(Attached: Sores and Tinea)
Discussion on Carbuncles, Furuncles, and Back Abscesses: For various red, swollen, hot, and toxic conditions, regardless of whether they occur on the limbs, hands, feet, head, or face, at the initial stage, one may use Loulu Tang (Ludwigia Decoction), Sanxian San (Three Immortals Powder), Tuoli San (Supporting the Interior Powder), Bao'an Gao (Security Ointment), Ruxiang Neixiao Gao (Frankincense Internal Resolution Ointment), Daochi Wan (Guiding Fire Out Pill), Sishun Yin (Four Smoothness Drink), and Tongqi San (Promoting Qi Circulation Powder). If a carbuncle or furuncle erupts accompanied by constipation, one may use Maren Wan (Hemp Seed Pill) or Sanhuang Wan (Three Yellows Pill); for those with robust constitutions, Shengong Wan (Divine Merit Pill) may be used. It is necessary to promote bowel movements three to five times to ensure the dissipation of toxic pathogens. If the abscess has already ruptured and pus and blood discharge are sluggish, one may apply Sanxian San, Loulu Tang, Tuoli San, Bao'an Gao, or Ruxiang Neixiao Gao externally. For sores that fail to heal over a long period, Taoren San (Peach Kernel Powder) or Qilin San (Qilin Powder) may be used.
Discussion on Malignant Sores, Scabies, and Tinea: For all unnamed malignant sores, fistulous sores, yin sores, and cold sores that have persisted for years without healing, one may apply Taoren San, Qilin San, or Bao'an Gao externally. For wind-toxic heat sores, also known as Kidney Wind, characterized by purulent exudate, blistering, wet rot, and spreading infiltration, one may use Heshouwu San (Polygonum Multiflorum Powder), Sisheng Wan (Four Generations Pill), Huma San (Sesame Powder), or Sishun Yin. For cases with severe constipation, use Zaojiao Jian Wan (Soap Pod Decoction Pill) and Taoren San, and apply Huaji San (Slippery Skin Powder) topically. For widespread itching sores or urticaria (wind wheals), first administer bowel-relieving medications such as Zaojiao Jian Wan, Shengong Wan, Maren Wan, Heshouwu San, Sishun Yin, Sisheng Wan, Xiaofeng San (Eliminating Wind Powder), Huma San, Huaji San, Huangbo Wan (Phellodendron Pill), or Bailong Wan (White Dragon Pill). For dry and painful sores, use Yulong Mofeng Gao (Jade Dragon Wind-Massaging Ointment). For burns and scalds resulting in sores, use Foshou San (Buddha's Hand Powder). For scabies and stubborn tinea, use Mofeng Gao (Wind-Massaging Ointment), Bailong Wan, Huaji San, or Qingxin Wan (Clear Heart Pill).
Discussion on Menstrual Disorders: These often arise from irregular menstruation, occurring either early or late, in excessive or insufficient amounts, persisting as dripping leakage, or being completely obstructed. Symptoms include limb fatigue, weakness, and loss of appetite. For those who frequently take tonics, Siwu Tang (Four Substances Decoction), Shugan Dihuang Wan (Prepared Rehmannia Pill), Neibu Danggui Wan (Internal Supplementing Angelica Pill), Hupo Wan (Amber Pill), Danggui Jianzhong Tang (Angelica Decoction to Tonify the Middle), Chenxiang Biejia San (Aquarium Shell with Agarwood Powder), Huoxue Dan (Blood-Activating Elixir), Zeluan Wan (Marsilea Pill), or Yiyin Dan (Nourishing Yin Elixir) may be taken. For a cold and deficient Blood Sea, Nuangong Wan (Warming the Palace Pill), Huixiang Wan (Fennel Pill), Dasheng San (Great Sage Powder), Xiaobaiwei Wan (Little White Anemone Pill), Heixi Dan (Black Tin Elixir), Shexiang Lurong Wan (Musk and Deer Antler Pill), or Neibu Danggui Wan may be used. For excessive menstrual bleeding and persistent dripping, Jiao'ai Tang (Collagen and Mugwort Decoction), Wenjing Tang (Warming the Meridians Decoction), Xiaobaiwei Wan, Wujin San (Black Gold Powder), Yiyin Dan, Zeluan Wan, Neibu Danggui Wan, or Shugan Dihuang Wan may be administered. For leukorrhea (white discharge) mixed with blood that does not respond to various