Chong Ju (Puncture Abscess) arises two cun above the umbilicus, caused by blazing Heart Fire toxin invading the Kidney meridian, resulting in redness. If the swelling is high and prominent, pain is severe, and it suppurates rapidly, it is auspicious; if the "Seven Evils" syndromes appear, it is definitely ominous.
[Note] This condition occurs on the Ren Mai (Conception Vessel), specifically at the Xiawan (CV10) acupoint, two cun above the umbilicus. It is also known as Zhongfa Ju (Middle Eruption Abscess) or Yong Shen Chuang (Congested Kidney Sore). It results from excessive Heart Fire flowing into the Kidney meridian. The presentation features redness and high swelling; it should suppurate around the twenty-first day. If the pus is thick and the patient can tolerate tonifying treatments, it is a favorable course. Initially, use Chuang Ke Liu Qi Yin (Wound Department Flowing Qi Decoction) or Xian Fang Huo Ming Yin (Immortal Life-Protecting Decoction) to dissipate it. When pus is about to form, internal and external treatment methods follow those outlined in the chapters on Sores and Ulcers of Carbuncles and Boils. If the lesion is flat, purple-black, with thin pus, and accompanied by the Seven Evils syndromes, it is an unfavorable course.
(Attachment: Oozing from the Umbilicus)
Qi Yong (Umbilical Carbuncle) develops in the center of the umbilicus, swelling large like a melon and protruding like a bell, without redness or heat. It is suitable for Garlic-Separated Moxibustion; thick pus indicates a good prognosis, while watery pus indicates a poor prognosis.
[Note] This condition is caused by Heart meridian fire-toxin flowing into the Large Intestine and Small Intestine. It arises in the umbilicus, belonging to the Shenque (CV8) acupoint of the Ren Mai (Conception Vessel); this acupoint is contraindicated for needle insertion. The swelling is large like a melon, protruding high like a bell, without redness or heat. It is most suitable for Garlic-Separated Moxibustion. Initially, take Xian Fang Huo Ming Yin (Immortal Life-Protecting Decoction) added with Sheng Ma (Cimicifugae Rhizoma) to dissipate it; if stool is dry and hard, take Nei Shu Huang Lian Tang (Internal Draining Coptis Decoction) to promote unblocking. When pus is about to form, internal and external treatments follow the chapters on Sores and Ulcers of Carbuncles and Boils. After ulceration, the emergence of thick pus is favorable, while frequent discharge of foul-smelling watery pus is unfavorable. There are cases where the umbilicus is neither painful nor swollen but extremely itchy, with yellow fluid oozing intermittently; this belongs to long-standing damp-heat accumulation in the Stomach and Intestines. One should take Huang Lian Ping Wei San (Coptis Harmonize the Stomach Powder) and apply San Miao San (Three Marvels Powder) externally as a dry dusting to drain dampness, which will lead to recovery. Alcohol, wheat products, raw/cold foods, and fruits/vegetables must be avoided to prevent recurrence. If water flows out incessantly, it is also considered unfavorable.
