Pulse
When the heart feels startled and palpitations occur, the pulse must be large and knotted (Jie). For palpitations caused by dietary indiscretion, the pulse is typically deep (Chen), hidden (Fu), and slippery (Hua).
Diseases
Zhen Tong (Palpitations with a sense of unrest) refers to a restless and uneasy feeling in the heart, characterized by fearfulness as if someone were about to capture one. It often arises from the worries of the wealthy or the unfulfilled desires of the poor. It belongs to Blood Deficiency, where symptoms are triggered by overthinking; it is also a Deficiency pattern, where symptoms come and go intermittently. If caused by Phlegm-Fire agitation, thin individuals usually have insufficient Blood, while fat individuals tend to have Phlegm.
Jing Ji (Startled Palpitations) refers to sudden jumping and trembling due to fright, with a sensation of impending fainting, occurring intermittently. This belongs to Blood Deficiency. Occasional feelings of heart beating are also due to Blood Deficiency. The Heart governs the human body, and the Heart is nourished by Blood. Once Heart Blood becomes deficient, the Shen (Spirit) loses its residence, which is the origin of Jing Ji. Furthermore: "Jing" refers to terror. Zhen Tong, Jian Wang (Amnesia), and Jing Ji are three different names for the same disease pathology.
