According to *Guoxiao* (Small State): Seminal emission in dreams and involuntary seminal leakage are caused by the invasion of damp-heat. Heat causes free flow; due to heat damaging qi and blood, the essence cannot be consolidated and retained. Dai Shi said: All these cases are stirred by ministerial fire. Prolonged illness leads to deficiency, yet there is no cold pathogen involved.
According to *Jingui Yaolue* (Essential Prescriptions of the Golden Cabinet): The disease of consumptive fatigue presents with a floating and large pulse, vexation and heat in the hands and feet, coldness in the yin region, and spontaneous emission of semen. It also states: A string-like and large pulse is called "ge" (leather) pulse; men suffer from seminal emission, while women suffer from blood loss. A flooding and rapid pulse at both chi positions indicates seminal loss and turbid urine. A short and small heart pulse is caused by heart deficiency and must involve dream emission.
Zhu Danxi said: Dream emission and seminal leakage are primarily governed by heat. Heat causes excessive free flow; treatment should focus on nourishing yin and descending fire. For those ill due to mental labor and overthinking, one should calm the spirit and nourish the heart; for those who have become deficient and collapsed after prolonged illness, tonifying herbs combined with astringent herbs must be used, ensuring recovery.
According to *Bencao Gangmu* (Compendium of Materia Medica): A young adult suffered from dream emission and white turbidity; administering astringent herbs exacerbated the condition. Recognizing this as stagnation due to constraint, a large dose of Daochi San was administered, stopping the turbidity. Another middle-aged man suffered from dream emission; astringent herbs were ineffective. He was treated with Shenxiong Wan to purge, followed by Zhuling Wan, resulting in recovery. Zhu Danxi treated a patient with seminal leakage and turbid urine; medication was ineffective until the Daocang method (cathartic/vomiting therapy) was employed, leading to stabilization. These are all cases of excess pathogenic stagnation.
[Dream Emission]: Caused by fire in the heart meridian and unsettled spirit/thoughts, leading to sexual intercourse in dreams and seminal emission. Treatment should clear the heart and calm the spirit. Wendan Tang (Warm Gallbladder Decoction) can be modified with Huanglian (Coptis Rhizome), Shengdi (Rehmannia Root), Renshen (Ginseng Root), Yuanzhi (Polygala Root), Fushen (Poria with Hostwood), Suanzaoren (Ziziphus Seed), Lingyangjiao (Antelope Horn), etc.
[Spontaneous Emission]: Caused by deficiency of qi and blood leading to downward collapse of essence. Some cases involve excessive free flow due to heat. One must differentiate between deficiency and excess. Bayu Tang (Eight Substance Decoction) can be modified with Longgu (Fossilized Bone), Muli (Oyster Shell), Chushugenpi (Bark of Chuk Tree), etc.
For cases where semen flows uncontrollably after urination, or flows spontaneously without urination, or flows from the urethra accompanied by itching and pain, with a constant sensation of needing to urinate, one should first administer Chensha Miaoxiang San (Cinnabar Wonderful Fragrance Powder), Weixi Wan (Weixi Pill), or Liqing Yin (Clearing Decoction). Additionally, Longgu (Fossilized Bone) wrapped in cotton should be decocted together with Wubeizi (Galla Chinensis), Muli (Oyster Shell), Baifuling (White Poria), and Wuweizi (Schisandra Fruit).
