经曰∶诸热瞀螈,皆属于火;诸痛疮痒,皆属于心;诸禁鼓栗,如丧神守,皆属于火;
暴喑,无声也。金受火邪,浊而不鸣也。金清则善鸣,火盛则金浊,难鸣而喑也。冒,昏冒也;
(Li Dongyuan) Run Chang Wan (Intestine-Moistening Pill): Treats latent fire in the Spleen and Stomach. Symptoms include constipation with difficulty, or dryness and obstruction, complete loss of appetite. This is due to wind-dryness and blood-deficiency constipation. This formula moistens dryness, harmonizes blood, and disperses wind, allowing natural bowel movement.
Hemp Seed (Cannabis Fructus), Peach Kernel (Persicae Semen) (each 1 liang, remove skin and tips, grind separately), Qiang Huo (Notopterygii Rhizoma et Radix), Dang Gui Wei (Angelicae Sinensis Radix caulis), Da Huang (Rhei Radix et Rhizoma, baked), Zao Jiao Ren (Gleditsiae Fructus Semen), Qin Jiao (Gentianae Macrophyllae Radix) (each 0.5 liang).
Grind the above herbs into fine powder, mix evenly with refined honey, and make pills the size of a paulownia seed. Take 40 pills each time, swallowed with plain warm water on an empty stomach.
Ma Ren Wan (Hemp Seed Pill):
Yu Li Ren (Prunus japonica Seed), Hemp Seed (Cannabis Fructus) (each 6 liang), Da Huang (Rhei Radix et Rhizoma) (2 liang, half raw, half stir-fried), Shan Yao (Dioscoreae Rhizoma), Fang Feng (Saposhnikoviae Radix), Zhi Ke (Aurantii Fructus, stir-fried) (each 7 qian 5 fen), Bing Lang (Arecae Semen), Qiang Huo (Notopterygii Rhizoma et Radix), Mu Xiang (Aucklandiae Radix) (each 0.5 liang).
Grind the above herbs into fine powder, make pills with refined honey, the size of a paulownia seed. Take 30-40 pills each time, swallowed with warm plain water.
(Liu Wansu's *Ji Yao*) Qin Jiao Wan: Treats blood deficiency causing tendon dryness, Yin deficiency failing to nourish Ying (nutritive Qi), resulting in hands and feet lacking moisture and mobility, dry nails, and severe wind-heat. (Formula see "Wind Gate" chapter.)
(Zhang Congzheng) Pi Yue Wan (Spleen-Restriction Pill): Treats constipation caused by wind-dryness. (Formula see this gate chapter.)
(Li Dongyuan) Sheng Ye Gan Lu Yin (Life Fluid Sweet Dew Drink) (also known as Qing Liang Yin Zi): Treats Xiao Ke (Wasting-Thirst) where the patient eats well but becomes emaciated, has dry mouth and tongue, spontaneous sweating, dry and hard stools, and frequent urination.
He Xue Yi Qi Tang (Harmonize Blood and Boost Qi Decoction): Treats dry mouth and constipation, red meridians on the tongue. This medicine generates fluids, removes dryness, and promotes muscle growth.
Xin Run Huan Ji Tang (Pungent-Moistening Muscle-Soothing Decoction): Treats skin fissures and various skin dryness, roughness, and lack of moisture. (The above three formulas are all in the "Xiao Ke Gate" chapter.)
Huo Xue Run Zao Wan (Invigorate Blood and Moisten Dryness Pill): Treats Wind Bi (constipation due to wind) and Blood Bi (constipation due to blood deficiency), characterized by frequent dry and hard stools.
Run Chang Wan (Intestine-Moistening Pill): Treats dry and hard stools. (The above two formulas are both in the "Bi Jie Gate" chapter.)
The Classics state: All cases of heat-induced dizziness and convulsions belong to Fire; all pain, sores, and itching belong to the Heart; all cases of jaw stiffness, shivering, and chattering teeth, appearing as if losing spiritual control, belong to Fire; all cases of rebellious Qi rushing upward belong to Fire; all cases of abdominal distension and enlargement belong to Heat; all cases of dryness, mania, and unruliness belong to Fire; all diseases producing sounds, where percussion yields a drum-like resonance, belong to Heat; all diseases involving swelling, pain, soreness, fright, and alarm belong to Fire; all cases of twisting and contracture, with turbid urine, belong to Heat; all cases of vomiting acid, sudden profuse diarrhea, and tenesmus belong to Heat.
