Zhang Zhongjing, in the *Shanghan Lun* (Treatise on Cold Pathogenic Diseases), records: "Heart pain radiating to the back, back pain radiating to the heart." The treatment is governed by Wutou Chishizhi Wan (Aconite and Red Halloysite Pill).
Ingredients:
Wutou (Fried Aconiti Lateralis Radix Praeparata, 2 fen, processed, skin removed) Fuzi (Processed Aconiti Lateralis Radix Praeparata, 1 fen, processed, skin removed) Chishizhi (Halloysitum Rubrum, 2 fen) Ganjiang (Dried Zingiberis Rhizoma, 2 fen) Shujiao (Fructus Zanthoxyli, 1 fen, perspiration removed)
The above five ingredients are crushed, sieved, and mixed with honey to form pills. Taken before meals, sized like hemp seeds, starting with three pills per dose, gradually increasing the amount. Avoid pork and cold water. (Recorded identically in *Qianjin Fang*, *Bixiao Fang*, works of Wen Zhong, *Fan Wang Fang*, and *Jingxin Lu*, originating from Volume 15; slight differences in dosage exist in *Qianjin Fang*)
Zhang Wenzhong's Shujiao Wan, a formula for treating chest fullness of qi, and heart pain radiating to the back.
Ingredients:
Shujiao (Fructus Zanthoxyli, 1 sheng, perspiration removed) Banxia (Pinelliae Rhizoma, 1 sheng, washed) Fuzi (Processed Aconiti Lateralis Radix Praeparata, 1 liang, processed)
The above three ingredients are crushed, sieved, and mixed with honey to form pills, sized like sophora seeds. Take five pills per dose, three times daily. Avoid pork, mutton, maltose, etc. (Originating from Volume 3)
*Fan Wang Fang* treats severe pain below the heart radiating to the back, accumulation of qi below the chest, and retained food in the stomach, using the Zhuyu Decoction.
Ingredients:
Wuzhuyu (Fructus Evodiae, 1 sheng) Shujiao (Fructus Zanthoxyli, 5 sheng) Gancao (Licorice Root, 2 liang, honey-fried) Dihuang (Prepared Rehmanniae Radix, 1 jin)
The above four ingredients are soaked in three sheng of clear wine for three nights, then pressed to extract the juice. Boil in a copper vessel until boiling. Add Maidong (Ophiopogonis Radix, 5 sheng, core removed) and Qianxi (Drymopteridis Rhizoma, 1 jin). When the color turns yellow, strain off the dregs. Then add Shihu (Dendrobii Caulis, 5 liang), Ejiao (Donkey-hide Gelatin, 1 jin), and Baimi (White Honey, 6 sheng). These nine ingredients are decocted with water until suitable for pill formation. Form pills sized like jujubes. Hold in the mouth and swallow slowly, three times daily. For severe cases, take five to six times daily. If there is heaviness and pain in the knees and shins, add Shihu; if there is shortness of breath, add Maidong. After five days of medication, the disease will be cured, and pathogenic factors should be purged. Avoid seaweed, Chinese cabbage, and Xuyi. (Tested successfully by Chen Gai, Attendant-in-Waiting for Chariots)
Another formula, Yuanhua Tang, governs sudden heart pain connecting to the back, back pain radiating to the heart, simultaneous distress and pain in the heart and abdomen, feeling as if pierced by a ghost, with strangling urgency threatening death.
Ingredients:
Yuanhua (Genkwa Flos, 10 fen) Dahuang (Rhei Radix et Rhizoma, 10 fen)
The above two ingredients are crushed and sieved. Take one square-cun-bi (a specific measuring spoon) of powder, place it in two and a half sheng of vinegar, and decoct together until one sheng and two he remain. Take all at once. Vomiting should occur shortly after; recovery follows vomiting. Start with smaller doses for the elderly and children. This formula is excellent for treating robust excess patterns; however, for deficiency-cold heart pain, it may not be suitable. (Both formulas originate from Volume 18)
