Xia Ziyi recorded in *Qiji Fang* (Book of Strange Diseases): There was a person who lay bedridden, unable to move his four limbs, could only eat food, and liked to loudly talk about wanting to eat certain things. This disease is called "Shishuo Wuwang Bing" (Disease of Losing the Desire for Food by Talking). The treatment method is: when the patient says he wants to eat pork, tell him he can have a meal of pork, and the patient will be pleased upon hearing this. Then place the meat where the patient can see it, but do not give it to him to eat when he is about to. This is what is meant by "Shishuo Wuwang". When saliva flows from the patient during sleep, the illness will heal.
Commentary by Shen Yuan: This syndrome occurs because Yang Qi is excessive internally, hence the tendency to speak loudly and discuss food. Moreover, excess Yang leads to Qi stagnation; Qi stagnation causes obstruction in the meridians and collaterals, resulting in the inability to move the four limbs. The treatment involves mentioning the food he desires and promising to feed it to him, thereby pleasing his mind. This aligns with the statement in the *Neijing* (Inner Canon) that "Joy disperses Qi."
However, the reason for placing the food within sight but not giving it to him is based on the principle that "food enters Yin and nourishes Yang." By allowing him to see the object of his desire without consuming it, saliva is induced to flow. Saliva is phlegm, phlegm is fire, and fire is Qi; they are different names for the same substance. When saliva flows, Qi naturally diminishes, and the illness heals spontaneously.
It is also recorded: A person suffered from foul-smelling fluid flowing from the mouth and nose. When collected in a bowl, it resembled iron-colored shrimp or fish, about the size of a grain of rice, jumping incessantly. Upon being touched by hand, it turned into water. This is the condition known as "Rouhuai" (Flesh Decay). Eating chicken at will will lead to self-healing.
Commentary by Shen Yuan: This syndrome is named "Rouhuai" due to the foul-smelling fluid from the mouth and nose. It arises because toxic pathogens invade the internal mucosal layers, causing flesh to rot like pus and water, hence the fluid is foul. The resemblance to iron-colored aquatic creatures when placed in a bowl is due to the pathology affecting the regions governed by the Kidney. Since the Kidney belongs to Water and its color is black, the transformation by Kidney Qi results in an iron-like color and aquatic form. Why does this fluid not discharge downward internally? It is because it has been infused with the spiritual vitality of human Qi and Blood, enabling it to jump in water. Thus, it ascends with the rising Qi and exits through the mouth and nose. If grasped, it turns to water because the Qi dissipates. Generally, toxins transformed by Lung Qi appear white in pus, as the Lung belongs to Metal and its color is white. This toxin originates in the internal membranes, in the region governed by the Kidney, not the Lung. Therefore, it transforms according to the Kidney's Qi and assumes its associated color. Chicken tonifies deficiency and warms the middle, while also attacking toxicity. Extreme Yin toxins require attack to disperse them, which corresponds to the *Neijing* saying "treatment should employ medicinal substances" (folklore notes that steaming chicken to induce sweating cures smallpox instantly, verifying the power of attacking toxicity).
It is also recorded: A person had a fleshy ball growing in the mouth, with a root like a thread over five inches long, resembling the shaft of a hairpin. He could only eat after spitting it out; pinching it caused pain piercing the heart. One qian of musk powder taken orally with water, three times daily, caused it to disappear spontaneously.
Commentary by Shen Yuan: Although described as being in the mouth, this fleshy growth must originate on the tongue. The *Neijing* states: "The Heart meridian connects to the root of the tongue, and the Spleen meridian winds around the sides of the tongue and connects below it." All tongue diseases result from disorders of the Heart and Spleen meridians. Hence, pinching it causes pain corresponding to the Heart. This condition arises from Spleen Earth dysfunction and meridian obstruction. As stated, "when the child is sick, the mother is also sick," and in excess conditions, one should drain the child. Therefore, using musk, which enters the Spleen and treats flesh-related issues, unblocks Earth Qi and opens the meridians, causing the abnormal growth to vanish and the illness to heal.
It is also recorded: A person suddenly experienced upward rushing Qi, panting, inability to speak, drooling, vomiting, and loose teeth as if falling out. The panting worsened, leading to suffocation and fainting before recovery. This is named "Shanghan Bing Re Huoluan" (Cold Damage Complicated by Hot Cholera). Use half a liang each of Dahuang (Rhubarb Root) and Renshen (Ginseng), decocted in three cups of water until one cup remains. Strain and drink hot; the body will settle.
