Fan Hua Chuang (Flower-like Ulcer) is caused by liver fire and dry blood generating wind after the ulceration of a sore. In some cases, flesh protrudes from the wound opening like a mushroom, varying in size; or it resembles a snake's head; or even if it heals, it ulcerates again. If mistakenly treated with knives, needles, caustic medicines, or moxibustion for removal, the condition will worsen, potentially leading to uncontrolled bleeding and symptoms such as alternating chills and fever, and vomiting. The best approach is to strongly tonify the Spleen and Stomach.
Case Records:
Zhang Cheng'en, a Judge, had an abscess on his inner thigh that was nearly healed when a mushroom-like piece of flesh protruded. I stated, "This belongs to Wind-Fire and Blood Dryness in the Liver Meridian; one should clear Liver Heat and nourish Liver Blood." He subsequently took Zhi Zi Qing Gan San (Gardenia Decoction to Clear the Liver) internally and applied Li Lu Gao (Veratrum Ointment) externally, resulting in a cure.
A student at the Imperial Academy, accustomed to a rich diet and prone to anger, developed a nodule below his ear. After it ulcerated, the wound opened and flared out like a mushroom, accompanied by headache, chest and hypochondriac distension, internal heat, or alternating chills and fever. Previous treatment with heat-clearing and detoxifying herbs failed to cure him after more than a year. I used Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) and Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia), and he recovered quickly.
A man had a back sore that contracted to the size of a bean but then produced a piece of flesh over an inch long. Using methods of erosion, cutting, and striking, the flesh was removed repeatedly but grew back larger each time. This indicated Blood Deficiency and Wind-Heat in the Liver Meridian. I prescribed Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) for over thirty doses and applied Veratrum Ointment until the growth subsided; subsequently, Ba Zhen San (Eight-Treasure Powder) with increased amounts of Ren Shen (Ginseng Root), Bai Zhu (Atractylodes Rhizome), and Dang Gui (Angelica Sinensis Root) was used to promote healing.
A woman who was prone to anger developed an arm abscess with two inches of flesh protruding from the wound. This was due to Liver and Spleen depression and anger, causing Qi and Blood deficiency and internal wind movement. She was cured using Jia Wei Xiao Yao San and Ba Zhen Tang (Eight-Treasures Decoction).
A man suffered from neck swelling with excessive phlegm and saliva. Treatment with formulas for dissipating hardness and moving Qi made the swelling and hardness worse, leading to panting, fever, spontaneous sweating, night sweats, fatigue, and poor appetite. I said, "This belongs to deficiency of the Foot Three Yin meridians; one should nourish the source of transformation." The patient did not believe this and instead pursued corrosive treatments, causing the affected area to open up about six inches, turning red like rock, with fresh blood flowing daily for over three months. The Liver pulse was wiry, surging, tight, and solid. I used Da Bu Tang (Great Tonification Decoction) plus Mai Men Dong (Ophiopogon Tuber) and Wu Wei Zi (Schisandra Fruit). After over fifty doses, all symptoms gradually improved, and bleeding stopped for three or four times. Later, due to anger and a sudden decrease in food intake, blood gushed out again. This indicated that the injured Liver could no longer store blood, and the injured Lung could no longer control it. I primarily used Bu Zhong Yi Qi Tang, adding Wu Wei Zi and Mai Men Dong, which immediately stopped the bleeding; then, Liu Wei Wan (Six-Ingredient Pill) with added Wu Wei Zi was taken regularly, causing the wound to contract to about one inch. Thereafter, no medicine was used, and he died because he did not observe dietary and lifestyle restrictions.
