In women, blood-wind sores are caused by wind-heat stagnating and transforming into fire in the Liver and Spleen meridians, leading to dryness of the blood. Externally, symptoms include nodules appearing on the body, resembling erysipelas in some cases, with intermittent itching and pain; scratching breaks the skin causing sores that weep pus and fluid continuously. Internally, symptoms include irregular menstruation, dysuria, night fever, internal heat sensation, spontaneous sweating during the day, and nocturnal sweating at night, aversion to cold, limb fatigue, poor appetite, and alternating chills and fever. If fever is accompanied by pain, it indicates wind-heat in the Liver meridian causing blood dryness; use Danggui Yin (Angelica Decoction) added with Chaihu (Bupleuri Radix) and Shanzhi (Gardeniae Fructus). If there is limb fatigue, reduced appetite, dry mouth, and tidal fever, it indicates Liver-Spleen stagnant fire damaging the blood; use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder). If nodules are painful and itchy with alternating chills and fever, it indicates Liver wind-heat damaging the blood; use Xiao Chaihu Tang (Minor Bupleurum Decoction) added with Shanzhi (Gardeniae Fructus) and Huanglian (Coptidis Rhizoma). If there is night fever, thirst, and delirium, it indicates heat entering the Blood Chamber; use Xiao Chaihu Tang (Minor Bupleurum Decoction) added with Shengdihuang (Rehmanniae Radix); for blood deficiency, combine Siwu Tang (Four Substances Decoction) with Xiao Chaihu Tang (Minor Bupleurum Decoction). If there is insomnia and nocturnal sweating with internal heat and afternoon fever, it indicates Spleen blood deficiency; use Guipi Tang (Restore the Spleen Decoction); if accompanied by chills and fever, add Shanzhi (Gardeniae Fructus) and Shudihuang (Prepared Rehmanniae Radix). Using wind-dispelling drugs will further damage Yin and blood, potentially causing other complications.
Case Record 1:
A woman accustomed to a frugal life developed this condition on her limbs. She mistakenly used cooling and detoxifying herbs, resulting in afternoon fever, internal heat, spontaneous and nocturnal sweating, amenorrhea, dry mouth, and dry throat. This indicated stagnation injuring the Spleen (the limbs are governed by the Spleen). She was treated with several doses of Guipi Tang (Restore the Spleen Decoction), followed by concurrent administration of Xiaoyao San (Free and Easy Wanderer Powder). After more than fifty doses, she recovered.
Case Record 2:
A woman with an irritable temperament suffered from alternating chills and fever, bitter taste, hypochondriac pain, tinnitus, abdominal distension, and dysuria for over a year. The syndrome belonged to Liver fire. Initially, she received several doses of Sijunzi Tang (Four Gentlemen Decoction) added with Chaihu (Bupleuri Radix), fried Shanzhi (Gardeniae Fructus), and fried Longdancao (Gentianae Radix). Then, Xiaoyao San (Free and Easy Wanderer Powder) was administered concurrently until the sores healed. Subsequently, Liuwei Wan (Six-Ingredient Pill) and Xiaoyao San (Free and Easy Wanderer Powder) were given for over seventy doses, resolving all symptoms. If white scales appeared after recovery and scratching felt like fabric separating the skin, it indicated Qi and blood deficiency failing to nourish the Couli (interstices); large tonifying agents should be used. If fever recurred after recovery with nodules, itching, pain, weeping sores, and irregular menstruation, it indicated Liver fire and blood heat; Siwu Tang (Four Substances Decoction) was used added with Chaihu (Bupleuri Radix), Shanzhi (Gardeniae Fructus), Baizhu (Atractylodis Macrocephalae Rhizoma), Fuling (Poria), Danpi (Moutan Cortex), and Gancao (Glycyrrhizae Radix et Rhizoma).
Case Record 3:
A woman experienced whole-body itching every afternoon for over a month, followed by dry mouth for another month, and then sores formed. After taking wind-dispelling agents, pus flowed continuously. She had chills before noon and fever after noon, resembling wind syndromes. I diagnosed this as Liver stagnant fire combined with external pathogens. She was treated with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Shanzhi (Gardeniae Fructus) and Gouteng (Uncariae Ramulus cum Uncis). She recovered after being treated with Xiaoyao San (Free and Easy Wanderer Powder) added with Chuanxiong (Ligustici Chuanxiong Rhizoma) and Beimu (Fritillariae Thunbergii Bulbus).
Case Record 4:
A woman suffered from itching and fever, worsening in the afternoon, with red marks on the skin and delayed menstruation. This indicated blood deficiency with heat. Xiaoyao San (Free and Easy Wanderer Powder) was used with doubled Shudihuang (Prepared Rehmanniae Radix). Once the heat subsided and itching stopped, she was regulated with Siwu Tang (Four Substances Decoction) added with Renshen (Ginseng Radix), [missing character], Chaihu (Bupleuri Radix), Zhi Gancao (Honey-fried Glycyrrhizae Radix et Rhizoma), and Fuling (Poria), leading to recovery.
Case Record 5:
A twelve-year-old girl who was prone to anger suffered from whole-body itching. Chaihu (Bupleuri Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Shanzhi (Gardeniae Fructus), and Shaoyao (Paeoniae Radix) were used to clear Liver fire; Shengdihuang (Rehmanniae Radix), Danggui (Angelicae Sinensis Radix), and Huangqin (Scutellariae Radix) were used to cool Liver blood; Baizhu (Atractylodis Macrocephalae Rhizoma), Fuling (Poria), and Gancao (Glycyrrhizae Radix et Rhizoma) were used to strengthen the Spleen Earth, resulting in recovery. Half a year later, red marks appeared all over her body, accompanied by occasional dizziness. This indicated excessive Liver fire causing blood to move recklessly due to heat; indeed, menstruation arrived that night.