It is also recorded: A person felt compelled to drink oil until reaching five jin to feel satisfied; otherwise, he fell ill. This is caused by hair entering the Stomach, wrapped by Qi and Blood, transforming into worms. Take half a liang of Xionghuang (Realgar) ground into powder, mixed with water and ingested; the worms will exit naturally.
*Qibing Fang* records: A person developed sores on the upper lip, which eventually grew teeth-like structures protruding from the sore. This is caused by emotional depression and Fire stirring up tooth formation. The prescription uses Chaihu (Bupleurum Root), Baishao (White Peony Root), Danggui (Angelica Sinensis), Shengdi (Rehmannia Root), each 3 qian; Huangqin (Scutellaria Root) 1 qian; Tianhuafen (Trichosanthes Root) 2 qian; Baiguo (Ginkgo Seed) 10 pieces. Decoct in water and take. Externally, mix Bingpian (Borneol) 1 fen, Jiangcan (Bombyx Batryticatus) 1 qian, and Chao Huangbai (Phellodendron Bark, stir-fried) 3 qian into powder, apply to the teeth; they will dissolve.
Zhang Jifeng recorded in *Beiji Liangfang*: A person suffered from teeth growing longer daily, making eating difficult due to anal blockage. This is named "Muyi Bing" (Marrow Overflow Disease). Rinse and swallow with decoction of Baizhu (Atractylodes Macrocephala Rhizome); it heals immediately.
*Soushen Ji* records: Huan Xuanwu had a commanding officer who, after suffering from seasonal illness and subsequent deficiency-heat, could consume one hu and two dou of fragrant tea to feel full. If less, he felt unsatisfied. Later, a guest visited and offered five sheng more. The officer vomited a large object, about one sheng in volume, with an opening, wrinkled texture, resembling a cow's stomach. The guest instructed placing it in a basin and pouring two hu of fragrant tea over it. The object dissolved completely, leaving the patient feeling slightly bloated. Adding another five sheng caused the entire mass to gush out from the mouth. After vomiting this object, the illness cured. Asked what disease this was, the reply was: "This disease is named 'Huming Jia' (Tea Lump)."
Dongpo recorded in *Wulei Xiangyingzhi*: Qixie recorded that in Anlu County, Jiangxia, during the Long'an era, a man named Guo Yuan, after contracting epidemic fever, developed a voracious appetite, eating up to one hu daily. After five years of poverty, he begged in the market. One day, starving, he entered a garden and ate a plot of wild leeks and a plot of garlic. He soon felt stifled and lay flat on the ground. Soon after, he vomited a cage-like object that gradually shrank. When someone placed rice on it, it dissolved into water, and the illness healed.
*Youyang Zazu* records: Liu Luoshi of Hezhou, after resigning his post during the Dali era, lived in a county near Hezhou. His appetite exceeded several people's, especially for zhajiang (fermented fish paste). He never felt full. Local guests gathered over a hundred jin of fish in the wilderness to watch him eat. After eating several plates of zhajiang, Liu seemed to choke slightly and coughed out a bone bead, bean-sized, which he placed in a teacup covered by a plate. Before finishing half the meal, the plate covering the cup tipped over. Looking closely, the bone bead had grown several inches, taking a human shape. Guests watched as it grew with their gaze, reaching human height in moments. It then attacked Liu, leading to a bloody fight until they scattered. One circled right, the other left, meeting at the back door and merging into one person—Liu. He became mentally confused, speaking only after half a day. He remembered nothing and subsequently hated zhajiang.
*Guangyiji* records: An assistant official in Jurong County could eat dozens of jin of zhajiang, yet always remained hungry. The county magistrate, hearing of his appetite, provided a hundred jin. The official ate it joyfully but soon felt suffocated. After a long time, he vomited an object shaped like a hemp shoe. The magistrate ordered it washed and placed somewhere, where it turned to water. Doctors could not identify it. The magistrate sent a clerk to sell it in Yangzhou, hoping for recognition. If bought, the price would be raised to see how much it fetched. A Hu (foreigner) wanted to buy it, raising the price to three hundred guan, but the Hu refused. People asked the Hu what it was. The Hu said: "This is essence that dissolves fish; it also dissolves abdominal masses. A patient takes a piece the size of a finger tip, ties it with a string, and places it on the affected area; the mass dissolves. My country's crown prince suffered from this as a child; his father offered a thousand taels for a cure. If you sell it, you will gain great profit." The magistrate ultimately sold half to him.
*Xuanshi Zhi* records: In the Yonghui era, Xu Shuang loved eating zhajiang, requiring three dou of raw fish per meal. One day, hungry, while preparing zhajiang, he couldn't resist hunger and vomited an object resembling a toad. He could no longer eat zhajiang thereafter.
