Huoluan (Acute Gastroenteritis with Vomiting and Diarrhea) arises from excessive consumption of sweet and fatty foods, leading to putrefaction and accumulation forming phlegm; or from emotional stagnation where qi excess transforms into fire; these pathogens accumulate in the stomach. Upon sudden exposure to cold or heat pathogens, righteous qi and pathogenic factors contend, causing yin and yang to mix chaotically. This results in severe colic pain in the heart and abdomen, with simultaneous vomiting and diarrhea, characterized by rapid and chaotic changes. If the pathogen is located in the upper jiao (upper epigastrium), there will be heart pain and predominant vomiting; if in the lower jiao (lower epigastrium), there will be periumbilical pain and predominant diarrhea; if in the middle jiao (middle epigastrium), there will be abdominal pain with both vomiting and diarrhea being profuse. Excessive vomiting damages qi, while excessive diarrhea damages blood. When qi and blood are damaged, they cannot protect and nourish the fetus. Pathogenic factors agitate and strike the fetal origin, resulting in inevitable death for both mother and child. This is a critical condition requiring urgent treatment. It is appropriate to use Xiangsu San (Perilla and Bupleurum Powder) added with Huoxiang (Patchouli Herb), administer it first, and then induce vomiting to expel the pathogen.
Diarrhea during pregnancy primarily involves the Spleen and Kidney organs, with deficiency patterns being predominant. Blood is governed by the Spleen; when blood congregates to nourish the fetus, Spleen Yin becomes deficient, impairing food transformation and transportation. The Spleen governs robust transportation; if the lower jiao is obstructed, clear qi struggles to diffuse, making water and grain difficult to digest, leading to diarrhea. Furthermore, the fetus is anchored to the Kidney; the fetus draws upon maternal qi for support, weakening Kidney Qi. With the decline of Mingmen Fire (Gate of Life Fire), it fails to steam and warm Spleen Earth upwards, which is the root cause of diarrhea during pregnancy. Although external contractions of wind, cold, summer-heat, and dampness, or internal injuries from raw and cold food may occur, the underlying etiology lies in Spleen and Kidney deficiency.
Food retention during pregnancy often stems from weakness of Middle Qi in the Spleen and Stomach, impairing transformation and transportation; if Middle Qi were robust, this disease would not occur. The fetus relies on the Spleen and Stomach as its foundation; strong Spleen and Stomach function keeps the fetus secure like a suspended bell, whereas food retention causes Spleen distress and fetal insecurity. Therefore, in cases requiring digestion promotion and stasis resolution, the primary approach must be tonifying the Spleen and strengthening the Stomach. By promoting Grain Qi, food digests naturally. Merely employing digestive and purgative methods not only risks fetal expulsion but also exacerbates Spleen deficiency, exhausting the source of transformation.
Internal injury from seven emotions and external contraction of six climatic pathogens can both lead to hemorrhage. Hematemesis (vomiting blood) during pregnancy is primarily attributed to Heat-Fire. Since Qi and Blood congregate to nourish the fetus, any disturbance can cause Qi rebellion, allowing Fire to ascend to the Heart. This results in mental restlessness and chest fullness, with blood overflowing following the Qi. However, Fire has slight distinctions between Excess and Deficiency. Excess Fire requires clearing Heat to nourish Blood; Deficient Fire requires nourishing Yin to supplement Water. In this manner, Blood can be stabilized and the fetus secured. Indiscriminate use of blood-invigorating or blood-breaking agents will inevitably cause miscarriage, bringing disaster swiftly.
