[Yan Yonghe] Chao Yuanfang, in *Zhu Bing Yuan Hou Lun* (Treatise on the Origins and Manifestations of All Diseases), discusses that women have thirty-six ailments, including seven *jia* masses, eight *jia* masses, nine pains, and twelve types of leukorrhea. However, leukorrhea is not explicitly defined by its symptoms; nowadays people only know of red and white leukorrhea. This arises from labor injury to the Chong and Ren meridians, with wind-cold pathogens occupying the uterine collaterals. In healthy women, blood should be abundant while Qi should be relatively scarce, preventing various diseases. If Qi exceeds Blood, excess Qi generates cold; Blood fails to transform into red, resulting in white leukorrhea. If Qi is balanced but Blood is deficient, deficiency of Blood generates heat; Blood fails to transform into menstrual blood, resulting in red leukorrhea. When cold and heat intermingle, both red and white discharge occur. The pulse at the right Chi position is floating; floating indicates Yang. If Yang is exhausted, conception is impossible. If accompanied by cold feet and leukorrhea, mild cases present as spotting (non-menstrual light bleeding), while severe cases present as metrorrhagia (non-menstrual heavy bleeding). These are caused by the Heart failing to nourish Blood and the Liver failing to store Blood. The pulse at the Cun position is wiry and large; wiry indicates reduction (deficiency of Yang), and large indicates hollow (deficiency of Yin/Blood). Reduction implies cold, and hollow implies deficiency. When cold and deficiency combine, the pulse becomes革 (leather-like), indicating spontaneous abortion or spotting. Additionally, deficiency pulses at Cun and Chi indicate bleeding; if the pulse is floating during bleeding, it is difficult to treat.
*[Chanbao]* The thirty-six leukorrhea ailments refer to twelve *zheng* masses, nine pains, seven harms, five injuries, and three consolidations, collectively called the thirty-six ailments. The twelve *zheng* masses refer to the discharged substances: one like paste, two like green blood, three like purple juice, four like red skin, five like pus scabs, six like bean curd dregs, seven like mallow soup, eight like clotted blood, nine like clear water-like blood, ten like rice washings, eleven like menstrual washing residue, and twelve like menstruation arriving out of schedule. The nine pains are: one pain in the yin, two painful urination in the yin, three pain during urination, four cold pain, five abdominal pain during menstruation, six foot pain when Qi distension occurs during menstruation, seven sweating with insect-biting pain in the yin, eight pain in the hypochondriac skin, and nine lower back pain. The seven harms are: harm to food intake, harm to Qi movement, harm to cold, harm to labor, harm to intercourse, harm to pregnancy, and harm to sleep. The five injuries are: one orifice pain, two cold pain, three lower abdominal pain, four organ insensibility, and five misalignment of the cervical os referring pain to the back. The three consolidations refer to amenorrhea due to blockage; the other two are missing in the text. Zhang Zhongjing's thirty-six ailments all arise from cold-heat and labor damage to the uterus, leading to leukorrhea originating internally in the yin.
