According to *Yilu Buiyi* (Supplemental Records of Medical Records): When a woman's Blood Sea is full, menstruation occurs. However, women's temperaments are ten times more stubborn than men's. Even if there are deficiency syndromes requiring tonification, one must consider the issue of stubbornness. Moreover, amenorrhea largely belongs to blood deficiency and qi stagnation.
Luo Longji stated: The medical classics record a condition characterized by distension and fullness in the chest and hypochondrium, interfering with food intake. Upon onset, the patient first smells a fishy or foul odor, secretes clear fluid, experiences hematemesis, cold limbs, dizziness, and frequent bleeding from both the anterior and posterior orifices. This disease is named "Blood Withering" (Xue Ku). This occurs because in youth, due to massive blood loss or intercourse after intoxication, the Kidney and Liver are depleted. Since the Liver stores blood and receives the Qi of Heaven One to provide nourishment, its meridian passes through the diaphragm and spreads over the hypochondrium. If blood is lost and essence is leaked, the Liver Qi is injured, and Liver blood becomes withered and unable to nourish, leading to chest and hypochondriac fullness. Interference with eating indicates that Liver disease transmits to the Spleen, resulting in the perception of fishy/foul odors and secretion of clear fluids. If Lung Metal attacks due to Liver disease, hematemesis occurs. Cold limbs, dizziness, and frequent bleeding from anterior and posterior orifices are all signs of Liver blood injury.
Yu Tianmin stated: All diseases in women originate from the Heart. When the Fire of the Five Emotions arises, Heart Fire burns along with it. In cases of amenorrhea and obstruction, it initially stems from insufficient mental care, depleting Heart Blood, thus lacking blood to return to the Liver, exhausting its functions of reception and discharge. The classics state, "The Mother can cause the Child to be deficient"; therefore, the Spleen fails to transform and transport, leading to poor appetite. This is what is meant by "Diseases of the Two Yangs originating from the Heart and Spleen." Due to poor appetite, Lung Qi also loses nourishment. Qi stagnation prevents the nourishment of Kidney Yin. Furthermore, menstrual water relies entirely on the transformation and distribution of Kidney Water. Once Kidney Water is scarce, menstrual flow dries up daily, occurring irregularly or dripping continuously without time limits. If not treated early, it gradually progresses to complete obstruction, becoming an extreme consumption syndrome that is difficult to treat.
Xu Chunfu stated: The Heart belongs to Yang and governs Blood, while the Spleen envelops Blood to move Qi. If menstruation is obstructed, it may well be due to insufficient mental care, where excessive thinking injures the Spleen Qi, accompanied by fatigue and weariness. Grain Qi is not distributed, Lung Metal loses nourishment, and Kidney Water lacks moistening. Consequently, menstrual blood withers, leading to irregular cycles (every three or five days/months) and eventually complete cessation. Deficiency damage with internal heat, bone-steaming tuberculosis, and other consumptive diseases arise, making them generally difficult to treat. Only by nourishing the Heart will Blood generate; only by strengthening the Spleen will Qi distribute. When these two harmonize, Qi flows smoothly and Blood moves, achieving the essential principle of regulating menstruation.