It is also said: All heat-induced dizziness and convulsions, sudden aphonia, mental confusion, restlessness, mania, cursing, and fright; swelling, pain, and soreness; rebellious Qi rushing upward; jaw stiffness and shivering as if losing spiritual control; sneezing, vomiting, sores, and itching; throat obstruction, tinnitus, and deafness; vomiting and fluid overflow, inability to eat, blurred vision, sudden profuse diarrhea and convulsions, sudden illness and sudden death—all belong to Fire.
*Yuan Bing Shi* states: "Mao" means dizziness; "Yong" means twitching. When heat is severe, the spirit becomes dull, the heart palpitates, and muscles twitch; these are caused by Fire.
"Sudden Aphonia" refers to sudden loss of voice. Metal (Lung) affected by Fire pathogen becomes turbid and cannot resonate. When Metal is clear, it resonates well; when Fire is excessive, Metal becomes turbid, making speech difficult and leading to aphonia. "Mao" means dizziness/confusion; "Mei" means dimness/obscurity. Muggy Fire Qi causes the spirit to be dim and unclear. "Zao" means restless agitation and vexation; "Rao" means disturbance. Severe Fire leads to a vexed heart, insomnia, restlessness in sitting and lying down, and unsettled spirit/mind. "Kuang Yue" means madness and deviation, disordered speech, and abnormal behavior. "Ma Li" means insulting others with foul language. "Jing Hai" means being startled by sound, feeling alert fear and alarm; these occur when Fire pathogen is excessive and Righteous Qi loses mastery. Swelling, pain, and soreness refer to fleshy areas near the feet; excess Heat causes swelling, excess Wind causes movement/twitching. "Teng" is mild pain; "Tong" is severe pain. "Suan" is like pain but not quite pain, a prickling soreness in the marrow resembling pain. This is the Fire of Liver Wood, hence the sensation of sourness. It is like extreme drunkenness where limbs and joints feel deep soreness. Rebellious Qi rushing upward is due to Fire flaring up. "Jin Li Ru Sang Shen Shou" refers to trembling and shaking where the body loses its spiritual guardian. "Ti Ou" is sneezing due to itchiness in the nose from Qi. Fire scorching Lung Metal causes pathogens to exit through nasal orifices, causing itchiness and sneezing. Sores and ulcers are caused by blazing Heart Fire in the skin. "Hou Bi" is heat lodging in the Upper Jiao, causing sore throat and swelling, difficulty swallowing saliva, and insensitivity. Tinnitus is caused by Kidney Water failing to nourish, with Fire flaring to the upper orifices, producing sounds like bells, drums, or cicadas.
Vomiting fluid overflow and inability to eat: Fire flares upward, causing intense heat in the Stomach and Diaphragm, disrupting transmission and transformation, leading to vomiting and reflux. Blurred vision is due to heat Qi stagnating and steaming. Inability to see clearly in broad daylight is due to severe heat Qi stagnation. "Bao Zhu Yong": "Bao Zhu" is sudden diarrhea; severe Heat causes rapid flow, resulting in watery diarrhea. "Yong" is jumping/twitching. Sudden illness and sudden death: Fire nature is swift. If heat Qi stagnates for a long time, it suddenly causes mental confusion, immobility of sinews and bones, leading to sudden collapse and death. These are all caused by Fire.
The Classics state: The Qi of Strong Fire declines; the Qi of Weak Fire strengthens. Strong Fire consumes Qi; Qi nourishes Weak Fire. Strong Fire dissipates Qi; Weak Fire generates Qi. Strong Fire consumes Qi, so Qi dissipated by Strong Fire is depleted; Qi nourished by Weak Fire grows long. This means Weak Fire nourishes Qi.
*Yuan Bing Shi* states: Excessive emotion of the Five Zang organs damages their respective spirits. All injuries from the Five Spirits generate Heat. Yang moves, Yin is still; thus, physical and mental labor leads to dryness and unrest. Stillness brings clarity and peace. Therefore, the supreme good is like water; the lowest ignorance is like fire. Sudden fainting often results from excessive Five Spirits and Seven Emotions causing sudden illness. Thus, managing Heart Fire emotions: illness manifesting as laughter indicates severe Fire. Extreme excess of the Five Spirits all transforms into Fire.