treatments, Zeluan Wan may be taken, combined with Pingbu Zhenxin Dan (Balanced Tonification and Calming Heart Elixir), Su Xiao Dasheng San (Rapid Action Great Sage Powder), Wenjing Tang, Zeluan San, Xiaobaiwei Wan, or Zhiyanshi Wan (Purple Quartz Pill). For persistent white discharge accompanied by frequent cold pain in the abdomen, Nuangong Wan, Shexiang Lurong Wan, Falian Heixi Dan (Refined Black Tin Elixir), Huixiang Wan, Weixi Wan (Vitality Joy Pill), Zeluan Wan, Zhengyuan San (Upright Origin Powder), or Laifu Dan (Returning Life Elixir) may be used. For multicolored leukorrhea, Yiyin Dan, Zixue Tang (Nourishing Blood Decoction), or Fulonggan San (Dragon's House Earth Powder) may be given. For metrorrhagia with excessive bleeding leading to dizziness and vertigo where the patient feels like falling, Xiongdi Tang (Peony and Ligusticum Decoction), Jiao'ai Tang, Wujin San, Hupo Wan, Nuangong Wan, Dasheng San, or Neibu Danggui Wan may be used. For uterine hemorrhage with stagnant blood persisting for days, Zixue Tang is indicated. For chronic deficiency of qi and blood with cold constitution, presenting as metrorrhagia with indeterminate colors or bean-soup-like discharge, Wenjing Tang, Fulonggan San, or Siwu Tang may be used. For amenorrhea or primary amenorrhea in unmarried women, Puhuang San (Cattail Pollen Powder), Xiaoyao San (Free and Easy Wanderer Powder), Dasheng San, Heishen San (Black Deity Powder), Hupo Zeluan Jian (Amber and Marsilea Decoction), Tongzhen Wan (Reaching Truth Pill), Huoxue Dan, Siwu Tang, or Dihuang Wan (Rehmannia Pill) may be prescribed. For blood-wind consumptive disease with chest oppression, meridian stagnation, limb irritability and pain, and palpitations, Ren Shen Jingjie San (Ginseng and Schizonepeta Powder), Shugan Dihuang Wan, or Xiaoyao San may be used. For dizziness due to blood-wind and qi deficiency, including pre- and postpartum dizziness, Baosheng Wan (Protecting Life Pill), Siwu Tang, or Jiao'ai Tang may be used. For consumptive fever or alternating chills and fever, Huangqi Biejia San (Astragalus and Turtle Shell Powder), Xiaoyao San, Dihuang Wan, Zeluan Wan, Jingjie San, Jiahe San (Harmonious Grain Powder), or Shenling Baizhu San (Ginseng, Atractylodes, and Poria Powder) may be taken. If heat persists, Ren Shen Danggui San (Ginseng and Angelica Powder) is excellent. For those with strong constitutions, Xiaoyao San may be used. For cough with phlegm or hemoptysis, Ren Shen Yangpi Wan (Ginseng Spleen-Nourishing Pill) should be taken before bed. For young women with strong constitutions experiencing bone-steaming fever, Shugan Dihuang Wan should be taken before meals, and Ren Shen Huangqi San (Ginseng and Astragalus Powder) after meals. For hernia masses, cold pain in the yin region, headache penetrating to the brain, cold bi syndrome with tendon contraction, blood obstruction causing infertility, or leaking red and white discharge, Shugan Dihuang Wan, Zhongru Zeluan Wan (Stalagmites and Marsilea Pill), or Mudan Jian Wan (Tree Peony Decoction Pill) may be used. For women unable to conceive, or those with deficiency patterns featuring upper heat and lower cold with myriad ailments, Xiaobaiwei Wan, Yuyuliang Wan (Halloysite Pill), or Yangqishi Wan (Yangqi Stone Pill) may be administered. For prior cold sensations, stabbing heart and abdominal pain, swelling or coldness, persistent dysentery, and shortness of breath, Wuzhuyu Tang (Evodia Decoction) or Huoxue Tang (Blood-Activating Decoction) may be used. For blood-wind invasion causing five-palm heat, whole-body itching, urticaria, or red swelling, Ren Shen Jingjie San, Ren Shen Danggui San, Xiaofeng San, or Xiaoyao San decocted with Siwu Tang plus Jingjie may be used; for severe cases, Zaojiao Jian Wan may be taken. For qi and blood stagnation, Qingmuxiang Wan (Indigofera Root Pill) may be taken to unblock and guide.
