Chen Liangfu stated: Women are primarily governed by blood. If the Spleen and Stomach are weak, leading to poor appetite, the Ying (Nutritive) and Wei (Defensive) Qi will be insufficient, resulting in amenorrhea, alternating chills and fever, abdominal pain, or conditions such as metrorrhagia and leukorrhea. All these are diseases caused by Spleen-Stomach deficiency. Therefore, if a woman suffers from amenorrhea due to overexertion or blood loss damaging the Liver and Spleen, nourishing the source of transformation and generation will naturally restore menstruation. If there is difficulty in urination, severe dizziness, lower back pain, cold feet with intermittent pain, and prolonged blood stasis internally forming masses (Zhang), it indicates internal pathology. If blood and water combine, and Spleen-Stomach weakness causes stagnation and obstruction, edema may develop. If Spleen Qi is weak and fails to control water, causing water to infiltrate the muscles, swelling and fullness may occur. Treatment should focus on replenishing body fluids and greatly tonifying the Spleen and Stomach.
Wu Shuqing stated: The Spleen Qi transforms fluids to generate blood, which aligns with the concept that "when water enters the meridians, blood is generated." This refers to the origin of Ying Qi emerging from the Middle Jiao, hence termed the "source of transformation and generation." The Heart governs the blood vessels, but blood formation relies on Spleen Qi transforming fluids into blood, which then enters the Heart. Thus, although the Heart governs blood, its generation depends on Spleen Qi. Consequently, for irregular menstruation in women, one must examine the source of Spleen Qi's transformation and generation; strengthening the Spleen is the key to regulating menstruation.
He Song stated: The Dao of Qian creates men, and the Dao of Kun creates women. Women operate according to the Dao of Kun, so treating female disorders focuses on Yin Blood. At age fourteen (two sevens), Tian Gui arrives in girls, Yin Qi becomes abundant, and when abundant, it flows out; this is natural. If Yin nature is robust, menstruation begins before fourteen; if weak, it begins after fourteen. In treating such cases, one must assess the condition of the Spleen and Stomach. For instance, if menstrual irregularities are caused by Spleen-Stomach damage failing to generate blood, priority must be given to nourishing the Spleen and Stomach. When the Spleen is strong, it can control blood, and menstruation will normalize. One must not hastily use attacking agents that damage Middle Qi, as this would worsen the irregularity.
Shen Zhai commented: The above three passages outline the major principle of regulating menstruation by nourishing the Spleen and Stomach. Zhao Yangkui’s two points emphasize nourishing Kidney Water to tonify the source of innate True One. Chen Liangfu’s subsequent points emphasize tonifying Earth (Spleen) to cultivate the root of acquired Yuan Qi. Consolidating the Kidneys and supporting the Spleen is the essential path for regulating menstruation; students should carefully consider this.