*Mingyi Lu* records: Vice Minister Yu, from Suzhou, loved eating raw fish zhajiang. In middle age, he developed abdominal hardness, unable to bend over. Each episode caused near-fatal pain; multiple treatments failed. A skilled pulse-reader diagnosed: "The Right Guan pulse is deep and hidden (Fu), indicating accumulation. Cold/raw food accumulation has formed a Jia (lump) in the abdomen, causing unbearable pain. Medication can remove it." He prescribed swallowing pills with olive juice. That night, the patient passed a basin-full of stool containing fish intestines/accumulation; the front part had turned into fish. He recovered thereafter.
*Shuo Yuan* records: Lu, a native of Wujun, loved wheat noodles since childhood; the more he ate, the weaker he became. A Hu person used medicine to expel a worm, about two cun long, green and frog-shaped. This is called "Xiaomian Chong" (Noodle-Dissolving Worm), actually a rare treasure. The explanation is strange and not detailed here.
Hermit of Yutian recorded: A young woman suddenly craved river mud, eating several bowls daily to feel satisfied. A doctor used soil from wall cracks, mixed with water, and gave it to her; she healed. Danxi stated: Craving mud indicates Heat in Stomach Qi. Treat with Huangqin (Scutellaria Root), Baizhu (Atractylodes Macrocephala Rhizome), Fuling (Poria), and Ruang Shigao (Soft Gypsum) decocted and taken.
*Yangshi Jingyan Fang* records: Liu Qingxiao, a performer from Yichang, had a courtesan named Hua Cui, over fifteen years old, who loved eating raw rice. Zhao Yin, supervisor of the Huimin Bureau, soaked Cangzhu (Atractylodes Lancea Rhizome) in rice wash overnight, ground it, and made pills the size of wu zi seeds with steamed dough. Take fifty pills before meals with rice water, twice daily. Cured in twenty days. This syndrome arises from raw/cooked food stagnating in the intestines and stomach, generating worms. Over time, craving raw rice develops; otherwise, the patient is unhappy all day, becoming emaciated and yellow, losing appetite, endangering life.
*Yishuo* records: Formerly, Shen Gongdao was emaciated like consumption, loving only rice. Without it, clear water flowed from his mouth; eating restored normalcy. Doctors could not diagnose. Monk Daoguang from Shu roasted chicken dung with white rice, ground into powder. Mixed with water and taken, after a while, rice-shaped matter was vomited, and he healed.
Also recorded: Chancellor Wang Huizhi, from Qimen, loved foods that stirred Wind, especially crabs, often eating crab soup or strips. One day, he contracted Wind-Heat. Flesh grew between his teeth, expanding until it blocked his mouth, preventing closure, intake of fluids, and causing extreme pain. A Daoist claimed he could cure it. The Chancellor ordered treatment, and he healed quickly. The method: use one bowl of Shengdihuang Juice (Rehmannia Root Juice), soak several stalks of Zhuya Zaojiao (Chinese Tamarisk Fruit) over fire until hot, dip until juice is absorbed, then grind to powder. Apply to the obstructing flesh; it shrinks immediately.
*Shengji Lu* records: A person suffered recurrent vomiting with water sounds in the abdomen. This is Jiupi (Alcohol癖/Lump). Chuanxiong (Sichuan Lovage Rhizome) and Pao Sanleng (Prepared Sparganium Rhizome), each one liang, ground to fine powder. Take two qian per dose, with scallion white decoction.
A scholar from Zhenyang loved wine, drinking several dou daily. At midnight, his craving became uncontrollable. One night, after heavy drunkenness, he vomited a tongue-like object with no pores. When craving wine again, his eyes rolled up, and the object stood upright. Family poured wine on it, which it drank instantly until reaching his usual amount. He subsequently developed an aversion to wine.
Another person loved wine since childhood; without it for even a moment, he screamed incessantly, refusing food. He wasted away. Someone tied his hands to a pillar, showed him wine but didn't let him drink. He vomited a pig-liver-like object, which dissolved in the wine. He thereafter hated wine.
*Yiyuan* records: During the Yuanjia era of Song, a person developed a Jia from eating duck. Taking powder from Shumi (Glutinous Rice) mixed with water caused restlessness, followed by vomiting a duckling, curing the illness. Sun Simiao said: If illness results from eating duck meat, expect chest fullness and red face, loss of appetite. Drink one cup of glutinous rice wash water.
*Chuangyang Jingyan* records: A person with Cold Phlegm was treated with one boiled clove of garlic, vomiting an object the size of a sheng, wrapped in phlegm. Inside was a chick. After another dose, thirteen chicks were vomited, curing the illness named "Ji Jia" (Chicken Lump). This resulted from excessive consumption of Bai Lun Ji (White Run Chicken).