Discussion on Contraindications for Medications During Pregnancy: The medicinal substances contraindicated during pregnancy for damaging fetal qi are summarized in a song from Lu Yi's "Zhou Ding Ji": Crab, Leech, Di Dan Chong (Ground Beetle), Aconitum (Wutou), Prepared Aconite (Fuzi), and Wild Aconite (Tianxiong). Daphne (Zhizhu), Wild Knapweed (Yege), and Mole Cricket types, Aconitum Carmichaelii Lateral Root (Wuhui), Side-root Aconite (Cezhi), and Horsefly (Mengchong). Musk Ox Gallstone (Niuhuang), Mercury, and Croton Seed (Badou). Giant Spurge (Daji), Snake Slough, and Centipede (Wugong). Achyranthes Root (Niuxi), Veratrum (Lilu), and Coix Seed (Yiyi). Metal stones, Tin powder, paired with male and female counterparts. Glauber Salt (Yapo), Mirabilite (Mangxiao), and Tree Peony Decoction. Gecko, Flying Gecko, and Ground Beetle (Shichong). Ochre (Daizhe), Grasshopper, Cicada, Camphor, and Musk. Genkwa Flower, Weilingxian, and Grass-based Sanleng. Sophora Seed, Pharbitis Seed (Qianniu), and Soap Pod. Peach kernel, Larva of Silkworm Moth (Qicao), and Imperata Rhizome (Maogen). Ranunculus (Raohua), Nuo Sha, and Dried Lacquer. Tingchang, Zhouliu, and Arum (Mangcao). Buckwheat, Coriander, Crab Claw Shell. Fish skin, Arrow Wood (Guijian), and Red-headed varieties. Knife-shell, Stone Silkworm, and Silkworm moth, along with Pinellia (Banxia), Star Aconite (Nanxing), and Tetrapanax (Tongcao). Dried Ginger, Garlic, Chicken, and Duck. Donkey, Horse, and Rabbit meat need not be provided. Strictly avoid these for pregnant women; keep this in mind at this time.
Discussion on Prenatal Diseases: During the third and second months of pregnancy, if there is daily mental restlessness, heavy dizziness, insomnia, vomiting, and irritability, this is Ezu disease (Morning Sickness). Only Ren Shen Dingxiang San (Ginseng and Clove Powder), Banxia Fuling Tang (Pinellia and Poria Decoction), Fuling Wan (Poria Pill), Xiao Qixiang Wan (Small Seven Fragrances Pill), or Yao Dingxiang may be used. For prenatal bleeding and restless fetus, named Lotai (Leaking Fetus), An Tai Yin (Fetus-Calming Drink), Siwu Tang, Jiao'ai Tang, Baosheng Wan, Hupo Wan, or Zeluan Wan may be used. In the month of delivery, one may regularly consume Huatai San (Fetus-Slippery Powder), An Tai Yin, or Baosheng Wan. For dystocia, whether transverse or breech, or failure to deliver after two to three days, or fetal death in utero, or retained placenta, Huaruishi San (Flower Rhyolite Powder) may be used, or Bao'an Gao applied as one dose, formed into pills the size of Chinese bayberry seeds, swallowed with warm wine; if not delivered, administer another dose. For abnormal labor presentation such as transverse or breech birth, Tutui Wan (Rabbit Marrow Pill) may be taken. After the infant is born but the placenta remains, accompanied by hard, distended, and urgent abdominal pain, Niuxi Tang (Achyranthes Decoction) may be administered.
