Discussion on the Manifestations of Phlegm-Fluid Retention: Various phlegm-fluids fail to transform and stagnate in the chest and epigastrium, causing dizziness of the head and eyes, nausea and vomiting with discomfort, and gurgling sounds in the abdomen. One may administer Xiaoyin Wan (Eliminate Fluid Pill), Beishu Wan (Double Turmeric Pill), or Wuling San (Five-Ling Powder). If there is cold, administer Lizhong Wan (Regulate the Middle Pill), Qingzhou Baiwanzi (Qingzhou White Pill), or Yushanren Jiangqi Tang (Dr. Yu's Qi-Descending Decoction). For those with thirst after drinking wine and retained fluids leading to regurgitation, nausea, and aversion to food, as well as for headache or excessive alcohol consumption causing back pain radiating to the waist and loss of appetite, administer Xinfa Banxia Tang (New Method Pinellia Decoction), Xiaoyin Wan, Beishu Wan, Chensha Huatan Wan (Cinnabar Phlegm-Resolving Pill), Shengqi Tang (Qi-Vitalizing Decoction), Kuaiqi Tang (Qi-Swiftening Decoction), Banxia Wan (Pinellia Pill), Tiannanxing Wan (Arisaema Pill), Dayangqi Wan (Great Qi-Nourishing Pill), Jupi Banxia Tang (Tangerine Peel and Pinellia Decoction), or Xiaojiangqi Tang (Minor Qi-Descending Decoction).
Discussion on Cough and Asthma: Generally, coughs originate from the Lung. Physicians discuss the etiology and medication broadly in three categories: those caused by Cold, those by Wind, and those by Heat. Wind and Cold enter from the exterior, while Heat arises from within. Wind and Cold affect the meridians directly, whereas Heat causes disease through mutual steaming and attacking among meridians and zang-fu organs. Wind is treated by dispersing, Cold by warming, and Heat by harmonizing. "Purging" refers to purging the Lung meridian, not the zang-fu organs themselves; one should use herbs such as Tinglizi (Descurainiae Semen) and Sangbaipi (Mori Cortex). Those caused by Wind cough severely when exposed to wind; those caused by Cold cough intensely in cold weather; those caused by Heat cough immediately when overheated. A very reliable diagnostic method is to ask about their condition during nighttime alcohol consumption. If cough worsens after drinking, there is Heat; if it improves, there is Cold. Clear and white sputum indicates Cold, while thick and yellow sputum indicates Heat. Prescribe medications according to the syndrome.
For acute cough due to Wind-Cold invasion, with thick and sticky sputum and chest diaphragm discomfort, administer Jinfeicao San (Boilweed Powder), Banxia Wan, Kuanhua Gao (Coltsfoot Paste), Huagai San (Huagai Powder), Wusou Wan (Five-Cough Pill), Runfei Wan (Moisten Lung Pill), Kunfei San (Moisten Lung Powder), Qingjin Wan (Blue Gold Pill), Xiaorunfei San (Children's Moisten Lung Powder), or Kundonghua San (Coltsflower Powder). Regarding Cold Cough, characterized by recurrent cold coughing, expectoration of green phlegm, and worsening at night, administer Xixin Wuweizi Tang (Asarum and Schisandra Decoction), Yangzhong Tang (Nourish Middle Decoction), Wusou Wan, Yushanren Jiangqi Tang, Renshen Huoxiang Tang (Ginseng and Agastache Decoction), Hujiao Lizhong Wan (Pepper Regulate-Middle Pill), Wenfei Xixin Tang (Warm Lung Asarum Decoction), Zhongru Bufei Tang (Stalactite Tonify Lung Decoction), Xiaoyin Wan, Beishu Wan, Dingxiang Banxia Wan (Clove and Pinellia Pill), Fenxin Qi Yin (Separate Heart Qi Drink), or Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder). For those averse to wind with Cold, administer Xiaoqinglong Wan (Minor Blue Dragon Pill), combined with Kunfei San and Renshen Kuanhua Gao (Ginseng Coltsfoot Paste). Regarding Heat Cough, characterized by chest discomfort, qi congestion and upward excess, red face, and dry mouth and tongue, administer Jinfeicao San, Da Ejiao Wan (Large Donkey-Hide Gelatin Pill), Renshen Yangfei Wan (Ginseng Nourish Lung Pill), Qingxin Yinzi (Clear Heart Infusion), Renshen Kuanhua Gao, Banxia Wan, Huagai San, or Renshen Runfei Wan (Ginseng Moisten Lung Pill). For Wind-Phlegm with upper jiao heat congestion, dry throat, and hemoptysis, administer Chensha Huatan Wan, Da Ejiao Wan, Jianwan (Wax-Fried Pill), Renshen Yangfei Wan, Jinfeicao San, Qingzhou Baiwanzi, Chuanxiong Wan (Ligusticum Pill), or Jisu Wan (Elsholtzia Pill). For chronic cough, deficiency syndromes, and the elderly, administer Wenfei Wan (Warm Lung Pill), Renshen Bufei Tang (Ginseng Tonify Lung Decoction), Dingxiang Banxia Wan, Renshen Huoxiang Tang, or Huatan Wan (Resolve Phlegm Pill). When Cold and Heat intermingle, this syndrome often occurs in autumn and winter; administer Kundonghua San, Banxia Wan, Huagai San, Renshen Kuanhua Gao, Renshen Runfei Wan, Xiaorunfei San, Qingjin Wan, or Runjin San (Moisten Gold Powder). Take one dose divided into two servings; this is especially suitable for the elderly. For asthma with rapid breathing and inability to lie down, administer Dingchuan Tang (Stop Asthma Decoction), Ruiying Wan (Auspicious Response Pill), Jian San (Wax-Fried Powder), Jiangqi Tang, Kuanhua San, Runjin San, Renshen Yangfei Wan, or Huagai San. For expectoration of pus and blood, administer Kundonghua San or Jiuxian San (Nine Immortals Powder).
