In women, swelling and pain in the hypochondrium and lower abdomen, or swelling and pain of the Yumen (external genitalia), accompanied by aversion to cold with high fever, dysuria, acute abdominal pain, or a sensation of fullness in the lower abdomen attacking upward to the hypochondrium, indicates stagnation of damp-heat in the Liver channel. Longdan Xiegan Tang (Gentiana Decoction to Drain the Liver) should be used. If there is only swelling of the Yumen, it is due to Liver fire and blood deficiency; Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) should be used. To indiscriminately administer blood-invigorating and toxin-resolving agents would be a mistake.
Swelling and pain in the heels and toes of women, along with heat in the soles of the feet, are caused by improper care during pregnancy, childbirth, or menstruation, leading to deficiency of the three Yin channels of the foot and resulting in deficient heat. If swelling and pain occur with suppuration, Liujun Wan (Six-Ingredient Rehmannia Pill) should be the primary formula, assisted by Bazhen Tang (Eight-Treasure Decoction). If there is poor appetite due to Spleen-Stomach deficiency, Liu Junzi Tang (Six-Gentlemen Decoction) should be added. If there are alternating chills and fever with internal heat, Xiaoyao San (Free and Easy Wanderer Powder) should be added. If afternoon tidal fever worsens with cloudiness of the head and eyes, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) should be added. All conditions such as fever, afternoon tidal fever, internal heat, spontaneous sweating, and night sweats belong to false heat caused by Yin deficiency. Therefore, Danxi stated that "fire rises from the nine springs," referring to extreme Yin deficiency. The heel is the origin of the Du vessel (Conception Vessel's counterpart on the back), the pathway of the Kidney channel, and the foundation bearing all bones. If one does not seek their root affiliation and indiscriminately uses cold and bitter drugs, many will suffer premature death. Men who overindulge in alcohol and sexual activity often suffer from this condition.
When the ten toes of women feel as if scalded by hot oil, it is due to deficiency of Ying and Wei (Nutritive and Defensive) qi, with damp-toxin qi stagnating in the meridians. Attacking upward to the Heart causes heart pain; attacking downward to the feet causes foot pain, with toes burning like fire. This is similar to Jiaoqi (Foot Qi disease); the Classic states: "This is Rejue (Heat Jue)."
Sores appearing on the external genitalia of women may result from improper regulation after pregnancy or childbirth damaging the Spleen and Stomach, or from worry, thought, depression, and anger damaging the Liver and Spleen, causing damp-heat to sink downward. The exterior belongs to the three Yang channels of the foot and is easy to treat; the interior belongs to the three Yin channels of the foot and is difficult to cure. If initially swollen, red, and painful, it is caused by damp-heat toxin invasion; Renshen Baidu San (Ginseng Rescue the Extremities Powder) should be used. If there is diffuse swelling and pain, or no swelling and no pain, it is due to Spleen deficiency with damp-heat sinking down; Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) or Bazhen Tang (Eight-Treasure Decoction) should be used, with additional herbs such as Bibi (Dioscorea Hypoglauca Rhizome) and Jinyinhua (Lonicerae Flos). If pus and water drip continuously, with fatigue, poor appetite, internal heat, and dry mouth, it is due to Spleen qi deficiency; Buzhong Tang (Tonify the Middle Decoction) should be used with added Fuling (Poria) and Jiushao (Wine-processed White Peony Root). If there is afternoon fever with fatigue, it is due to blood deficiency; add Chuanxiong (Ligusticum Chuanxiong Rhizome) and Shudi (Prepared Rehmannia Root) to the previous decoction, or use Liujun Wan (Six-Ingredient Rehmannia Pill). If there is aversion to cold in the limbs and reduced desire for food, it is due to Spleen-Kidney deficiency-cold; Shiquan Daybu Tang (Ten-Ingredient Great Tonifying Decoction) or Bawei Wan (Eight-Ingredient Pill) should be used. Red color indicates heat toxin and is easy to treat; dark color indicates deficiency-cold and is difficult to treat.