Discussion on Postpartum Diseases: These often result from excessive blood loss during childbirth, leading to dryness and deficiency of body fluids, deficiency of both yin and yang, and weakness of the zang-fu organs. Sweating must absolutely not be induced, nor should laxatives, emetics, or purgatives be used. Overly hot and drying tonics must also be avoided. Only mild and gentle medicines should be used for gradual adjustment. One must not seek immediate relief by indiscriminately administering decoctions, which may cause other diseases; the consequences are serious, so caution is essential. Since new postpartum qi and blood are both deficient, over-tonification should be avoided as it may exacerbate chills and fever. It is best to allow lochia to clear completely. Heishen San, Dasheng San, Zeluan San, or Puhuang San may be used. For postpartum coma, fainting, and mental confusion due to excessive blood loss where qi and blood lose their mastery, Xiongdi Tang or Dasheng San may be taken with rice water. If ineffective, Huaruishi San or Bao'an Gao may be used. It is beneficial for postpartum women to inhale the smoke of vinegar-treated charcoal. For sudden postpartum fever and general stiffness, Siwu Tang may be used, combined with Ren Shen Danggui San or Xiaoyao San. For postpartum abdominal masses causing moving pain, caused by incomplete expulsion of lochia where new blood clashes with stale blood, Heishen San or Puhuang San may be decocted as previously described, combined with Peng Jian Wan (Curcuma Decoction Pill) or Hupo Wan. If lochia suddenly stops after initially flowing, accompanied by alternating chills and fever, quietness during the day and worsening at night, and delirious speech resembling hallucinations, this is due to heat entering the Blood Chamber. Hupo Wan or Siwu Tang with an additional 3 grams of Chaihu (Bupleurum) decocted cleanly may be used. If the fever does not subside, Xiao Chaihu Tang (Minor Bupleurum Decoction) with three to four stalks of Sheng Dihuang (Fresh Rehmannia), crushed and decocted together, may be used. For postpartum patients with excessive blood loss, low fluids, and dual yin-yang deficiency, even if sweating is indicated for cold damage or seasonal epidemics, it must not be excessive. Light formulas such as Qinggu San, Wuji San (Five Accumulations Powder), Baidu San (Defeating Toxin Powder), Shenzhu San (Divine Artisan Powder), Hejie San (Harmonizing Solution), or Dasheng San may be used. For sudden umbilical and abdominal pain six to seven days postpartum, caused by cold air entering through respiration due to deficiency, Danggui Jianzhong Tang, Lizhong Wan (Regulating the Middle Pill), Tongzhen Wan, Wenjing Tang, Da Chenxiang Wan (Large Agarwood Pill), or Yanjian San (Salt-Fried Powder) may be used. If symptoms persist, Pingwei San may be taken to swallow Huixiang Wan. For severe postpartum abdominal pain, Huaruishi San or Taiyue Huoxue Dan (Taiyue Blood-Activating Elixir) may be used. For postpartum irritability, thirst, and desire for cold drinks, caused by excessive blood loss leading to yin deficiency and opportunistic yang, internal support is needed. Siwu Tang with two Wumei (Chinese Plum) added per dose may be decocted. If thirst persists, Wuling San (Five Ingredients Powder) may be taken with warm water. For postpartum convulsions, delirium, hallucinations, and unsettled spirit, Longhu Dan (Dragon and Tiger Elixir) may be used, three pills taken as one dose, mixed with finely ground Cinnabar (Zhusha) and taken with warm wine, combined with Hupo Wan. For frequent exacerbation of postpartum blood-wind, lethargy, unconsciousness, sensation of needle pricks, hair falling out with combing, or generalized edema, Longhu Dan may be used, three pills as one dose, chewed with Danggui decocted wine. If itching occurs, Ren Shen Jingjie San may be combined. For postpartum lactation failure, blood-moving drugs must not be used to avoid causing other diseases. Only blood and qi-nourishing drugs should be taken to naturally promote abundant blood vessels and regulate milk. Siwu Tang, Shugan Dihuang Wan, Baiwei Wan, Neibu Danggui Wan, Yiyin Dan, Dashun San (Great Smoothness Powder), or Zeluan San may be used. For postpartum constipation or difficult defecation, caused by lack of internal fluids and dry intestines, harsh purgatives must absolutely not be used to avoid other diseases. Siwu Tang with peeled Qingpi (Citrus Reticulata Green Peel) added, half a qian per dose, may be mixed and decocted. If still obstructed, Maren Wan or Sanhe San (Three Harmonies Powder) may be used.