[Luo Tianyi] For red and white leukorrhea, Master Luo's methods include using Shizao Tang (Cantaloupe Seed Decoction) [see Shaoyang Hypochondriac Fullness Disease], Shenyuan Wan (Divine Protection Pill) [see Miscellaneous Diseases Phlegm-Fluids], or Yuzhu San (Jade Candle Powder), which is Siwu Tang (Four Substances Decoction) added with Tiaowei Chengqi Tang (Regulate the Stomach and Order the Qi Decoction). All are applicable. Deficient patterns must not be treated with harsh purgation; excess patterns can undergo treatment. For Blood deficiency, use Modified Siwu Tang. For Qi deficiency, alternate Baidi Tang (White Flower Decoction - note: context implies Bai Zhu, Ren Shen, Chen Pi mixture) with Atractylodis Rhizoma, Ginseng Radix, and Citri Reticulatae Pericarpium. Red belongs to Blood, white belongs to Qi; the primary treatment is drying dampness. For severe dampness, use Guchang Wan (Intestine Consolidating Pill) [see later]. If Minister Fire moves, add fried Coptidis Rhizoma (Huangbai) to the formulas. For slippery discharge, add Dragon Bone and Red Stone Lip. For stagnation, add Hibiscus flower (white for white leukorrhea, red for red leukorrhea). For irritable nature, add Coptidis Rhizoma (Huanglian). In cold months, add small amounts of Zingiberis Rhizoma (Shengjiang) and Aconiti Lateralis Radix Praeparata (Fuzi). Adapt to circumstances, first cutting off rich flavors. Leukorrhea is treated similarly to nocturnal emission. Women with obesity having leukorrhea often have damp-phlegm; use Haifushi (Fluitans Lapillus), Banxia (Pinelliae Rhizoma), Nanxing (Arisaematis Rhizoma), fried Huangbai, Qingdai (Indigo Naturalis), Cangzhu (Atractylodis Rhizoma), and Chuanxiong (Chuanxiong Rhizoma). Thin women rarely have leukorrhea; if they do, it is mostly heat; use fried Huangbai, Ge Fen (Concha Meretricis), Huashi (Talcum), Chuanxiong, Qingdai, and Chupipi (Phellodendri Cortex). Leukorrhea and spotting are both phlegm accumulations in the Stomach flowing down to permeate the Bladder and exit through the Large and Small Intestines. Ascending and lifting is indicated; for severe cases, induce vomiting above to lift Qi, and below use Erchen Tang (Two-Cured Decoction) added with Baizhu and Cangzhu, still in pill form.
[Dai Sigong] Red and white leukorrhea all result from internal injury by the Seven Emotions or deficiency-cold of the Lower Origin, with causes not limited to one aspect. Generally, white leukorrhea is common, with occasional red discharge. Both are suitable for Shunqi San (Qi-Smoothening Powder) swallowed with Zhenling Dan (Spirit-Shaking Pill), assisted by Aifu Wan (Artemisia and Ligusticum Pill), or Sandaren powder mixed with rice water. If leukorrhea persists causing emaciation, use Siwu Tang added with half a qian of Oyster Shell powder, swallowed with Guyang Wan (Yang-Consolidating Pill); take frequently for effect. (Guyang Wan formula see Miscellaneous Diseases Nocturnal Emission section.) Some leukorrhea cures relapse after one or two months, or every half year, starting with blood followed by discharge, unstoppable, accumulating briefly then pouring out again; this is named Loudai (Leak-Leukorrhea), most difficult to treat. Blood from the lower section is governed by the lower abdomen. Due to Blood deficiency, deficient heat sinks into the Small Intestine, causing dysuria with pain, white color like rice water, or forming sand-like grains; these must not be treated as strangury with cold agents. Use Siwu Tang and Wuling San (Five-Ling Drink) half doses each, decocted together.
[Li Chen] Women's leukorrhea has five names, caused by wind pathogens entering the Uterine Gate during menstruation or postpartum, transmitting to the Zang-Fu. If the Foot Jueyin Liver Meridian is injured, color is like green mud; if the Hand Shaoyin Heart Meridian is injured, color is like red saliva; if the Hand Taiyin Lung Meridian is injured, shape is like white snot; if the Foot Taiyin Spleen Meridian is injured, yellow like rotten melon; if the Foot Shaoyin Kidney Meridian is injured, black like clotted blood. Humans have the Dai Mai (Girdle Vessel) crossing the waist like a belt; disease arising here is named Dai (Leukorrhea).
