Those suffering from lower back and waist pain are invariably caused by deficiency of Kidney Qi (Kidney energy) combined with lying on cold, damp ground while exposed to wind. If not treated promptly, the pathogenic factors tend to flow into the feet and knees, leading to hemiplegia (partial paralysis), cold Bi syndrome (painful obstruction due to cold-dampness), weakness in the legs, heavy pain, or lower back pain accompanied by leg spasms, heaviness, and numbness. In such cases, this prescription should be taken immediately.
Radix Angelicae Pubescentis (Duhuo, 独活) — 3 liang; Radix Dipsaci (Sijisheng, 桑寄生) [Note: *Gujin Luyan* uses Radix Cistanchis (Xuduan, 续断)]; Cortex Eucommiae (Duzhong, 杜仲); Radix Achyranthis Bidentatae (Niuxi, 牛膝); Radix Asari (Xixin, 细辛); Ramulus Cnidii (Qinjiao, 秦艽); Poria (Fuling, 茯苓); Cortex Cinnamomi (Guixin, 桂心); Radix Saposhnikoviae (Fangfeng, 防风); Rhizoma Chuanxiong (Chuanxiong, 川芎); Radix Rehmanniae Preparata (Shengdihuang, 干地黄); Radix Ginseng (Renshen, 人参); Radix Glycyrrhizae (Gancao, 甘草); Radix Angelicae Sinensis (Danggui, 当归); Radix Paeoniae Alba (Shaoyao, 芍药) [each 2 liang].
The above fifteen ingredients are coarsely crushed. Decoct with one dou (approx. 10 liters) of water until three sheng (approx. 3 liters) remain. Divide into three doses. Keep the body warm after ingestion; avoid cold. For those with diarrhea due to wind-deficiency, remove Radix Rehmanniae Preparata. While taking the decoction, take leaves of *Akebia quinata* (Shuodie, 蒴藋), burn them to ash, spread the hot ashes on a thick mat, and lie upon it. Reheat when cool. In winter, use the roots; in spring, use the stems for steaming/lying upon, which is optimal. Other methods such as external application or moxibustion-ironing are inferior to the *Akebia* steam therapy. This method is also applicable for wind-dampness in various parts of the body. For women who, immediately after childbirth, suffer from abdominal pain preventing movement, or lower back and leg spasms/pain preventing flexion and extension, along with numbness and weakness, this decoction is suitable to dispel wind and resolve blood stasis. (*Zhouhou Beiji Fang* includes one piece of Radix Aconiti Lateralis Preparata (Fuzi, 附子), but lacks Sijisheng, Renshen, Gancao, and Danggui.)
This formula treats men and women suffering from lower back and waist pain due to wind-deficiency and cold, decreased appetite, emaciation, pallor, and shortness of breath. It dispels wind-cold and tonifies deficiencies.
Flos Chrysanthemi (Juhua, 菊花) and Cortex Eucommiae (Duzhong, 杜仲) [each 1 jin]; Radix Saposhnikoviae (Fangfeng, 防风); Radix Aconiti Lateralis Preparata (Fuzi, 附子); Radix Astragali (Huangqi, 黄芪); Rhizoma Zingiberis Preparatum (Ganjiang, 干姜); Cortex Cinnamomi (Guixin, 桂心); Radix Angelicae Sinensis (Danggui, 当归); Radix Dendrobii (Shihu, 石斛) [each 4 liang]; Semen Lazulis (Zhi Shiyin, 紫石英).
The above fifteen ingredients are coarsely crushed. Soak in seven dou of wine for five days. Take two he (approx. 40 ml) per dose, gradually increasing to five he (approx. 100 ml). Three times daily. (*Qianjin Yifang* omits Rhizoma Zingiberis Preparatum.)
This formula treats pain and immobility of the lower back and legs due to wind-deficiency.
Cortex Eucommiae (Duzhong, 杜仲) — 8 liang; Herba Pyrolae (Shinan, 石南); Radix Notopterygii (Qianghuo, 羌活); Radix Aconiti Lateralis Preparata Magna (Da Fuzi, 大附子) — 5 pieces.
The above four ingredients are coarsely crushed. Soak in one dou of wine for three nights. Take two he per dose, twice daily. Particularly suitable for women suffering from cold conditions.
(Two discussions, nine formulas, one moxibustion method)
Discussion: Fengfei (Wind Paralysis) refers to sudden inability to speak, locked jaw, and stiff, immobile limbs. The treatment involves using Fulonggan (Caosinis Oxycocci, 伏龙肝 - kiln heart soil) five sheng, powdered, mixed with eight sheng of cold water, stirred, and the juice extracted and drunk. Complete consumption yields the best results. (*Zhouhou Beiji Fang* notes this formula treats mental restlessness, confusion, abdominal pain and fullness, used for resuscitation.) The following nine formulas all treat this type of wind disease; their sequence of application should be carefully considered.
Discussion: To treat this disease effectively, one must understand the correct sequence of treatment. One must not indiscriminately administer decoctions, lest opportunities be missed. This negligence can lead to death or result in chronic, intractable diseases. Once the disease has manifested, one should first administer Zhuli (Bambusae Succus Preparatus, 竹沥 - prepared bamboo sap) in small amounts, which is superior to normal intake, followed by decoctions. For patients with Wind-Heat syndromes, Zhuli drink should be used first to control heat toxicity.
