(Attachment to Zi Zhong, Zi Xuan, and Zi Qi)
During pregnancy, Spleen deficiency leads to the retention of dampness, and the functions of distribution and transformation are impaired, resulting in fullness and distension in the heart and abdomen. Alternatively, coldness of the uterus causing a sensation of coldness in the lower abdomen as if fanned by a wind also leads to distension; this is referred to as Zi Man (Excessive Fullness during Pregnancy) or Zi Zhang (Distension during Pregnancy). If fetal heat causes qi stagnation, and damp-heat fails to transform, rising upward to congest the chest and heart region leading to distension, it is called Zi Xuan (Fetal Distension). If Earth qi fails to control Water, and water diffuses into the skin, causing edema of the head, face, hands, and feet, it is called Zi Zhong (Fetal Edema). If pressing the edema leaves no indentation, and it subsides immediately after delivery, this is due to the obstruction of fetal qi, known as Zi Qi (Fetal Qi Stagnation).
Fullness with a deep pulse indicates Zi Qi; Zi Xuan presents with a rapid pulse; Zi Zhong presents with a floating, moderate, or choppy pulse. If it belongs to Zang Cold, the pulse must be deep and fine.
Treatment primarily focuses on strengthening the Spleen and transforming qi, combined with methods to resolve pathogenic factors from both interior and exterior, ensuring not to damage the fetal qi. For damp-heat, clear it; for cold-damp, warm it. If it is Zi Qi, peace can only be achieved after delivery.
The main prescription is Zi Su Yin (Perilla Decoction) without Gan Cao (Licorice Root). Add Bai Zhu (Atractylodes Macrocephala Rhizome) for Spleen deficiency; add Hou Po (Magnolia Bark) for abdominal distension; add Sheng Di Huang (Rehmannia Root) for Blood deficiency; add Mu Xiang (Saussurea Root) for abdominal pain. For Spleen deficiency with abundant dampness, use Si Ling San (Four-Poria Powder) plus Zi Su Geng (Perilla Stem), Xiang Fu (Cyperus Rhizome), Mu Xiang, and Sha Ren (Amomum Fruit). For Liver Wood overacting on Spleen, use Xiao Yao San (Free and Easy Wanderer Powder) plus Zi Su Geng, Da Fu Pi (Areca Peel), Mu Xiang, and Sha Ren. For Stomach deficiency with poor appetite, use Ren Shen Yang Wei Tang (Ginseng Decoction to Nourish the Stomach). For Spleen deficiency with abdominal distension, use Liu Jun Zi Tang (Six-Gentlemen Decoction). For damp-heat distension, use Qin Lian Er Zhu Tang (Scutellaria, Coptis, and Two Atractylodes Decoction). For cold-damp distension, use Zhang Zhongjing's Fu Zi Tang (Aconite Decoction).
Zi Su Yin (Perilla Decoction)
Treats pregnant women with edema and fetal movement with a floating pulse.
Zi Su Ye (Perilla Leaf) (1.5 qian), Da Fu Pi (Areca Peel) (1.5 qian), Chuan Xiong (Ligusticum Rhizome) (0.8 fen), Bai Shao (White Peony Root) (1.5 qian, stir-fried with wine), Dang Gui (Angelica Sinensis Root) (3 qian), Ren Shen (Ginseng Root) (0.8 fen), Chen Pi (Citrus Reticulata Peel) (1.5 qian), Zhi Gan Cao (Honey-fried Licorice Root) (0.5 fen), Sheng Jiang (Fresh Ginger) (3 slices), Cong Bai (Scallion White) (3 stalks).
Decoct in water, remove dregs, and take warm.
When fetal qi is obstructed internally and wind pathogens constrain externally, blood and qi are insufficient to nourish the fetus, resulting in fetal movement and edema. Zi Su regulates blood and qi to dissipate swelling; Da Fu Pi regulates stagnant qi to calm the fetus; Dang Gui nourishes blood to enrich the fetus; Bai Shao astringes yin and harmonizes blood; Chuan Xiong moves blood in the Sea of Blood and effectively regulates qi within the blood; Ren Shen tonifies Spleen and Lung to nourish the origin of fetal respiration; Chen Pi promotes qi flow and harmonizes the middle; Zhi Gan Cao moderates the middle and benefits the Stomach; Sheng Jiang disperses exterior pathogens; Cong Bai unblocks yang qi. Decocting in water and taking warm helps resolve exterior wind pathogens, clearing and harmonizing blood and qi so that fetal qi becomes smooth naturally. With proper nourishment, fetal movement will be stable, and edema will subside.
Qian Jin Li Yu Tang (Thousand Gold Carp Soup)
Treats pregnancy with edema, scanty/difficult urination, and a deficient pulse.
Li Yu (Carp) (one whole fish, scales and entrails removed), Dang Gui (3 qian), Bai Shao (1.5 qian, stir-fried with wine), Bai Zhu (2 qian, stir-fried), Fu Ling (Poria) (3 qian), Ju Hong (Citrus Reticulata Red Peel) (1.5 qian).
First, decoct one carp in plain water until three sheng of liquid remains. Add the herbs and boil until one sheng remains. Remove dregs and take warm.
In pregnancy, deficiency of both Liver and Spleen prevents the transport and transformation necessary to control dampness diffusion, resulting in generalized edema and scanty urine. Li Yu directs qi downward and promotes diuresis; Ju Hong transforms qi and benefits the Lung; Dang Gui nourishes Liver blood to enrich the channels; Bai Shao astringes Liver yin to calm the fetus; Bai Zhu strengthens the Spleen to control dampness; Fu Ling clears the Lung and harmonizes the Spleen. Cooking the fish juice with the herbs aims to restore the qi transformation of the Liver and Spleen. Once dampness is transported and qi is regulated, urination will naturally become smooth. Why worry about persistent edema or unstable pregnancy?
