It is stated that prolapse of the anus following dysentery occurs due to deficiency and coldness stagnation in the Large Intestine (Da Chang). When going to the toilet, there is an urge to defecate but inability to do so. Straining forcefully downward causes the anus to protrude, hence it is called "prolapse of the anus" (Tuo Gang). Warming the viscera leads to recovery. Ancient prescriptions include sitz baths with decoctions and warm moxibustion/ironing therapies, which are all excellent methods.
For treating prolapsed anus not retracting, accompanied by tenesmus (Li Ji Hou Zhong - abdominal urgency and heaviness in the rectum), use Ci Shi San Formula:
Ci Shi (Magnetitum, calcined and quenched in vinegar, 4 liang)
Gui Zhi (Ramulus Cinnamomi, bark removed, 1 liang)
Fu Zi (Radix Aconiti Lateralis Preparata, roasted until yellow and cooked, 1 piece)
Crush and sieve the above three ingredients into powder. Take 2 qian bi per dose, mixed with rice water. Avoid lifting heavy objects and tight belts; abstain from sexual intercourse. Recovery may take a year.
For treating prolapse of the anus after dysentery, use Wo Niu San Formula:
Wo Niu (Cochlicopa, burned to ash and finely ground, 37 pieces)
Ci Shi (Magnetitum, calcined and quenched in vinegar, finely ground, 1 liang)
Grind the above two ingredients together until powdery. Take 2 qian bi per dose, mixed with rice water on an empty stomach; take another dose at noon.
For treating prolapse of the anus after dysentery, use Lang Dan San Formula:
Lang Dan Zi (Fructus Hyoscyami, stir-fried until yellow, half liang)
Bie Jia Tou (Carapax et Plastrum Testudinis, head part, burned to ash, 2 pieces)
Tie Jing (Ferrum, half liang, ground)
Crush and sieve the above three ingredients into powder. Take 2 qian bi per dose, mixed with rice water on an empty stomach; take another dose in the evening. Apply a small amount of the powder to the anus, and use a heated old hemp shoe sole to fumigate/iron the area, which will prevent further prolapse.
For treating pain from prolapsed anus due to chronic diarrhea/dysentery, use Hei Sheng San Formula:
Da Zhi Zhu (Araneus, wrapped in two layers of Benincasa hispida leaves, tied securely, and burned black inside a container without over-burning, 1 piece)
Finely grind this single ingredient, mix with a small amount of Dan Sha (Minium), and grind evenly. Before application, wash with a decoction of Bai Fan (Alumen), Cong Bai (Scallion), and Hua Jiao (Zanthoxylum), then dry. Sprinkle the medicine onto a soft cloth, support the prolapsed anus with the palm to prevent further descent.
For treating disharmony of cold and heat, lower body pain, tenesmus, loose stools or constipation/stagnation, and intestinal prolapse, use Nao Sha Wan Formula:
Nao Sha (Sal Ammoniac, purified, 1 liang)
Liu Huang (Sulfur, ground)
Bai Fan (Alumen, ground, each 1 fen)
Grind the above three ingredients uniformly. Take one radish weighing 5 liang, cut off the top to keep as a lid, hollow out a cavity, and fill it with the powder. Secure the lid with bamboo pins. Wrap the radish in 15 liang of dough. Dig a pit one chi square, line it with 3 cun of thick horse manure, place the dough-wrapped radish on top, cover with more horse manure, and ignite fire above. Stop when the dough turns black. Remove the dough, mash the radish contents into a paste. Add the following herbs: Rou Dou Kou (Nucis Arecae Semen Preparatum, shelled) Hu Lu Ba (Fenugreek Seed, half liang each) He Le Li Pi (Pericarpium Chebulae) Fu Zi (Radix Aconiti Lateralis Preparata, processed, skin and root removed) Bu Gu Zhi (Fructus Psoraleae, stir-fried, 1 liang each) Xiang Zi (Fructus Foeniculi, stir-fried, 1 fen).
Grind these six ingredients into powder, mix thoroughly with the radish paste, add a little good wine, and knead until uniform. Form pills the size of Sophora seeds. Take 15-20 pills per dose, before meals on an empty stomach, washed down with warm rice water.
For treating severe watery diarrhea leading to anal prolapse, use Zhu Gan San Formula:
Zhu Gan (Porcine Liver, sliced and slow-cooked until dry) Huang Lian (Rhizoma Coptidis, whiskers removed) E Jiao (Colla Corii Asini, stir-fried until dry) Chuan Xiong (Rhizoma Chuanxiong, 1 liang each) Wu Mei (Fructus Mume, pulp only, 2.5 liang, cooked) Ai Ye (Folium Artemisiae Argyi, stir-fried with vinegar, half liang)
Grind and sieve the above six ingredients into a powder. Take 2 qian bi per dose, mixed with warm wine or white rice water. Take twice daily before meals.
For treating prolapsed anus, use Bi Tu San Formula:
Gu Wu Dong Qiang Bi Shang Tu (Soil from the east wall of an old house, 5 he) Zao Jia (Fructus Gleditsiae Sinensis, 3 pods, each one chi long)
Grind the soil into fine powder and apply to the prolapsed anus. Heat the Gleditsia pods and apply warm compressions alternately until the anus retracts.
For treating non-retracting prolapse of the anus, use Pu Huang Fu Method:
Use Pu Huang (Pollinis Typhae) alone, mix with lard, apply to the anus, and press manually to insert. Apply 2-3 times daily until healed.
For treating cold accumulation causing dysentery and anal prolapse, use Zhi Shi Yu Method:
Take one Zhi Shi (Fructus Aurantii Immaturus), grind smooth on a stone, attach a handle, coat with honey, and warm by roasting. Use for warm compression on the anus. Replace when cool. Stop when the anus retracts.
