[Lou Quanshan] Postpartum extreme panting is critical and often fatal.
[Guo Jizhong] What is the cause of rapid breathing and panting in the throat after childbirth? The answer is: Ying (Nutritive) Qi is blood, and Wei (Defensive) Qi is air/qi. Ying circulates within the vessels, while Wei circulates outside the vessels; moving up and down together, they are called Ying and Wei. Due to excessive blood loss during delivery, Ying blood suddenly depletes, causing Wei qi to lose its master and gather exclusively in the lungs, thus inducing panting. This condition is named "Lone Yang and Absolute Yin" and is difficult to treat. If lochia (postpartum discharge) does not flow smoothly, and stagnant blood coagulates and steams upward to the lungs, it also causes urgent panting. Simply administering Duoming Dan (for retained placenta), once the blood stasis is removed, the panting will naturally subside.
Chen Wuyue commented: Postpartum urgent panting is indeed fearful. If it is caused by stagnant blood steaming upward to the lungs, one can still expect efficacy from Duoming Dan. However, if it is due to contracting wind-cold, or resulting from emotional distress/anger, or consuming salty/cold foods and drinks, Duoming Dan may not be effective, let alone for cases of "Lone Yang and Absolute Yin." If Ying blood suddenly collapses, a large dose of Xiongqi Tang can also save the patient. If injured by wind-cold, Xuanfuhua Tang is appropriate. If caused by emotional depression and stagnation, Xiaotiaojing San is appropriate, taken with a decoction of Sangbaipi and Xingren. If caused by food stagnation, Jian Shi Wan, Wuji San, or Xiongqi Tang are appropriate.
[Xue Ji] For the aforementioned conditions, if Spleen and Lung Qi are deficient, use Liu Jun Zi Tang with Jiegeng. If accompanied by external pathogens, add Zisu. If Middle Qi is deficient and cold, use Buzhong Yiqi Tang with Pao Jiang and Rougui. If Yang Qi is deficient and collapsing, add Fuzi. If blood stasis enters the Lungs, urgently use Erwei Rensu Yin. One postpartum patient suffered from urgent panting, spontaneous sweating, and cold hands and feet, frequently holding her hands over her umbilicus and abdomen. This indicated Yang Qi deficiency and collapse; she was cured after four doses of Shenfu Tang. The wife of Wu Hui from Pujiang had swollen feet during pregnancy. In early July, two months after childbirth, she took a bath and immediately developed panting, being able to sit but not lie down for five months. She feared wind but felt slightly relieved by warmth. Her Guan pulses were active, while Cun and Chi pulses were both deficient. Various medicines were ineffective. A decoction of Mudanpi, Taoren, Guizhi, Fuling, Ganjiang, Zhishi, Houpo, Sangbaipi, Zisu, Wuweizi, and Gualou Ren was administered, providing immediate relief. After two or three doses, she could lie down, and her phlegm disappeared. This was treated as blood stasis complicated by cold invasion.
Xuejie San treats postpartum stagnant blood rushing to the Heart, causing chest fullness and upward panting, with life hanging in the balance. It is suitable for administration.
Zhen Xuemei (if unavailable, substitute with Zijian) Moya
Equal parts of the above, ground into fine powder, repeatedly ground until completely fine. Take 2 qian per dose, mixed with half a large cup of good wine and child's urine, boiled once, and taken warm. Immediately after delivery, take one dose and go to bed; take another dose after a long interval. The evil blood will naturally descend along the meridians without rushing upward, preventing various diseases.
Yunqi Rensu Yin treats postpartum blood entering the Lungs, presenting with blackish complexion and panting approaching death.
Renshen (1 liang, ground into powder) Sumu (2 liang)
Boil Sumu in two bowls of water until reduced to less than one bowl. Remove the dregs and mix in the Renshen powder. Adjust dosage according to circumstances. The effect is miraculous.
Xuanfuhua Tang treats postpartum wind-cold injury, presenting with panting, cough, abundant phlegm-saliva, and restlessness in sitting or lying.
Xuanfuhua Chishao Yao Jingjiesui Banxiaqu Qianhu Gancao (roasted) Fuling Wuweizi Xingren (skin and tip removed, stir-fried with bran) Mahuang (equal parts). In another prescription, Chishao Yao and Fuling are doubled.
Cut into pieces. Take 4 qian per dose with 1.5 cups of water, 3 slices of Shengjiang, and one Zao. Decoct until 0.7 remains, remove dregs, and take warm before meals. (Not suitable for those with sweating.)
If panting is caused by consuming salty, frozen drinks or food, Jian Shi Wan is appropriate. (See previous section on Chest Fullness and Stagnation.)
Wuweizi Tang treats postpartum urgent panting with deep, hidden pulse and cold extremities.
Wuweizi (pounded, stir-fried) Renshen Xingren (each 2 qian) Maidong (heart removed) Chenpi (each 1 qian)
Add 3 slices of Shengjiang and two Zao. Decoct in water and take.
Dabu Tang treats postpartum cases beyond one hundred days, presenting with bluish-green facial swelling, pale lips, urgent breathing with sweating. This is a severe deficiency syndrome, and this formula should be taken urgently.
Danggui Tou Dachuanyuxiong Baizhu Baishao Bai Fuling (use more) Renshen (use more) Huangqi (use more) Wuweizi Shudihuang Ganjiang (appropriate amount) Gancao (small amount)
Cut into pieces, decoct in water, and take. If symptoms do not recede after two doses, add Chuanwu and Muxiang, grind separately, and mix into the medicine. If diarrhea occurs, add Hezi, Roudoukou, and Yingsuke.
