Chaos (Zaocao) is caused by liver fire taking advantage of spleen deficiency to gather and attack. If the location is in the stomach opening, use Xiong Gui Shao Yao Tang (Safflower and Angelica Decoction) added with Gardenia seed (Shanzhiren) and Agarwood (Chenxiang); if the location is in the chest, use Xiong Gui Shao Yao Tang added with Perilla leaf (Zisu); if the location is in the middle jiao, use Atractylodes macrocephala Rhizome (Baizhu) as the sovereign herb, with Tangerine peel (Chenpi) and Coptis Rhizome (Chuanlian) as ministers, or combine Atractylodes macrocephala Rhizome (Baizhu), Chinese Yam Rhizome (Shanyao), White Peony Root (Baishao), Lotus Seed (Lianzi), Ginseng Radix et Rhizoma (Renshen), and Licorice Root (Gancao); if the location is in the lower jiao, use Liu Wei Wan (Six-Ingredient Pill with Rehmannia), strictly avoiding drying herbs. After sweating, purging, or vomiting, if there is a feeling of tightness in the chest and diaphragm accompanied by chaos in the lower abdomen, use Ba Zhen Tang (Eight-Treasure Decoction); Chuanxiong Rhizome (Chuanxiong) expands the chest, and Rehmannia Root (Shengdi) clears heat/fire; regardless of other diseases, whenever chaos occurs, add Rehmannia Root (Shengdi).
Case Record:
A person with spleen and stomach deficiency-cold experienced chaos in the epigastrium. They took decoctions of Atractylodes macrocephala Rhizome (Baizhu) 1.5 qian, Coptis Rhizome (Chuanlian) 0.1 qian, Tangerine peel (Chenpi) 0.5 qian, and Evodia Fruit (Wuyu) 1 qian. For pills, they used Atractylodes macrocephala Rhizome (Baizhu) 1 liang, Coptis Rhizome (Chuanlian) 0.5 qian, Tangerine peel (Chenpi) and Evodia Fruit (Wuyu) 2 qian each, bound with dough made from Medicated Leaven (Shenqu), taken with warm water.
Yu Syndrome (Stagnation Syndrome) occurs when earth qi fails to ascend and heaven qi fails to descend, causing turbid qi to rise upward while clear yang sinks downward. One should nourish the Lung to facilitate its descending function, secure the Kidney to assist the mechanism of generating Stomach qi, and soothe the Liver to turn the pivot of Shaoyang. Thus, heaven and earth will be in their proper positions, and the middle jiao will be peaceful. Xiao Yao San (Free and Easy Wanderer Powder) should be applied to unblock stagnation.
Case Record:
A person presented with choppy pulses in all six positions, hypochondriac pain, expectoration of foul-smelling phlegm, nausea, and inability to eat. The hypochondrium is the region governed by Shaoyang. When suppressed and not flowing smoothly, turbid qi stagnates in the Shaoyall collaterals, causing pain; turbid qi obstructs body fluids, leading to expectoration of foul-smelling phlegm and nausea; the inability to eat is due to the failure of Shaoyang clear qi to ascend, which prevents the Liver from distributing essence. Therefore, one should regulate and smooth Liver Wood Qi, using Bupleurum Root (Chaihu) and White Cardamom Seed (Baidoukou) 0.2 fen each, Black Gardenia Fruit (Hei Shanzhi) and Licorice Root (Gancao) 0.5 fen each, White Peony Root (Baishao) and Moutan Cortex (Danpi) 1 qian each, Poria (Fuling), Broad-leaved Citrus Peel (Guangpi), and Pinellia Tuber (Banxia) 1.5 qian each, Angelica Sinensis Radix (Guishen) 0.8 fen, and Ophiopogon Tuber (Maidong) 2 qian. After ten doses, full recovery was achieved.
The syndrome of Fright and Alarm (Jinghai) is due to the non-interaction between the Heart and Kidney. The fu organ of the Heart is the Small Intestine, and the fu organ of the Kidney is the Bladder. Kidney qi ascends from the Bladder to the Lung, then to the Heart, and returns from the Heart to the Kidney. How can it cross an zang and a fu organ directly to interact? If there are pathogenic factors in the zang-fu organs, barriers arise, preventing Yang from ascending and Yin from descending. Consequently, Heart-Kidney non-interaction leads to Heart deficiency causing frequent alarm, and Kidney deficiency causing frequent fright. (Zhang Dongfu stated: This is because internal qi is first deficient, and suddenly encountering dangerous or bizarre objects causes Heart-Kidney non-interaction resulting in fright and alarm. If referring to the Liver disease causing fright mentioned in Neijing, or the sudden startle upon hearing wood sounds due to Foot Yangming meridian,)
Case Record:
A person developed fright and alarm after their mother became seriously ill. They were treated with Guipi Tang (Restore the Spleen Decoction) added with Salvia Root (Danshen) for ten doses, and pills of Tianwang Buxin Dan (Emperor of Heaven's Special Pill to Tonify the Heart) were used, leading to recovery.
