Commentary states: When Qi and Blood are deficient, the Couli (pores and interstitial spaces of the skin) become loose and sparse. Wind-Dampness pathogens invade between the Fenrou (muscle intervals). If this condition persists without healing, the True Qi dissipates while the Pathogenic Qi remains alone, resulting in the disease of Pianku (hemiplegia). The manifestations include limb motor impairment, where one side of the muscles is atrophic, small, and painful, yet speech remains unchanged and mental clarity is preserved; such cases are treatable. It is advisable to lie warm to induce sweating, thereby replenishing the deficiency and reducing the excess. Palpation of the Stomach pulse revealing a deep and large quality, or the Heart pulse revealing a small, firm, and rapid quality, are both pulses associated with Pianku.
Treatment for Zhong Pianfeng (Wind-stroke affecting half the body), chronic for years without healing, with withered and thin hands and feet, facial deviation, loss of mental focus, and disordered speech, use Gancao Tang (Licorice Decoction):
Gancao (Zingiberis Rhizoma Preparatum, prepared and crushed), Cizu (Aconiti Lateralis Radix Preparata, roasted, peel and umbilicus removed), Guizhi (Cinnamomi Ramulus, outer bark removed), Fangji (Stephaniae Tetrandrae Radix), Fuzi (Aconiti Lateralis Radix Preparata, roasted, peel and umbilicus removed), Xiong (Ligustici Chuanxiong Rhizoma), Renshen (Ginseng Radix et Rhizoma), Tufuling (Smilacis Glabrae Rhizoma, 3 fen), Fushen (Poria cum Radix Pini, heartwood removed), Xixin (Asari Radix et Rhizoma, seedlings and leaves removed, slightly stir-fried, 0.5 liang).
The above seventeen ingredients are crushed like hemp seeds. Each dose consists of 6 qianbi (approx. 18-24g depending on era/measure), boiled with 2 zhan (approx. 400ml) of water, adding 0.5 fen of sliced fresh ginger. Decoct until 1 zhan remains, filter dregs, add 0.5 he of Zhuli (Bambusae Caulis in Taeniam), boil again until boiling, and take warm. Take on an empty stomach, in the evening, or near night.
Treatment for Zhongfeng causing hemiplegia of hands and feet, painful facial deviation, and inability to close one eye, use Danggui Tang (Angelica Decoction):
Danggui (Angelicae Sinensis Radix, cut and baked), Baizhi (Angelicae Dahuricae Radix), Fangfeng (Saposhnikoviae Radix, forked parts removed), Baixianpi (Dictamni Cortex), Baizhu (Atractylodis Macrocephalae Rhizoma), Xiong (Ligustici Chuanxiong Rhizoma), Xingren (Armeniacae Semen Amarum, soaked in water, peel and tip removed, double-kernel stir-fried), Gancao (Licorice Root, prepared).
The above eleven ingredients are crushed like hemp seeds. Each dose consists of 5 qianbi, boiled with 2 zhan of water, adding 0.5 fen of sliced fresh ginger. Decoct until 1 zhan remains, filter dregs, and take warm. Take three times daily and once at night, alternating with meals.
Treatment for Zhongfeng Pianku with bone soreness and weakness, use Cishi Tang (Magnetite Decoction):
Cishi (Magnetitum, calcined red and quenched in vinegar seven times, 3 liang), Fangfeng (Saposhnikoviae Radix, forked parts removed, 3 liang), Wuweizi (Schisandrae Chinensis Fructus, 2 liang), Gancao (Licorice Root, prepared and crushed, 1 liang), Xuanshen (Scrophulariae Radix, 1 liang).
The above seven ingredients are crushed like hemp seeds. Use 5 qianbi per dose, boiled with 2 zhan of water and 35 black beans. Decoct until 1 zhan remains, filter dregs. Take on an empty stomach, at noon, midnight, or before lying down.
