Zicao (Arnebiae Radix seu Lithospermi Radix), Ejiao (Colla Corii Asini), Nuomi (Oryza Sativa Glutinosa), Baizhi (Angelicae Dahuricae Radix), Gancao (Glycyrrhizae Radix et Rhizoma), Chuanxiong (Chuanxiong Rhizoma), Suosharen (Amomi Fructus) (each 0.3 qian).
