Postpartum primordial qi is deficient, and lochia attacks upward taking advantage of the deficiency, causing blurred vision and dizziness, or fullness and oppression in the epigastrium, unconsciousness with clenched jaw, or excessive phlegm obstruction. In such emergencies, Shixiao San (Pain-Dispersing Powder) should be used as the primary treatment. If fainting occurs due to excessive blood loss, or if there is unconsciousness with restlessness, a large dose of Xiong Gui Tang (Ligusticum and Angelica Decoction) should be administered to tonify, or Yun Tai Zi San (Brassica Napus Seed Powder), or urine from an uncircumcised boy. If phlegm is present, Er Chen Tang (Two-Cured Decoction) should be added. If caused by overexertion and mental strain, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with Xiang Fu (Cyperus Rhizome) is appropriate. If unconsciousness results from extreme deficiency of both qi and blood, Qing Hun San (Clear the Soul Powder) should be used, followed by Xiong Gui Tang and other formulas that greatly tonify qi and blood. Generally, before delivery, a lead weight can be pre-heated until red, placed in a vessel, brought urgently to the bedside, and splashed with vinegar, or vinegar applied to the mouth and nose; smelling it will restore consciousness. Alternatively, broken lacquerware can be used, or smoke from burning dry lacquer can be inhaled, or a pill made from Ban Xia powder (Pinellia Rhizome) mixed with cold water can be inserted into the nostrils; these methods prevent the aforementioned conditions. Mr. Danxi stated: Blood fainting is due to deficiency of both qi and blood, with phlegm-fire surging upward. It is suitable to guide out phlegm with Er Chen Tang, or Jia Jian Zhu Sha An Shen Wan (Modified Cinnabar Calming the Spirit Pill), which can also be taken with Mai Men Dong Tang (Ophiopogon Decoction). Generally, for postpartum symptoms such as deviation of the mouth and eyes, the main approach should be to greatly tonify qi and blood, while simultaneously treating phlegm. If spleen and stomach are weak and unable to consolidate, Liu Jun Zi Tang (Six-Gentlemen Decoction) should be used. At five to seven months, An Tai Yin (Fetus-Calming Infusion) is suitable. At eight to nine months, Da Fu Pi (Areca Peel) and Huang Yang Nao (Buxus Seed) should be added. When approaching labor, Bao Sheng Wu You San (Life-Preserving Worry-Free Powder) is even more appropriate, thereby avoiding previous complications.
Clinical Cases
A parturient was more than a month postpartum. Due to anger, she experienced distending pain in both hypochondria, suddenly vomited a large amount of blood, had fever and aversion to cold, and chest/abdominal fullness. She was stabilized using Ba Zhen Tang (Eight-Treasure Decoction) with Chai Hu (Bupleurum Root), Dan Pi (Moutan Cortex), and Pao Jiang (Prepared Ginger). Subsequently, she recovered completely using Shi Quan Da Bu Tang (Complete Great Tonifying Decoction) still containing Pao Jiang. The preceding symptoms were due to deficiency and impairment of qi and blood in the spleen and lungs, leading to empty fullness in the chest and abdomen. Although major tonifying agents were administered, without the acrid-warm properties of Jiang (Ginger) and Gui (Cinnamon Twig) to assist the spleen and lungs and promote the movement of medicinal potency, the efficacy would not have been realized, and instead, it might have aggravated the distension.
The wife of a family member suffered lower abdominal pain after delivery, followed suddenly by trismus (lockjaw). After administering Shixiao San, she regained consciousness after a long period. She then recovered using Si Wu Tang (Four-Agent Decoction) with Pao Jiang, Bai Zhu (Atractylodes Macrocephala Rhizome), and Chen Pi (Tangerine Peel).
A parturient presented with muscle spasms, flaccid arms, and twitching muscles. This indicated deficiency of both qi and blood, and she recovered using Shi Quan Da Bu Tang.
A parturient experienced numbness in both hands. After taking Yu Feng Dan (Wind-Dispelling Pills) and Tian Ma Wan (Gastrodia Pill), she developed generalized numbness, mental fatigue, afternoon fever with thirst, spontaneous sweating, and night sweats. This indicated deficiency of both qi and blood. She took Shi Quan Da Bu Tang with Pao Jiang for several doses, after which all symptoms subsided. Pao Jiang was then removed, and she took the formula for a few more doses until recovery. However, internal heat remained, which was attributed to blood deficiency.
