The term "Zu" (Obstruction) refers to the situation where menstrual blood has ceased, phlegm and fluid accumulate in the middle, obstructing the Qi of the Zang organs, preventing its free circulation.
The *Jin Gui Yao Lue* states: When a woman presents with a normal pulse, but the Yin pulse (Chi position) is slightly weak, and she experiences thirst and inability to eat, without aversion to cold or fever, this is named Pregnancy. According to the method, around sixty days, this syndrome should appear. If a physician treats it incorrectly, delaying by a month and exacerbating vomiting and diarrhea, one should "Jue Zhi" (Stop treatment). (Lou Quanshan said: "E Zu" refers to vomiting, nausea, dizziness, food aversion, and selective eating. "Jue Zhi" means to cease medical intervention and wait for self-recovery. I once treated one or two pregnant women with E Zu; the more they were treated, the worse their vomiting became. Reflecting on Zhang Zhongjing's principle of "Jue Zhi," I stopped medication for over a month, and they recovered naturally. Great is the Way of the Sages! Xu Zhongke said: If the time is less than sixty days, the fetus is not yet formed, and vomiting/diarrhea occur. Because physicians focus on calming the fetus, using herbs like Huang Qin (Scutellaria Root) and Sha Ren (Amomum Fruit), the Spleen and Stomach may be injured. One should focus on eliminating the root cause rather than adhering strictly to fetal calming, hence "Jue Zhi.")
The food eaten by women transforms into Blood and Qi, flowing downward as menstruation. In early pregnancy, when menstruation suddenly stops, the first month is called "Mei," the second "Pei," and the third "Tai." Once formed, the male or female fetus nourishes itself from the mother. However, during the third month, when the Ministerial Fire transforms the fetus, the fetus cannot yet draw nutrition from the mother. The Blood and Qi are not yet heavily utilized, and the five grains are not fully digested. The Qi of the Middle Jiao becomes stagnant and full. All stagnation, Phlegm-Fire, and foul odors rush to the Stomach, causing nausea and obstruction. Symptoms include mental restlessness, dizziness, flaccidity of the four limbs, aversion to the smell of food, excessive sleep with little rising, and a strong craving for sour and salty fruits. This indicates insufficiency of the Liver and Kidney, drawing these flavors for self-rescue.
In the second and third months of pregnancy, if the pregnant woman has a weak constitution, she will suffer from E Zu disease: vomiting, inability to eat, mental irritability resembling drunkenness, heaviness of the limbs, selective eating, food aversion, and dizziness. This occurs because Blood and Qi accumulate to nourish the Fetal Origin. The essence and Blood stagnate internally, and foul/rotten Qi attacks the Stomach upward, causing vomiting and inability to accept food. Since Blood nourishes the fetus, the Heart loses nourishment, leading to Heart deficiency and mental irritability. The treatment method should regulate Blood and dissipate stagnation. Use Renshen (Ginseng Root), Baizhu (Atractylodes Rhizome), and Gancao (Licorice Root) to tonify Middle Qi; use Juhong (Tangerine Red Peel), Zisu (Perilla Leaf), Muxiang (Costus Root), and Shengjiang (Fresh Ginger) to disperse stagnant Qi; use Fuling (Poria), Maidong (Ophiopogon Tuber), Huang Qin (Scutellaria Root), and Zhuru (Bamboo Shavings) to clear heat and relieve irritability. This formula is named Shen Ju Yin (Ginseng and Tangerine Peel Decoction), illustrating the principle that "during pregnancy, Qi must be smoothed." Generally, there is no Cold before pregnancy and no Heat after pregnancy. However, E Zu vomiting mostly belongs to Heat. Yet, some vomiting is caused by Cold, which is due to illness rather than E Zu. If unchecked, it leads to deficiency. One should treat with Renshen Gan Jiang Ban Xia Wan (Ginseng, Dried Ginger, and Pinellia Pill) and avoid excessive use of acrid/warm herbs. If vomiting is severe and stopping it makes it more urgent, follow Zhang Zhongjing's method: stop medication for over a month, and it will settle naturally.
The *Qian Jin Fang* contains Ban Xia Fu Ling Tang (Pinellia and Poria Decoction) and Fu Ling Wan (Poria Pill), specifically treating E Zu. Recently, few people take them, fearing that Ban Xia (Pinellia Tuber) disturbs the fetus. Since the fetus is newly formed, its acrid-dry nature might scatter the fetus. It must be stir-fried with Jiang Zhi (Ginger Juice) to mitigate toxicity. E Zu cannot be stopped without Ban Xia, so Zhang Zhongjing used Renshen Ban Xia Gan Jiang Wan (Ginseng, Pinellia, and Dried Ginger Pill). Luo Qianfu used Er Chen Tang (Two-Cured Decoction) removing Chen Pi (Tangerine Peel) and Gancao (Licorice Root), naming it Ban Xia Fu Ling Tang. Zhu Danxi stated that fat people often have Phlegm, while thin people often have Fire. He used modified Er Chen Tang to treat pre-pregnancy E Zu, Phlegm-reversal vomiting, mental irritability, dizziness, and food aversion, all effectively. The Classic says, "If there is an existing pathogen, there will be no injury," which applies here. Xue Lizhai said: Ban Xia is the main herb for strengthening Spleen Qi and resolving Phlegm stagnation. For Spleen-Stomach weakness with vomiting, or Phlegm-saliva congestion with poor appetite and unstable fetus, Ban Xia Fu Ling Tang must be used, with doubled Baizhu (Atractylodes Rhizome) to calm the fetus and strengthen the Spleen. It is commonly very effective. For E Zu accompanied by lower back pain, to prevent miscarriage, Er Chen Tang combined with Si Wu Tang (Four-Substance Decoction) is especially suitable, adding Huang Qin (Scutellaria Root) and Baizhu (Atractylodes Rhizome), focusing on harmonizing the Middle and regulating the Spleen. Ascending herbs should be avoided. Since vomiting involves ascending Qi, using ascending drugs would violate the principle of "ascending only, descending never," making the Upper fuller and the Lower emptier, thereby hastening miscarriage.
