Cough after childbirth may be caused by the depletion of Yin and blood, damage to Lung Qi, flaming up of ministerial fire (Yin fire), or contraction of wind-cold. The primary treatment methods are as follows: For deficiency of Yin and blood, use Chuanxiong Rhizoma (Chuanxiong), Angelicae Sinensis Radix (Danggui), Prepared Rehmanniae Radix (Shudi), Ginseng Radix et Rhizoma (Renshen), and Atractylodis Macrocephalae Rhizoma (Baizhu). For damage to Lung Qi, use Sijunzi Tang (Four Gentlemen Decoction) with added Chuanxiong Rhizoma (Chuanxiong), Angelicae Sinensis Radix (Danggui), and Platycodonis Radix (Jiegeng). For flaming up of ministerial fire (Yin fire), use Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) with added Ginseng Radix et Rhizoma (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu). For contraction of wind-cold, use Buzhong Yiqi Tang (Decoction for Boosting the Middle and Augmenting Qi) with added Platycodonis Radix (Jiegeng) and Perillae Folium (Zisu). If blood stasis enters the Lung causing wheezing, urgently administer Erwei Renshen Yin (Two-Ingredient Ginseng Infusion); many patients survive. If accompanied by black discoloration around the mouth and nose, or epistaxis, urgently administer Qiansan (Prior Powder); some patients survive. However, all these conditions stem from insufficiency of Stomach Qi, because the Stomach is the root of the five Zang organs and the foundation of the human body. Once Stomach Qi is deficient, the five Zang organs lose their nourishment, and myriad diseases arise. Patients often attribute this to loose interstices (Wei Qi failing to secure the exterior), not realizing that the Lung belongs to Xin Metal and is born from Ji Earth (Spleen Earth). It is precisely because Earth deficiency fails to generate Metal that the interstices become loose and susceptible to external pathogens. Only when ministerial fire (Yin fire) flares up can it strengthen Earth; subsequently, Metal generates Kidney Water to control Fire, which is the optimal approach. If one merely treats the symptoms directly without addressing the root, it is an error.
Case Records:
A postpartum woman suffered from cough with a heavy voice, nasal congestion, and runny nose, indicating contraction of wind-cold. After taking one dose of Renshen Yin (Ginseng Infusion), her symptoms improved significantly. She was then prescribed Buzhong Yiqi Tang (Decoction for Boosting the Middle and Augmenting Qi) with added Platycodonis Radix (Jiegeng), Poria (Fuling), and Pinelliae Rhizoma Preparatum (Banxia), recovering after one dose. Subsequently, she took Liujunzi Tang (Six Gentlemen Decoction) with added Astragali Radix (Huangqi) to consolidate the interstices and restore health.
A postpartum woman vomited phlegm in the morning, had fever in the evening, and suffered from insomnia. Adhering rigidly to methods of clearing phlegm and descending fire, her body grew thinner daily, her appetite decreased, and her previous symptoms worsened. I stated: Vomiting phlegm in the morning indicates Spleen Qi deficiency; fever at night indicates Liver Blood deficiency; inability to sleep day and night indicates consumption of Spleen Blood. Therefore, she was sequentially treated with Liujunzi Tang (Six Gentlemen Decoction), Jia Wei Xiaoyao San (Modified Free and Easy Wanderer Powder), and Jia Wei Guipi Tang (Modified Restore the Spleen Decoction), resulting in recovery within a month.
A postpartum woman presented with cough, copious sputum, flushed face, dry mouth, internal heat, afternoon tidal fever, and irregular episodes of heat. This indicated flaming up of ministerial fire (Yin fire). Treatment focused on tonifying the Spleen and Kidneys using Buzhong Yiqi Tang (Decoction for Boosting the Middle and Augmenting Qi) and Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia), leading to recovery.
A postpartum woman experienced cough, abdominal fullness, refusal to eat, excessive nasal discharge and saliva, facial edema, and rebellious Qi. This pathology originated in the Stomach and affected the Lung. She recovered after taking Yigong San (Extraordinary Assistance Powder).
