Postpartum alternating chills and fever are generally caused by deficiency and damage of Qi and Blood, leading to disharmony between Yin and Yang. If Yang is predominant, there will be alternating chills and fever with a tendency toward heat; if Yin is predominant, there will be alternating chills and fever with a tendency toward cold. For cases where Yin is predominant and cold symptoms are more frequent, Zengsun Siwu Tang (Modified Four Substances Decoction) or Liyin Jian (Yin-Nourishing Decoction) should be used. For cases where Yang is predominant and heat symptoms are more frequent, Siwu Tang (Four Substances Decoction) or Sanyin Jian (Three Yin Decoction) should be used. For cases where Yang Qi sinks into the Yin level causing alternating chills and fever, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) or Buyin Yiqi Jian (Nourish Yin and Augment Qi Decoction) should be used. For cases where both Yin and Yang are deficient resulting in alternating chills and fever, Bazhen Tang (Eight Treasures Decoction) or Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction) should be used. For cases where lochia (postpartum residual blood) fails to disperse and flows into the Yin level causing chills and fever, iejuejin Jian (Juejin Decoction) or Dianbao Jian (Dianbao Decoction) should be used. For cases of Blood excess and Qi stagnation, Mingdan (Life-Restoring Pill) should be used.

Chen Wuyue stated: When broken blood obstructs the Yin meridians, it causes cold; when it obstructs the Yang meridians, it causes heat. Wuji San (Five Accumulations Powder) should be used. If there is an external pathogenic invasion, one should follow the treatment principles for "Postpartum Fever" mentioned earlier.

Ru refers to straw mats. The difficulty of childbirth while sitting on straw mats leads to excessive mental and physical fatigue, hence the term Ru Lao (Childbed Labor). This is essentially postpartum exhaustion. Its symptoms may include alternating chills and fever resembling malaria, headache with spontaneous sweating, dizziness and lethargy, pain in all joints, fatigue with panting, poor appetite, emaciation, and other signs of deficiency. Treatment should primarily focus on tonifying Yuan Qi (Primary Qi). In the immediate postpartum period, if Ru Lao causes fatigue and weakness, Zhu Yao Tang (Pig Kidney Soup) is excellent, or Huang Ci Ji Tang (Yellow Hen Chicken Soup) and Bai Fuling San (White Poria Powder) can be used. If Ru Lao results in incessant deficiency sweating, Mother Chicken Soup is suitable. If accompanied by internal cold of the Zang organs, Lamb Soup is suitable. If both Qi and Blood are deficient, Wufu Yin (Five Blessings Drink) or Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction) should be used. If accompanied by external pathogenic heat with central cold and aversion to cold on the back, Liyin Jian (Yin-Nourishing Decoction) should be used with detailed modifications. If accompanied by Yang deficiency and internal cold, Wujunzi Jian (Five Gentlemen Decoction) or Liyin Jian (Yin-Nourishing Decoction) should be used. If Yang is exuberant and Yin is deficient with internal heat, Wufu Yin (Five Blessings Drink) added with Shaoyao (Peony Root), Huangqin (Scutellaria Root), and Digupi (Groundcherry Bark) should be used according to circumstances.
Zhu Yao Tang (See "Ancient Formulas of Furen Gui") treats postpartum childbed labor, alternating chills and fever like malaria, spontaneous sweating with weakness, cough, headache, and abdominal pain.
Pig Kidney (one pair) Danggui (Angelica Sinensis Root) Baishao Yao (White Peony Root, stir-fried with wine, each 1 liang)

Combine with one he measured unit of late rice, one qian of Xiangchi (Fermented Soybean), scallion, Sichuan pepper, and salt, and cook into a thin porridge. Take once daily on an empty stomach for miraculous effect. Adding Renshen (Ginseng Root) is even better.