[Xue Ji] Mr. Xu Yongcheng said: Previous symptoms, white belongs to Qi and red belongs to Blood. Mr. Dongyuan said: Long-term blood metrorrhagia leads to Yang loss, thus white slippery substances flow down; this is not necessarily confined to the Dai Mai. There are also damp-phlegm flowing to the Lower Jiao, or excessive dampness from Kidney/Liver Yin, or Wood overacting on Earth due to fright/panic causing turbid fluids to flow down, or longing causing sinew flaccidity. Dairen used Xuan Dao method (inducing vomiting/purgation) for slippery-large-forceful pulses, draining excess. Dongyuan used Bu Yang Tiao Jing (Tonify Yang Regulate Menses) for fine-deep-tight or large-empty pulses, addressing deficiency. Danxi used Haishi, Nanxing, Chun Gen Pi, etc., treating damp-phlegm. I believe previous symptoms should primarily strengthen Spleen/Stomach and raise Yang Qi, assisted by herbs for specific meridian manifestations. Green belongs to Liver, use Xiao Chai Hu Tang (Minor Bupleurum Decoction) plus Zhizi and Fangfeng. Damp-heat congestion with red-dysuria, use Longdan Xiegan Tang (Gentian Drain Liver Decoction). Liver Blood deficiency or dry-heat/wind-heat, use Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia). Red belongs to Heart, use Xiao Chai Hu Tang plus Huanglian, Zhizi, Danggui. Overthinking injury, use Miaoxiang San. White belongs to Lung, use Buzhong Yiqi Tang (Middle-Warmer Boosting Qi Decoction) plus Zhizi. Yellow belongs to Spleen, use Liujunzi Tang (Six-Gentlemen Decoction) plus Zhizi, Chaihu; if ineffective, use Guipi Tang (Restore the Spleen Decoction). Black belongs to Kidney, use Liuwei Dihuang Wan. Qi and Blood dual deficiency, use Bazhen Tang (Eight-Treasure Decoction). Yang Qi sinking, use Buzhong Yiqi Tang. Damp-phlegm sinking, previous decoction plus Fuling, Banxia, Cangzhu, Huangbai. Qi deficiency with phlegm-fluids sinking, use Sizhi Tang sending Liuwei Dihuang Wan. Do not rigidly adhere to "fat people have much phlegm, thin people have much fire," lightly treating with drying-damp/fire-draining herbs.
[Zhang Congzheng] For leukorrhea with lower abdominal knotting and pain, first purge with Shizao Tang; then regulate with Kulian Wan (Melia Pill) and Da Xuanhu San (Large Corydalis Powder). This is first attack then tonify method. For knotted phlegm white leukorrhea, use Xiaowei Dan (Small Stomach Pill), taken with saliva between meals, wait for accumulation to move before taking tonics. Baizhu (1 liang), Cangzhu (0.5 liang), Hongbai Kuihua (Red/White Hibiscus Flowers, 2.5 qian), Baishaoyao (7.5 qian). Steam cake pills, empty stomach, take 20 pills with decocted Siwu Tang. Tao Zundao's aunt, age 70, thin but good appetite, white leukorrhea. Before meals, swallow 50-100 Dabu Wan (Great Tonifying Pills) with Ginger Water; after lunch and before bed, take 15 Xiaowei Dan each, cured.
[Zhang Zijiu] Recently, a woman from Dunqiu suffered continuous leukorrhea with white discharge for three years. I palpated her pulse; both hands were slippery, large, and forceful, 6-7 beats per breath. She often had upper heat, dry mouth, dizziness, and occasional vomiting of sour fluid. I knew there was actual cold-phlegm in the chest. Using Gudi San (Melon Sprout Powder), I vomited 2-3 sheng of cold phlegm, mostly sour fluid, sometimes yellow saliva like rotten glue. Then I nourished Stomach Qi with grain porridge, followed by Daoyu Gong San (Water-Deriving Yu-Gong Powder) to purge below, then used mild diuretics to benefit water pathways. Cured in a few days. Wife of Li Zuoya from Xicheng suffered white leukorrhea like leaking well water, continuous, foul-smelling, yellow face, reduced appetite for three years. All doctors claimed accumulated cold, using dense dry-tonics like Qiyanshi, Liuhuang, Jiang, Fu, increasing turbid water. Dairen diagnosed: This turbid water is originally heat invading Taiyang Meridian, cold water cannot consolidate, hence this. Water flows high to low; cut the source above first. Vomited 2-3 sheng phlegm; next day purged over a dou of water, repeated three times, sweating profusely. Next morning patient said: Filth stopped flowing. Then used cold-cool agents for half a year, delivered a son.
[Drying Agents] For damp-phlegm weakness unable to withstand attack, dry it. Hot-damp needs cool-drying; cold-damp needs warm-drying.
