此一卷论项筋拘挛强痛。每得斯疾，疗之似易而实难。然方册中所载亦少，纵有言之，

Ephedrae Herba (Mahuang, 麻黄) [roots and nodes not removed] Aconiti Lateralis Radix Preparata (Fuzi, 附子) [prepared, skin removed] Pinelliae Rhizoma Preparatum (Banxia, 半夏) [Note: Text says Nanting, usually refers to Pinelliae Rhizoma or Arisaemae Rhizoma; WHO term for Nanting is Pinelliae Rhizoma Preparatum or Arisaematis Rhizoma. Given context of wind-phlegm, likely Pinellia/Arisaema. Let's use Pinelliae Rhizoma Preparatum as standard for processed Nanting in many contexts, or Arisaematis Rhizoma. I will use Pinelliae Rhizoma Preparatum (Banxia, 半夏) as it is the most common counterpart, but strictly "Nanting" is often Arisaematis Rhizoma. Let's stick to the literal translation of the herb name if ambiguous, but WHO prefers specific Latin. Nanting = Pinelliae Rhizoma Preparatum (Banxia) or Arisaematis Rhizoma. In Song dynasty texts, Nanting often refers to Pinellia. I will use Pinelliae Rhizoma Preparatum.] *Correction*: Actually, Nanting (南星) is typically Arisaematis Rhizoma (Tian Nanxing). I will use Arisaematis Rhizoma (Tian Nanxing, 天南星). Bai Fuzi (白附子) Typhonii Rhizoma (Baifuzi, 白附子) [prepared]. Scorpio (Quanxie, 全蝎) [stir-fried]. Cinnabarum (Chensha, 辰砂) [ground]. Moschus (Shexiang, 麝香) [ground]. Zingiberis Rhizoma Recens Preparatum (Shengjiang/Shengjiang? Text says Bai Jiang, likely Ginger prepared). Zingiberis Rhizoma (Shengjiang, 生姜) or Zingiberis Rhizoma Preparatum [prepared].
Grind the above into a fine powder. Mix with wine and administer one "yi" (a small measure) at a time. After ingestion, remove the pillow and lie down for a short while.
The formula for hemianopia (偏头疼) transmitted by Emperor Shenzong (Yuling) to Wang Anshi (Jinggong) was said to be a secret palace prescription. Use one shell of fresh Raphani Fructus juice (Luofu, radish juice). While lying supine, instill the juice into the nostril corresponding to the side of the pain (left for left, right for right), or into both nostrils. Even if the condition has persisted for decades, it can be cured with a single instillation. Su Dongpo stated: Jinggong told me that this formula had already cured several people. Later, Chao Mingfu also confirmed its efficacy.
This section treats severe, unbearable cephalalgia due to Wind-Cold (Tou Feng Tong).
Natrii Sulfas (Xiaoshi, 硝石). Urinis Sedimentum (Ren Zhong Bai, 人中白). Borneolum Syntheticum (Naozi, 冰片) [equal parts].
Grind the above to an extremely fine powder. Instill one "yi" into the nose.
This volume discusses stiffness and pain of the neck tendons. When contracting such a disease, treatment appears easy but is actually difficult. Records in medical books are scarce; even when mentioned, the discussions are brief. Based on my research, since this disease exists, there must be corresponding formulas. Why did ancients discuss this syndrome so briefly without definitive theories? Upon detailed analysis, it is likely caused by complications from other diseases, hence lacking specialized treatments. During my leisure time, I collected ancient famous formulas for reference. However, Chen Ziming's knowledge is shallow and humble, and may not be complete. Learned scholars who find omissions, if you could supplement and perfect them, would it not be appropriate? Written by the Old Man of Medicine Cave (Yaoyin Laoren) at Xincin Hall on the First Day of the First Month, in the first year of Xianchun.
The neck region belongs to the Foot-Taiyang Bladder Meridian (Zu Taiyang Pangguang Jing). Additionally, Scholar Xu states it is the Foot-Shaoyin Kidney Meridian (Zu Shaoyin Shen Jing), likely because the Kidney and Bladder are exterior-interiorly related. From the perspective of external pathogenic factors involving the Taiyang Meridian, initial exposure to Wind followed by Cold-Dampness leads to exterior symptoms such as fever and aversion to cold, resembling Shanghan (Cold Damage) syndrome. Symptoms include stiff and rigid neck, hyperextension of the waist and body resembling Zhongfeng (Wind Stroke), convulsions, and locked jaw, with a stiff body. Ancients noted that for stiffness, the pulse is deep, slow, wiry, and thin. Postpartum women with blood deficiency and excessive sweating are prone to Wind invasion and may present with these symptoms. The above details are recorded in Volume 3, Treatise 2, and are not repeated here.
For cases of inability to turn the head due to strain or improper pillow support, use Sanwuqi San (see *Heji*) and Zhuifeng San, applied to the neck along with Jifeng San.
If dizziness and blurred vision occur due to wind exposure, accompanied by pain in the Taiyang region and brain, and stiffness of neck and back tendons, use Xie Fu San and Duliang Wan.
Scholar Xu treated stiffness and pain of the neck tendons preventing turning of the head using a decoction of Chaenomeli Fructus (Mu Gua, 木瓜).
