Double Throat Bi (Shuang Hou Bi)
Double Throat Bi develops in the upper part of the throat, on both sides inside the Guan position, resembling an olive in shape. It causes pain and difficulty in eating. It is caused by accumulated heat in the Stomach (Wei), or accompanied by chills and fever; a surging pulse at both Guan positions indicates this condition. One may needle the affected area or Shangyang (LI1). First, administer one dose of Liuwei Decoction (Six-Ingredient Decoction). The next day, add: Huangqin (Scutellariae Radix, stir-fried with wine), Shanzhi (Gardeniae Fructus), Mutong (Akebiae Caulis), Yuan Shen (Scrophulariae Radix, stir-fried with salt water), each qianban (approx. 5g). Continue taking it to resolve the symptoms. (If ulceration is present, do not needle; instead, blow Jinebuwan powder onto the lesion.)
Single Throat Bi (Dan Hou Bi)
Single Throat Bi occurs on either the left or right side. The treatment is the same as described above.
Pale Red Throat Bi (Dan Hong Hou Bi)
Pale Red Throat Bi presents with swelling the size of a chicken egg, inability to swallow food or drink, chills and fever, red eyes, and vomiting. There is fear of internal Ban Du (maculopapular toxin/rash poison). Urgently needle: Shaoshang (LU11), Shaoyin (KI10 or HT7 depending on context, here likely referring to specific throat points or Kidney meridian points), Shangyang (LI1), Guanchong (SJ1) (eight acupoints in total, left and right). Alternatively, puncture the affected area. Use Liuwei Decoction added with: Suye (Perillae Folium), Qianghuo (Notopterygii Rhizoma et Radix), Gegen (Puerariae Lobatae Radix), each 2 qian (approx. 6g); Xian Yuanxi (Coriandri Fructus, fresh), 5 qian (approx. 15g). After one dose, if macules and papules appear all over the body, vomiting will cease. If fever persists and swelling extends outside the throat, this indicates internal fire venting outward. Change the prescription to add: Sheng Dahuang (Rhei Radix et Rhizoma, raw), 3 qian (approx. 9g); Gegen, Huangqin, Shanzhi, Xuanshen (Scrophulariae Radix), Huafen (Trichosanthis Radix), each 2 qian (approx. 6g); Sheng Shigao (Gypsum Fibrosum, raw), 5 qian (approx. 15g); Huashi (Talcum), 2 qian (approx. 6g); Mutong, 1 qian (approx. 3g). After two doses, remove Dahuang and Shigao, then continue with the remaining herbs. Follow the prescription for four to five doses to recover. If there are ulcerated spots, take one dose of Baxian San (Eight-Immortal Powder), dissolve it in saliva and swallow, while also drinking juice from Cypress branches. This condition arises because pathogenic factors from Cold Damage (Shanghan) and seasonal evils have not been cleared. A sunken and fine pulse at both Guan positions, and a deficient and rapid pulse at the Cun and Chi positions on both hands, indicate this pattern.
Zouma Throat Bi (Zouma Hou Bi)
This is an acute condition. It is caused by the closure and blockage of Fire in the Liver and Spleen, resulting in Bi syndrome. If accompanied by chills and fever, a surging pulse indicates survival, while a sunken and fine pulse indicates death. Use Liuwei Decoction added with: Gegen (2 qian / approx. 6g), Chaihu (Bupleuri Radix, 1 qian / approx. 3g), Xixin (Asari Radix et Rhizoma, 5 fen / approx. 1.5g, used for rinsing). Further add: Jiaoci (Acanthopanacis Cortex, 2 qian / approx. 6g), Guiwei (Angelicae Sinensis Radix, tail portion, 2 qian / approx. 6g), Chishao (Paeoniae Rubra Radix, 2 qian / approx. 6g), Heche (Placenta Hominis, 2 qian / approx. 6g), Sheng Dahuang (5 qian / approx. 15g). If there is excessive phlegm, add Fushi (Lapilli, 3 qian / approx. 9g) and Zhi Banxia (Pinelliae Rhizoma, processed, 2 qian / approx. 6g). If there is body heat with coldness in the back, add Qianghuo (1 qian / approx. 3g) and Suye (1 qian / approx. 3g). Needle: Shaoshang, Shangyang, Guanchung (six acupoints on both hands). Profuse bleeding is considered auspicious/effective.