Chupi Wan (Toona Pill) treats red/white leukorrhea with damp-heat. Shaoyao (5 qian), Liangjiang (3 qian, burnt to ash), Huangbai (2 qian, fried to ash), Chun Genpi (1.5 liang). Powder, porridge pills. Take 30-50 pills empty stomach with rice water.
Red/white leukorrhea due to excess damp-heat: Cangzhu (salt-fried), Baishaoyao (1 liang each), Zhike (3 qian), Chun Genpi (fried, 2 liang), Ganjiang (baked, 2 liang), Diyü (0.5 liang), Gancao (3 qian), Huashi (1 liang, fried). Powder, porridge pills, rice water.
Treating leukorrhea: Chun Genpi (2 liang), Shenqu (fried), Maipi Qu (fried), Huangbai (fried, 1 liang each), Shaoyao (1.5 liang), Huashi, Zhike (0.5 liang each), Cangzhu (1 liang). Powder, paste pills wutong size. Take 50 pills empty stomach.
Guchang Wan treats damp diarrhea, bloody stool, leukorrhea. After removing old accumulation in Spleen/Stomach, use this to dry lower damp. Not used alone; made into decoction based on condition. Palpated Heart Protector Chi pulse extremely micro, white leukorrhea flowing continuously. He Suhe's *Mai Jing* says: Metrorrhagia lasting long becomes white leukorrhea; spotting for long time bones/water wither. Meaning metrorrhagia starts with unceasing blood loss, long duration reduces Blood then loses Yang, thus white slippery substances flow continuously. This is the Blood Sea about to wither, fluids lost, dryness cannot nourish sinews/bones. Use meridian-moving herbs as guides/envoys; use large acrid-sweet-greasy herbs to moisten dryness and generate fluids. Use large acrid-hot aromatics to tonify Yang pathway, generate Blood vessels. Use bitter-cold herbs to drain Lung and rescue upper. Heat injures Qi, tonify with Renshen, assist with slightly bitter-warm herbs to benefit Yuan Qi. Named Bu Jing Gu Zhen Tang.

[Han Zhihe] Mountain wife, 30+ years old, 18 pregnancies, 9 miscarriages, 8 early deaths. During Songpan disaster, brothers fled west, wife overly frightened/anxious, became unconscious, ulcers on lips/tongue/throat, lower part collapse, leukorrhea like pouring. For 40+ days, occasionally awake, wanted to hang herself, couldn't bear grief. Doctors gave cool agents for upper, lower disease worsened. Hot agents/steam for lower, heat fainting near death. Fourth brother returned, palpated, knew it was Yang Collapse syndrome. Cried: Lineage not established, almost killed sister-in-law. Urgently used salt-boiled large Fuzi 9 qian as sovereign, processed with Bohe, Fangfeng, assisted by Jiang, Gui, Xiong, Dang, decocted water dipped in well, given ice cold. Before finishing dose, snoring deep sleep all night, woke recognizing people. At that time only one adopted son, two daughters, hugging crying, relatives sad. Friend Zhao Xianchang shocked: What technique? Brother said: Formulas say, fake against fake, true against true. Upper is fake heat, so follow with fake cold medicine; lower is true cold, so oppose with true hot medicine, thus upper/lower harmonize and disease resolves. Later mainly used Nu Jin Dan (Woman's Golden Pill), complex with 2-3 formulas, not only removed disease but regulated Yuan Qi. Gengwu year born son, now inheriting. Ren Shen year born son. Last year malaria 13 months, also mainly nourishing Yuan Qi/regulating Sheng Qi, wait for food intake increase, then attack with toxic drugs, vomited/purged many chunks, gave Fuzi Tang 3 qian cured, did not demand quick effect. Nu Jin Dan is Shengjin Wan, obtained from strange person. Double Xiangfu, view Qi/Blood bias, add Jianghuang, Tiao Qin, double Chuanxiong etc., many effects. I think childlessness is common, thinking of my difficulty getting children, how bitter. Thus recorded formulas sequentially, including Nu Jin Dan to help people. Xueweng notes. (Nu Jin Dan formula see Seeking Children section).
