When treating sores and ulcers, medication should be administered according to the meridians. It is essential to understand the abundance or scarcity of Qi and Blood in each meridian: sores on meridians with abundant Qi and Blood are easier to heal, whereas those on meridians with scarce Qi and Blood are difficult to cure. Physicians specializing in surgery must comprehend this fundamental principle to avoid committing errors that lead to deterioration or adverse conditions (Ni Zheng), which are contraindicated.
For instance, the Hand Shaoyang Sanjiao Meridian, Hand Shaoyin Heart Meridian, Hand Taiyin Lung Meridian, Foot Shaoyang Gallbladder Meridian, Foot Shaoyin Kidney Meridian, and Foot Taiyin Spleen Meridian all possess abundant Qi but scarce Blood. Sores occurring in these six meridians are the most difficult to close and heal.
Conversely, the Hand Jueyin Pericardium Meridian, Hand Taiyang Small Intestine Meridian, Foot Taiyang Bladder Meridian, and Foot Jueyin Liver Meridian all possess abundant Blood but scarce Qi. For sores in these four meridians, treatment should preferably employ the "Tuo Li" method (supporting the interior and expelling toxins).
The Hand Yangming Large Intestine Meridian and Foot Yangming Stomach Meridian both have abundant Qi and Blood. Initially, it is suitable to use the "Nei Xiao" method (internal resolution) to dissipate the sore internally, making final healing relatively easy. Therefore, these distinctions are outlined; one cannot apply a uniform treatment approach indiscriminately.
The Heaven and Earth possess six types of pathogenic influences (Liu Yin): Wind, Cold, Summerheat, Dampness, Dryness, and Fire. When the human body contracts these pathogens, Ying Qi (Nutritive Qi) fails to flow smoothly and becomes reversed within the muscle tissue, leading to the generation of carbuncles, sores, boils, and furuncles. The pulse at the Renying acupoint will manifest as Floating, Large, Surging, and Rapid. With swelling visible externally, it is feared that the pathogenic qi is extreme and spreading; treatment should favor supporting the interior. The classics state, "Sweating will cure the sore." This aims to facilitate the circulation of Ying and Wei (Defensive) Qi, preventing pathogens from penetrating inward.
Humans have seven emotions: Joy, Anger, Worry, Thought, Fright, Fear, and Sorrow. Excessive indulgence in any of these can disrupt Zang-Fu organ harmony, causing Ying Qi to stagnate and reverse in the muscle tissue, resulting in carbuncles. These external manifestations lack redness, but the pain is deep-seated internally. The pulse is Deep, Solid, and Rapid, accompanied by fever and irritability. Treatment must focus on unblocking the Zang-Fu organs to eliminate the source.
Regarding causes that are neither internal nor external, such as individuals consuming excessive rich and greasy foods, inherent constitutional biases, or excessive labor and rest imbalances, these factors cause body fluids to become viscous and phlegm-saliva to obstruct the meridians. In such cases, there are no obvious external symptoms, nor internal obstruction of urination or defecation; diet remains normal, and excretions are regular. The pulse is typically Slippery, Rapid, Solid, and Large. Knowing the pathogen resides in the meridians, one should harmonize Ying and Wei Qi and regulate the meridians to restore normalcy. If these three therapeutic methods are applied appropriately, although rapid recovery may not occur, the pathogenic influence will be significantly reduced, and the sore will not deteriorate.
