The patient cried out in pain, with the small intestine protruding five cun outside the umbilicus, interlaced two and a half cun each, resembling bamboo nodes or the top of a pot. The penis was swollen and twisted, eight to nine cun long, bright as a lantern; such external symptoms had never been heard of before. Yu Jiayan used only Huangqin (Scutellariae Radix) and Ejiao (Colla Corii Asini), administering more than ten doses daily. After three days, urination began; after five days, the urinary pathway became clear and unobstructed, the umbilicus retracted, swelling subsided, and the patient recovered. A disciple asked for an explanation. Yu Jiayan replied: "The Qi of the entire body relies entirely on the Lung. When the Lung is clear, Qi moves freely; when the Lung is turbid, Qi becomes stagnant. The Lung governs the skin and hair. If acne-like eruptions fail to form pus, it is due to Lung heat preventing the flow of fluids. Skin dryness resembling scales is called 'Jiacuo' (crisscross scaling). Jiacuo often indicates Lung Abscess (Fei Yong). 'Yong' means obstruction. Is this not caused by Lung Qi stagnation? This is similar to severe abdominal pain and crying, which are also signs of extreme obstruction. Severe obstruction closes the water pathways, causing Qi to move horizontally within, leading to the protrusion of the small intestine. The flaccidity and elongation of the external kidneys are merely secondary phenomena. Since the disease has reached an extreme depth, only single herbs used in large doses can guide the medicinal action downward to achieve efficacy. Thus, the prescription is very mild, yet the effect is rapid. Let me explain this using the principles of investigating things. Generally speaking, among birds and beasts, those with lungs have urine, and those without lungs do not. Therefore, when urination is difficult, edema and fullness result. Clearing the Lung heat is urgent. This principle was not fully elucidated by predecessors. Those who later treated water-dampness disorders with formulas like Wuling San (Five-Ingredient Powder with Poria) or Bazheng San (Eight-Righteousness Powder) ultimately failed to grasp this essence. One should distinguish early from using harsh agents that forcibly drain the Bladder, akin to draining a pond."
In the middle of the Jiajing era, the wife of Zou Biaozhi from Changzhou suffered from a lump growing below the lower abdomen, shaped like a plum or a plumcot. Over time, it rotted and ulcerated, initially expelling material resembling rice dregs, and later resembling clam meat. When pinched open with fingers, several strands of long hair were found inside. Famous physicians could not cure it, and she eventually died. Shefa Jia (Snake-Hair Mass), often recorded in medical texts, may arise from food interactions generating blood stasis; this theory may have merit and should not be dismissed as absurd.
A twenty-three-year-old man experienced pulling pain at one point in the diaphragm, with a sensation of tight skin upon inhalation, indicating the presence of stagnant blood. He was prescribed Shihuang (Talcum) 1 liang, Taoren (Peach Kernel) and Huanglian (Coptidis Rhizoma) 5 qian each, Zhike (Aurantii Fructus) 1 liang, and Zhigancao (Prepared Licorice Root) 2 qian. These were ground into powder. Each dose was 1.5 qian, decocted and drunk with natural radish juice, taken five to six times a day.
A woman in my hometown accidentally swallowed a needle while biting thread. The needle head disappeared down her throat and into her abdomen, leaving her helpless. A neighbor suggested: "Quickly take the eyes of a toad and swallow them with warm water; they will guide the needle out from the tail end." The treatment proved effective. Similarly, a boy in Wulin played a game of swallowing needles, and the needle entered his abdomen. He was treated in the same manner and expelled the needle. If toads were unavailable, one should go to the south side of mulberry trees and dig about one zhang deep, where toads can be found.
