Dumen Carbuncle (Carbuncle of the Popliteal Fossa/Thigh Muscle) occurs in the muscle belly of the thigh, while Jimen Carbuncle (Carbuncle of the Jimen Acupoint area) occurs on the medial aspect of the thigh near the knee. Both conditions present with redness, swelling, heat, and pain, resulting from damp-heat accumulation in the Bladder Meridian and Spleen Meridian.
[Note] These two conditions are both caused by the coagulation of damp-heat. Dumen Carbuncle arises in the fleshy part of the thigh (gastrocnemius region) and belongs to the Foot-Taiyang Bladder Meridian; Jimen Carbuncle arises on the medial thigh near the knee and belongs to the Foot-Taiyin Spleen Meridian. In the initial stage, presenting with redness, swelling, heat, and pain, Shen Shou Wei Sheng Tang (Divine Grant Health Preservation Decoction) is appropriate. If there is severe swelling accompanied by constipation, irritability, a desire for cold drinks, and a rapid pulse, it indicates excessive heat rampant within; Neishu Huanglian Tang (Internal Draining Coptis Decoction) or Shuangjie Guijin Wan (Double Resolving Precious Gold Pill) should be used to purge the heat downwards. If swelling and pain are accompanied by alternating chills and fever, with a deep and weak pulse, chest and abdominal fullness, but normal appetite, Bingsu San (Areca and Perilla Powder) is appropriate. Once the swelling, pain, chills, and fever have subsided, switch to Xiaoyao San (Free and Easy Wanderer Powder). If the swelling and pain persist without color change, accompanied by chills/fever, poor appetite, and fatigue, this is due to liver deficiency causing damp-phlegm to sink downward; Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Fuling (Poria), Banxia (Pinellia Rhizome), and Shaoyao (Peony Root) is appropriate. If sexual intercourse during the illness leads to hard swelling and obstruction of urination and defecation, Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) added with Niuxi (Achyranthes Root) and Cheqianzi (Plantago Seed) is appropriate; once urination and defecation become unobstructed, resume taking Buzhong Yiqi Tang. For other treatments, refer to the sections on Carbuncles and Ulcers.
