In the initial stage of a Yang syndrome, the affected area is red and painful. The base is confined with clear boundaries, and the swelling is high and arched like a bow. After seven days, pain may fluctuate between severe and mild; by fourteen days (two sevens), pus gradually forms within the lesion. Pain decreases as pus drains, and the spirit becomes refreshed. As necrotic tissue falls off and new tissue grows, Qi and Blood are replenished. The tender new flesh resembles pearls in shape and beauty, appearing fresh and moist, with a color vibrant like pomegranate seeds. If the "Seven Evils" are naturally avoided, the "Five Goods" will likely be encountered. It must be known that this belongs to a pure Yang syndrome; with appropriate medical regulation, efficacy will naturally follow.
[Note] For any carbuncle or ulcer (Yongju) in its initial stage, if it presents with heat, redness, pain, and a confined base, it indicates that the pathogenic influence has not spread (the halo does not disperse); clear boundaries indicate no diffuse swelling; swelling high like a bow indicates prominent elevation. All these belong to Yang syndromes. Therefore, ulceration with pus discharge, shedding of necrotic tissue, regeneration of new tissue, and wound contraction are all easy to achieve.
In the initial stage of a Yin syndrome, the lesion is small like a millet grain, without redness or swelling, presenting as a stiff nodule. The skin is numb and hard, without pain or heat. The base of the sore is flat, wide, dark, and lusterless. After seven days, it fails to ulcerate or shed necrotic tissue; instead, it becomes sunken and soft with no pus formation, resembling an empty granary. Tissue growing on the sore surface resembles detached nails, and nodules forming in the pores look like flowers containing seeds. If purple-black, scanty, and foul-smelling pus appears, and the "Seven Evils" are observed, death is certain. It must be known that this belongs to a pure Yin syndrome; even with the medical skills of Qibo or Huangdi, life expectancy will not be long.
[Note] For any carbuncle or ulcer in its initial stage, if it presents as a millet-grain-sized nodule without redness, swelling, heat, numbness, hardness, or pain, with a diffuse base, dark and lusterless color, it belongs to a Yin syndrome. Therefore, failure to ulcerate, formation of an empty granary without pus, non-shedding of regenerating tissue resembling nail plates, formation of nodules in pores like flowers containing seeds, and purple-black, thin, foul-smelling pus all indicate difficulty in healing.
When Yin and Yang are mixed, it constitutes a dangerous syndrome. Yang syndromes portend good outcomes, while Yin syndromes portend bad outcomes, making life and death distinct. Symptoms resemble Yang but with mild pain and mild swelling; they resemble Yin but with half-hard and half-elevated swelling. Swelling without ulceration is due to Spleen deficiency; ulceration without contraction is due to excessive pus (or insufficient Zheng Qi to close the wound). Although the "Five Goods" may be present, the "Seven Evils" cannot be entirely escaped. If the patient can eat normally and enjoy food, and urination and defecation are harmonious, treatment is feasible. Following proper methods for treatment yields prompt effectiveness; as Yang increases and Yin decreases, recovery can be achieved.
[Note] For any carbuncle or ulcer, if symptoms resemble Yang but with insignificant heat, pain, and swelling, or resemble Yin but with insignificant numbness, hardness, flatness, and depression, it belongs to a semi-Yin, semi-Yang dangerous syndrome. If Good signs gradually appear, survival is expected; if Evil signs gradually appear, death ensues.