Treatment for Zhongfeng Pianku with limb immobility, slurred speech, and mental vagueness, use Jinbo Wan (Gold Foil Pill):
Jinbo (Folium Aurum, ground, 2 qian), Dansha (Cinnabar, ground, 1 liang), Ejiao (Colla Corii Asini, dried by heat, 2 liang), Dingxiang (Caryophylli Flos, 1 liang), Shexiang (Moschus, ground, 1 liang), Longnao (Borneolum Syntheticum, small amount).
The above ten ingredients: except those requiring separate grinding, all others are pounded and sifted into fine powder. Mix with the ground drugs, blend evenly, and bind with refined honey into pills the size of Sophora seeds. Take 2 pills per dose, chew thoroughly, and swallow with warm wine. This medicine also treats women's Blood-Wind disease, dizziness of head and eyes, and various chest/diaphragm disorders.
Treatment for Zhongfeng Pianku with Qi obstruction, where hands and feet cannot move, use Moyao Wan (Myrrh Pill):
Moyao (Myrrha, ground, 1 liang), Ruxiang (Olibanum, ground, 1 liang), Mahuang (Ephedrae Herba, roots and joints removed, 3 liang), Caowutou (Aconiti Kusnezoffii Radix, crushed, stir-fried until black but retaining nature, 1 liang), Ziran Tong (Chalcocitum), Baifuzi (Hydrastis Rhizoma), etc., nine ingredients in total.
The above nine ingredients: except those requiring separate grinding, all others are pounded and sifted into fine powder. Add the ground drugs and mix well. Bind with thick ink juice made from wine. Divide into ten blocks, each block further divided into twenty pills. Take one pill per dose, dissolved in warm wine. Take 3-5 times daily, at any time.
Treatment for Zhongfeng Pianku with immobility and drooling, use Sanling Dan (Three Spirits Elixir):
Dansha (Cinnabar, ground, 3 liang), Cihuang (Orpiment, ground, 1.5 liang), Liuhuang (Sulfur, ground, 0.5 liang).
The above three ingredients: first melt Cihuang and Liuhuang in a pot until liquid. Then add Dansha powder and stir evenly. Cool, then add Sangcha Hui Zhi (Ash juice from mulberry branches). Boil for three days and nights, adding warm ash juice as needed. When time is up, stop. Scrape into a ding vessel, dry using mild and strong fire until Yin Qi is exhausted. Seal in a container, and boil in water for one day to remove Fire Toxicity. Grind finely, mix with Jujube flesh, and form pills the size of mung beans. Take 3 pills on an empty stomach daily with cold pepper soup, gradually increasing to 5 pills. Healing occurs within half a month.
Treatment for Zhongfeng hemiplegia, paralysis, edema of head and face due to Hongxiangjiao (Gout attacking head/face), facial deviation, slurred speech with excessive saliva, mental vagueness, and wind-constipation, use Xijiao Jian (Rhinoceros Horn Decoction/Syrup):
Xijiao (Rhinocerotis Cornu, shaved and ground to fine powder, 2 liang), Weilingxian (Pedicularis Resupinatae Herba, 10 jin purple ones), Tianma (Gastrodiae Rhizoma, fine powder, 2 liang), Fuzi (Aconiti Lateralis Radix Preparata, peeled and umbilicus removed), Longnao (Borneolum Syntheticum), five ingredients in total.
First, boil Weilingxian in 1 shi (approx. 100 liters) of river water until reduced to 3 dou (approx. 30 liters). Filter through silk cloth to retain only clear juice. Add 1 dou of pure wine. Simmer in a silver or earthenware vessel until reduced to 1 dou. Clarify to remove fine sediment. Add Tianma and Fuzi powders to the medicinal juice. Simmer slowly over low heat until it becomes a paste. Allow to cool. Add Xijiao powder and Longnao powder, mix evenly. Store in a porcelain jar. For patients, dissolve 1 qianbi in Bohe Tang (Mint Decoction) and take.
