Yang stated: The syndrome of slow convulsion (Man Jing Feng) may arise from the excessive use of cooling herbs during acute convulsion (Ji Jing Feng), or result from transmission due to excessive purgation, or occur due to persistent vomiting and diarrhea, or develop from sudden vomiting caused by Qi deficiency. It can be formed by deficiency of the Zang-Fu organs and severe diarrhea (Dong Xie), where wind pathogen enters the intestines and stomach, causing incontinence of stool and dysentery. It can also be formed by chronic dysentery leading to exhaustion of Stomach Qi, or by the transmission of Cold Damage (Shang Han) into a Yin syndrome. It can develop from prolonged coughing evolving into epilepsy. It can result from harsh purgation of accumulated pathogens, or from hematemesis and hematochezia. It can be caused by unresolved wind pathogen combined with erroneous medication. It can arise from unceasing epileptic seizures, from intestinal parasites rushing to the heart, or from testicular swelling, hernia, and abdominal pain. Additionally, night sweats, spleen fatigue with excessive sleepiness, irritability with thirst, edema of the four limbs, constipation of both urine and feces, erysipelas-like swellings and toxic sores, herpes zoster (Dan Liu), and rapid-progressing cancrum oris (Zou Ma Ji Gan) can all transmit into the slow convulsion syndrome. Among these, only those becoming deficient due to prolonged vomiting, diarrhea, and accumulation are such that improper treatment leads to rapid development of complicated syndromes.
Slow convulsion is caused by prolonged vomiting and diarrhea gradually weakening Stomach Qi. The Stomach and Lung have a mother-son relationship; when the mother (Stomach) is deficient, it causes weakness in the son (Lung). Deficiency in these two meridians generates deficiency phlegm, which originates from the Lung and Stomach. This phlegm stagnates in the throat, producing a sound like sawing wood, accompanied by occasional twitching and convulsions.
In infants, after illness or due to vomiting and diarrhea, damage to the Spleen and Stomach results in coldness throughout the body, cold breath from the mouth and nose, intermittent convulsions of the hands and feet, lethargy with eyes half-open, indicating a lack of Yang. Acute and slow convulsions represent Yang and Yin syndromes respectively, requiring careful differentiation and treatment. Acute convulsion should be treated with cooling and purging methods, while slow convulsion requires warming and tonifying methods. Ignorant physicians of the world fail to distinguish between them, causing many deaths. When an infant contracts wind-cold and suffers from vomiting and diarrhea, if the physician diagnoses Spleen deficiency and treats with warming and tonifying herbs without improvement, then switches to cooling herbs without success, and further attacks under the assumption of initial wind injury, the Spleen Qi becomes deficient, leading to loose stools and diarrhea. At this stage, one should dispel Spleen wind; once the wind is eliminated, the diarrhea stops. The primary formula is Xuan Feng San (Disperse Wind Powder), followed by Shi Jun Zi Wan (Quince Seed Pill) to tonify the Stomach. There are also cases of prolonged vomiting and diarrhea that do not heal, resulting from Spleen deficiency generating wind and causing convulsions.
A deep, slow, scattered, and relaxed pulse indicates slow convulsion. A blueish-purple line on the Tiger's Mouth vein (Hu Kou Mai Wen), moving back and forth, indicates slow convulsion. Faintly mixed blue-purple or black lines, appearing as if emerging but not fully showing, indicate slow Spleen wind (Man Pi Feng).