*Bozhai Bian* records: Li Minqiu of Luoyang traveled to Eastern Wu. His wife suffered toothache, groaning in unbearable pain. Pressing a hairpin shaft between teeth turned silver black due to toxic vapors. Multiple doctors along the way failed. Monk Huihai of Jiahe prepared a remedy; after half a year of use, the suffering ceased. Later, eating hot noodles triggered recurrence. Drinking a decoction prepared while sitting cured it swiftly. The label merely said "invigorate blood and cool blood." Li asked further and learned it was Siwu Tang (Four Substances Decoction). Invigorating blood cools it; how could stagnation cause disease?
*Rumen Shiqin* records: A man suffered mouth ulcers for years, extending from mouth to throat to epigastrium, causing pain. He dared not eat hot food. Using inducing vomiting, purging, and sweating methods removed nine-tenths of the pathogen. Subsequently taking Huanglian Jiedu Tang (Coptis Decoction for Detoxification) resolved it in ten days.
*Bencao Gangmu* records: A person suffered tight, small lips unable to open for eating/drinking; untreated, it was fatal. Make a large moxa cone from white cloth, place on a knife/axe, burn until sweat appears, wipe and apply to lips, three to five times daily. Then take ash of blue cloth mixed with wine.
Commentary by Shen Yuan: The *Neijing* states: The Liver, Du, Chong, and Ren meridians all connect to the lips. If afflicted by pathogens, lips become diseased: Wind causes movement, Cold causes tightness, Dryness causes cracking, Qi stagnation causes sores, Blood deficiency causes roughness and lack of color. Treatment involves regulating Spleen and Stomach internally and applying herbs externally; all heal. The previous case used iron tools to induce sweating, restraining Liver Meridian toxicity, and ingesting burnt blue cloth ash to detoxify and dispel Wind. This was not merely Cold damage but involved Wind-Toxin. Treatment requires careful diagnosis, not rigid adherence to one method.
*Mingyi Lu* records: A Sui dynasty patient swallowed meat but vomited it upon reaching the chest. Doctors treated for Ye (Dysphagia), Ge (Diaphragmatic Qi), and Fanwei (Regurgitation) without success. Old physician Ren Du examined him and said: "Not these three diseases, but indigestion from snake meat. Feel for a snake-shape in the chest/abdomen." The patient replied: "I had severe wind-disease, ate snake meat to relieve it, wind subsided slightly, but this illness returned." He took Mangxiao (Mirabilite) and Dahuang (Rhubarb Root) together, had mild diarrhea, and healed.
Danxi treated a pregnant woman (three months) who vomited phlegm-water and food. Attacks occurred at Yin-Mao hours (5-9 AM), with Qi rushing up from lower abdomen, causing diaphragm pain and vomiting. Face red, slightly dry, dizzy, unable to rise, limb pain, slight thirst. This is Liver Fire carrying Chong Mai Fire upward. Severity varied day by day; pulse harmonious, Right Cun high. Medicine failed for nearly two months. Use Chenxiang (Agarwood) ground in water to dissolve Baolong Wan (Embrace Dragon Pill). Diaphragm relaxed, Qi stopped rushing. Two to three doses stopped vomiting, reduced dizziness, and brought peace.
A person suffered upward rushing Qi, mouth emitting smoke-like vapor, constant ear ringing. This is Chong Mai reversing upward. Prescribed Bawu Tang (Eight Matters Decoction) with fried Huanglian (Coptis Rhizome) 2 fen and Huangbai (Phellodendron Bark) 3 fen. Several doses cured it.
Zhang Zihe treated a man in his twenties whose breath smelled like a toilet, avoiding relatives. Dai Ren (Zhang Zihe's courtesy name) said: "Metal governs fishy smells; Fire conquers Metal; Fire governs foul odors. Thus the smell. Prolonged corruption affects Kidney (Water). This is extreme亢 (hyperactivity) mimicking Water transformation. Pathology is upper; use Vomiting method." Used Chatiao San to induce vomiting seven-tenths of the pathogen; at night, Zhouche Wan and Xunchuan Wan five to seven doses. By morning, odor ceased.
