The spleen and stomach are primarily responsible for receiving food without discharging waste, while the large and small intestines are responsible for discharging waste without receiving food; this constitutes the physiological process of intake and excretion of water, grain, and their residues during daily life. It is commonly stated that the stomach receives food. However, vomiting and regurgitation following variola or varicella (skin eruptive diseases) occur because pathogenic factors interfere, causing stomach qi to rebel and become unsmooth, thereby leading to rebellious qi and vomiting. Such vomiting is categorized into cold vomiting and heat vomiting. Vomiting occurring after variola/varicella indicates that residual toxic pathogens remain unresolved in the stomach; careful differentiation is required as it differs fundamentally from other conditions. When heat-toxin accumulates in the stomach causing vomiting, it induces mental restlessness and thirst. In infants, incessant nursing indicates thirst; urine is dark and scanty with dysuria; palmar sweat or heat is present; the face is red with a desire to drink. These symptoms stagnate in the stomach, forming phlegm, causing discomfort in the chest and diaphragm. Even if the infant can nurse, the accumulated milk may be vomited out forcefully like a spray, providing temporary relief in the chest, but thirst persists, leading to further nursing and subsequent vomiting. The pulse is surging and rapid, indicating heat vomiting. Cold vomiting, by contrast, presents without thirst; food and water are vomited immediately upon ingestion; the complexion is cyanotic and pale; hands and feet are cold; urination and defecation are normal. Careful differentiation is essential. For vomiting caused by residual heat-toxin in the epigastrium, Huangqin San (Scutellaria Powder), Wuling San (Five-Ingredient Poria Powder), and Renshen Zhuye Tang (Ginseng and Bamboo Leaf Decoction) are appropriate. If residual toxin affects the mouth, causing sores on the lips and tongue, with lesions in the lower body or purulent bloody stools, Huanglian San (Coptis Powder) and Sanhuang Shuai Ai Tang (Three-Yellow Prepared Mugwort Decoction) should be administered. For wheezing accompanied by irritability and scanty urination, Dengxin Tang (Juncus Decoction) is indicated. For gum swelling and pain due to stomach heat, which impedes chewing, hunger without appetite, mental irritability, fever, and sallow complexion, Ganlu Yin (Sweet Dew Drink) and Huaihua San (Sophora Flower Powder) are suitable. If the toxin invades the eyes, Jingxin San (Heart-Clearing Powder) is most effective. For sore throat and persistent cough, Mahuang Tang (Ephedra Decoction) augmented with Moschus (Shexiang) should be taken. For constipation where heat-toxin stagnates and attacks the zang-fu organs and eyes, along with swollen and obstructed throat, and ulcerated/necrotic mouth and tongue, Rusheng Tang (Like-Sage Decoction), Zihewsan (Placenta Powder), or Zixue (Purple Snow) may be selected; these are key herbs for resolving residual toxins.
To prevent disease before it arises, one should administer oil preparations preemptively to resolve heat-toxin before vesicles or pustules form. Generally, when an infant exhibits slight heat in the four limbs, reduced appetite, dry and erect hair, or slight forehead heat, raw oil preparation should be given for drinking according to the prescribed method each night. If the infant feels limb heat, they will sleep soundly after prolonged administration. After three to five doses, urination and defecation become smooth, limb heat subsides, and pathogenic toxins gradually recede; even if eruptions appear, they will be mild. This is an especially excellent method.
