The *Qianjin Fang* treats postpartum heat accumulation with constipation. Honey Suppository Method.
Take five he of white honey, simmer over low heat until it resembles hard syrup, dip into cold water, remove after a while, shape into a cylinder the size of a thumb and two inches long, insert into the anus to relieve constipation.
Discussion: Postpartum obstruction of both large and small bowels occurs because the intestines and stomach originally harbor heat. Due to childbirth, blood and fluids are discharged together, leading to dryness and exhaustion of body fluids. This causes stagnation and涩 (roughness/stagnation) in the intestines and stomach, where heat accumulates, resulting in the inability to pass stool or urine.
The *Qianjin Fang* treats postpartum dribbling urination. Okra Root Decoction (Kuigen Tang).
Okra Root (two liang, dried), Tongcao (two liang), Plantaginis Semen (Cheqianzi, one sheng), Hair Ash, Rhubarb (Dahuang), Cinnamon Twig (Guixin) (each one liang), Winter Melon Juice (seven he), Ginger (six liang), Talcum Powder (Huashi mo, separately ground, one liang).
Cut the above nine ingredients, boil in seven sheng of water until reduced to two and a half sheng, strain, add Talcum Powder, divide into three doses. Upon my examination, since Rhubarb is included, this formula can also treat constipation; thus, it is recorded here. It should be administered after carefully assessing deficiency and excess.
Treats postpartum exhaustion of body fluids with obstruction of both large and small bowels.
Peony Root (Shaoyao), Rhubarb (Dahuang), Aurantium Fructus (Zhiqiao), Sesame Seed (Maren) (equal parts).
Grind into fine powder, make pills with refined honey the size of wheat seeds. Take twenty pills on an empty stomach with warm water. Another version includes Licorice Root (Gancao) and Gardenia Seed (Shanzhiren).
Treats postpartum constipation and涩 (roughness/difficulty). Peach Blossom Powder (Taohua San).
Peach Blossom (Taohua), Okra Seed (Kuizi), Talcum (Huashi), Areca Nut (Binglang) (equal parts).
Grind into fine powder, take two qian per dose. Mix with Scallion White Decoction (Congbai Tang) and take on an empty stomach.
Formula for postpartum fecal incontinence: Take grass from old swallow nests, burn to ash, mix with wine and take half a qian. Also effective for men.
Formula: Alum (Fan Shi, calcined), Oyster Shell (Muli, roasted).
Grind the above into powder equally, take one fangcun bi with wine, three times a day. Also effective for men.
Another formula for postpartum fecal incontinence where the patient is unaware of defecation.
Ampelopsis Radix (Bailian), Peony Root (Shaoyao).
Grind each into powder equally, take one fangcun bi with wine.
Discussion: Various types of Lin syndrome (dysuria) after childbirth occur because heat pathogens lodge in the bladder during delivery. Internal deficiency leads to frequent urination, while heat causes涩 (painful/difficult urination), hence termed Lin. Additionally, due to injury from childbirth, Qi deficiency carries heat; heat clashing with blood flows into the uterus, causing blood to exit with urine, forming Blood Lin (Xuelin). "Lin" refers to dripping like rain.
The *Sanyin Fang* discusses that treating Lin and retention before and after childbirth differs. Before childbirth, the method should focus on calming the fetus; after childbirth, it should focus on removing blood stasis. For treatments involving Cold Lin, Heat Lin, Paste Lin, Stone Lin, and Qi Lin, the principles are consistent, but one must always gauge deficiency and excess. Epilobium Herb (Qumai) and Polygoni Multiflori Caulis (Puhuang - *Note: Standard term is Puhuang, usually Lygodii Spora or Polygoni Multiflori Caulis depending on context, here likely Lygodii Spora/Polygoni Multiflori Caulis used for hemostasis*) are essential herbs for postpartum use. One must seek the underlying cause to not miss the key to treatment.
