Commentary states: When the pathogenic heat of cold damage (Shanghan) attacks the hands and feet, excessive heat leads to swelling. The qi of heat-toxin lodges in the fu-zang organs; the pathogen knots within the interior, follows the meridians outward, and pours into the four limbs. Thus, it causes the person to experience vexatious heat, redness, swelling, and pain in the hands and feet.