Luo Qianfu treated Han Zi Wangfu, over sixty, with Xiao Ke (Wasting-Thirst). Winter brought dryness-heat; he stripped naked and sprayed ice water on chest/armpits for relief. Ate meat/noodles several times daily, hungry quickly. Duration: one month. Pulse: deep, thin, rapid. Luo predicted death. Ziyu wept: "Illness is critical; save him diligently, and I die without regret." Luo said: "Xiao Ke has many names: Shi Ji, Xiao Zhong, Xuan Ji. Caused by rich/greasy food. Yangming transforms to Dryness-Fire; fluids cannot stay; spontaneous sweating, frequent urination. Drink one, urinate two. Stomach heat digests food well, causing hunger despite eating, leading to thinness. Wang Shuhe said: 'Eating much yet hungry is deficiency.' Zhang Zhongjing said: 'Worse in Spring/Summer, better in Autumn/Winter' due to seasonal influence. Your father should improve but worsened. This is Kidney Water dried up, failing to control Heart Fire, flaring in weak times. Named 'True Hyperactivity,' isolated Yang, severed Yin. Body is Master, Heaven is Guest. Extreme Cold cannot control Heat; how can medicine? *Neijing* says: 'Master winning is adverse, Guest winning is compliant.' Following your treatment is futile." He declined, returned. Changed doctors and moxibustion; died in days.
Xue Ji treated Tu Shouqing of Siming. Early Summer, incisors felt struck, unbearable pain. Pulse: surging, large, wiry. Xue said: "Surging and wiry fighting indicates impending carbuncle-toxin." First used Qingwei San (Clear Stomach Powder) plus Baizhu (Angelica Dahurica), Yinhua (Honeysuckle Flower), Lianqiao (Forsythia Fruit). Pain stopped. Evening: boil appeared on nose, face swollen/dark/painful. Continued previous formula plus Xijiao (Rhinoceros Horn) for ten doses. Swelling reached temples, foul breath, pulse larger, aversion to cold with internal heat. Toxin blazing, blood stasis; medicine insufficient alone. Pricked affected areas and lips multiple times, pierced red flesh inside mouth, draining toxic blood. Continued previous formula for several doses; healed. Clinging to taboo against pricking or relying solely on medicine rarely avoids error.
Wujiang Shi Sheng's wife had loose gum roots after every birth, resolving after days. Pregnant again near term, Xue visited, discussed syndrome, left Shiquan Dabu Tang (Ten Substance Great Tonifying Decoction) for two doses post-birth. Teeth did not loosen thereafter.
A woman over thirty suddenly stopped eating, drinking only clear tea and fruit for over three years. Xue diagnosed Spleen Qi depression/stagnation. Used Guipi Tang (Restore the Spleen Decoction) plus Wuzhuyu (Evodia Fruit) for four doses; diet normalized. For those with Spleen/Stomach deficiency not eating, use Sishen Wan (Four Spirit Pill).
Sun Dongsu treated Ye Runzhai, forty, with heart/diaphragm buzzing, craving meat daily. Missing meat caused floating body, fatigue, mental distress; urgent need to eat meat relieved it. Eating caused severe abdominal pain, acid regurgitation, thick saliva, then relief. Relief led to craving meat again. Pain and screaming were unbearable. Advised: "Better to cut meat than suffer pain?" Chronic Spleen deficiency makes meat hard to digest, causing pain. Even women know this; why don't you? Runzhai replied: "Do I not know cutting meat helps? Pain is bearable; buzzing makes body tingle, helpless, unable to grasp anything, unable to survive moments, nearing death. Urgently need meat to soothe. Do I seek pain? Forced by necessity." Sun's pulse: unequal sizes. Color: red lips, yellow face. Sun said: "Based on color/pulse, this is Worm Syndrome, not demon possession. I can kill them." First gave Xionghuang Wan (Realgar Pill); no improvement. Changed to Nifen (Calomel) 5 fen, Shijunzi (Quisqualis Fruit) 1 qian, ground, made cake with egg, fed at dawn empty stomach. At Chen hour (7-9 AM), passed ten long worms, two large ones, chi-zhi long, red from head to tail. Afternoon passed over hundred small worms. No longer craved meat; buzzing cured.
Chongzhen era, Wulin Yao Yingfeng: A man had a red mole on left lip, no other symptoms. Yao said: "This is Chi Jin (Tooth Grass); death in three years." Man angry, ignored. Three years later, teeth decayed, sought help. Yao declined: "Heaven's punishment; cannot exceed time limit."
Jiang Xisheng of Danghu's wife: Middle age, trembling after eating pork, multiple changes ineffective. Later, treating another illness with Xiaoyao San (Free and Easy Wanderer Powder) for several doses cured old complaint. Wang was in Yanjing at Chancellor Jiang's house. Visiting, discussed syndrome. I said: "*Suwen* says: 'All spasms and tremors belong to Fire.' Trembling from pork indicates pre-existing Liver/Gallbladder depression-heat."
