Hemorrhoids present in various forms and names, their etiology lies not beyond Wind, Dampness, Dryness, and Heat. They can occur both inside and outside the anus; if ulcerated for a long time and forming fistulas (chronic infected tracts around the anus), they are difficult to cure.
[Note] This condition refers to sores growing in the anal region, some inside and some outside. Those that form masses without breaking are called hemorrhoids and are easy to treat; those that break open, discharging pus and blood, with yellow fluid oozing continuously, are called fistulas and are difficult to cure. The names arise from their shapes, totaling twenty-four types, all stemming from overindulgence in alcohol and food or sexual activity leading to flaccidity of vessels and loss of essence, allowing Heat-Toxin to sink downward due to deficiency; or from excessive worry and thought causing accumulation of Heat-Toxin, combined with frustrated Qi, resulting in the combination of Wind, Dampness, Dryness, and Heat. If nodules are swollen, stifling, and painful, it indicates excess Dampness; if nodules are swollen, painful like burning, with constipation and urinary retention, it indicates excess Heat in the Large and Small Intestines; if nodules itch significantly, it indicates excess Wind; if the anal folds rupture with dry stools, it indicates Fire and Dryness. Initially, all cases should take Zhitong Rushen Tang (Pain-Relieving Divine Decoction) to resolve the condition, and externally apply Puti Lu (Bodhi Dew) or Snail Water. For hard nodules, use Wubeizi San (Five-Gallnut Powder), mixed with saliva and applied, along with washing using a decoction of Piao Xiao (Mirabilite) and green onion heads. For hemorrhoids with a large head and small stalk, tie a medicinal thread around the base, tightening it daily until the hemorrhoid withers and falls off, then sprinkle Yuebai Zhenzhu San (Moon-White Pearl Powder) to heal the wound; for those with a small head and large stalk, use Kuzhi San (Hemorrhoid-Withering Powder). For internal hemorrhoids that do not prolapse, insert Huanzhi San (Hemorrhoid-Calling Powder) into the anus to induce prolapse, then wash with Piao Xiao and green onion head decoction. For hemorrhoids caused by hard labor and carrying heavy loads, disrupting Qi and Blood, use modified Zhitong Rushen Tang. For "Blood-Arrow Hemorrhoids" occurring inside or outside the anus, causing blockage, swelling, and sudden gushing of bright red blood during defecation effort regardless of timing, caused by Wind-Heat in the intestines/stomach combined with sudden anger, initially administer Shengshu Sanhuang Wan (Raw and Cooked Three-Yellows Pill). If lips are pale, complexion sallow, and limbs weak, indicating Qi and Blood deficiency, use Shiquan Dabu Tang (Ten Complete Tonifying Decoction) with increased Chuanxiong (Rhizoma Chuanxiong) and Renshen (Radix Ginseng); externally wash with one's own urine, preferably warm child's urine, to stop bleeding. For Wind-Fire bleeding dripping before stool, use Fangfeng Qinjiao Tang (Saposhnikovia and Gentian Decoction); for bleeding after stool due to alcohol toxicity, use Kushen Dihuang Wan (Sophora and Rehmannia Pill). After efficacy, take Zanglian Wan (Intestine-Linked Pill) for two to three courses to eradicate the root. For postpartum hemorrhoids due to excessive exertion, use Buzhong Yiqi Tang (Middle-Joint Replenishing Qi Decoction) with Taoren (Semen Persicae), Honghua (Flos Carthami), and Sumu (Lignum Sappan). For hemorrhoids from chronic diarrhea or dysentery, use Buzhong Yiqi Tang with Huaihua (Flos Sophorae) and Zhaojiaozi (Semen Gleditsiae Sinensis) powder. For hemorrhoids penetrating the intestine with discharge from fistula openings, use Hulian Zhuidu Wan (Picrorhiza Pursuing Toxin Pill) taken with wine; increased pus/water after taking indicates drug action reaching the lesion, do not fear. For fistulas with tubes, use Huanglian Guan Guan Wan (Coptis Closing Tube Pill), replacing the need for needles/knives or threads. For all hemorrhoids, broken or unbroken, and fistulas, wash with Quedu Tang (Toxin-Expelling Decoction) or decoction of Labaohua (Mandragora officinalis, i.e., land yellow herb seedling), twice daily. Abstain from sexual activity, pufferfish, seafood, spicy foods, and pepper wine. Prognosis is poor for long-term hemorrhoid patients developing cough; conversely, chronic cough patients developing hemorrhoids often prove fatal.
