Ren Mai (Conception Vessel) pathology manifests in women as leukorrhea with red and white discharge. This occurs because injury to the Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel) damages the Dai Mai (Girdle Vessel), impairing its ability to restrain and hold. If the pathogen injures the Qi level, it presents as white leukorrhea; if it injures the Blood level, it presents as red leukorrhea. This is largely due to endless thoughts and unfulfilled desires, or excessive sexual intercourse, leading to flaccidity of the Zong Jin (Tendons) and resulting in Jin Wei (Sinew Impotence). Over time, this develops into Bai Yin (White Emission), which is a severe form of white leukorrhea characterized by continuous overflow, often resembling rice water or sticky glue. Additionally, labor-induced damage to Yuan Qi (Original Qi), Liver Wood overacting on Spleen Earth, downward pouring of Damp-Phlegm, Yin injury by Damp-Heat, or organ damage by Cold-Dampness can all cause leukorrhea in women.
Wind causes the pulse to be Floating (Fu); Dampness causes the pulse to be Slow (Huan); Damp-Heat causes a Surging (Hong) and Choppy (Se) pulse; Slippery (Hua) indicates Phlegm pouring downward; Deficiency and decline present with a Weak (Ruo) pulse; Slippage and deficiency present with a Vacuous and Minute (Wei) pulse; Large (Da) indicates internal injury from overexertion; a Pulse that is Depleted (Tuo) indicates an untreatable condition.
The treatment principle focuses on supporting Original Qi and regulating Dampness, assisted by nourishing Yin and astringing slippage. One should dispel Wind, clear Damp-Heat, raise Yang, and strengthen Spleen and Kidney, adapting the method according to the specific syndrome.
The primary formula is Er Chen Er Zhu Tang (Two-Chen Two-Atractylodes Decoction). Add Ginseng for Qi deficiency; add Angelica Sinensis and Rehmannia for Blood deficiency; add Oyster Shell and Phellodendron Bark for accompanying Heat; add Alpinia Oxyphylla and Prepared Ginger for accompanying Cold; add White Peony Root and Donkey-hide Gelatin for red leukorrhea; add Fried Phellodendron Bark and Black Schizonepeta for Damp-Heat. For deficiency of both Chong and Ren, use Di Huang Tang (Rehmannia Decoction); for deficiency of Liver and Spleen, use Xiao Yao San (Free and Easy Wanderer Powder); for deficiency of Heart and Spleen, use Gui Pi Tang (Restore the Spleen Decoction); for deficiency of Spleen and Lung, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction).
