Commentary states: The retention of the placenta (Baoyi, fetal membrane) is less critical than the situation before the fetus descends; however, a dead fetus in utero (Zisi Fuzhong) is more dangerous than retained placenta. This is because when the placenta has not been expelled, the fetus and mother share respiration. If the fetus dies in the womb, the Qi of the uterus becomes cold, causing fetal blood to coagulate and freeze around the dead fetus. This prevents the ascending and descending of Qi, making expulsion difficult. Ancient formulas often utilized herbs that promote blood circulation and regulate Qi, as well as minerals such as Nuxit (Niter), Shuiyin (Mercury), and Naosha (Sal Ammoniac). However, once the fetus dies, its body becomes cold, blood coagulates, and Qi stagnates. Administering extremely cold herbs under these conditions is not only ineffective but may also endanger the mother's life.
Although ancient physicians did not use medications without intent, later scholars failed to distinguish the nuances. There are two main causes for fetal death, and the choice between warm or cold herbs should be appropriate to each case. Some cases involve fetal leakage during pregnancy leading to excessive blood loss and fetal death; others result from internal injury due to falls or impacts; still others occur due to fetal atrophy caused by prolonged illness. In such instances, an assistant should support the pregnant woman to keep her body upright, secure her waist with a cushion, and have her sit with legs spread facing the ground. If labor signs appear, administer three doses of Fuzi Tang (Aconite Decoction) [formula follows]. Then, wrap several heated tiles in cloth and have two or three people continuously apply heat compresses around the umbilicus, abdomen, waist, and flanks to warm the uterus, melt the coagulated blood, and promote flow. Since Fuzi Tang can dispel cold and expel the fetus, after taking it, one should feel intermittent abdominal pain, followed by the movement of lochia. Eventually, delivery will occur. This illustrates the rationale for using warm herbs.
In other cases, the fetus dies due to external pathogens such as Shanghan (Cold Damage), febrile diseases, or Wen Nue (Warm Malaria). The fetus is attacked by pathogenic heat and toxins from both inside and outside, leading to fetal death and retention in the uterus. Ancient practitioners feared that toxin accumulation would cause distension, hence the use of Nuxit, Shuiyin, and Naosha to separate the stem and branches, facilitating the expulsion of the dead fetus. A fetus dying in the womb relies on the mother; if gestation is incomplete, the umbilical connection remains firm. Like an unripe fruit, even if the branch withers, it does not detach unless broken. Therefore, proper methods and measured usage ensure safety without worry.
When diagnosing a dead fetus, careful differentiation is required. If a pregnant woman’s condition hides fetal movement, making survival uncertain, an assistant should gently palpate below the umbilicus towards the fetus. Coldness upon touch indicates fetal death; warmth suggests otherwise. Observe external signs: cyanosis of the face, cold tongue, and bluish-black sublingual veins are all indicators of fetal death.
