八珍汤(方见首卷)
What is the cause of lochia not flowing down? It is due to wind-cold invasion, qi stagnation, and blood stasis coagulation; if it is due to blood deficiency with no blood to flow, the complexion will be yellowish-white without abdominal distension or pain. For wind-cold causing blood coagulation, Shixiao San (Pain-Dispersing Powder) is used; for excessive blood loss during childbirth leading to blood deficiency, Shengyu Tang (Sage's Replenishing Decoction) is used to tonify and promote circulation.
[Note] Postpartum lochia not flowing down may be caused by wind-cold interfering, leading to qi stagnation and blood coagulation, which must present with abdominal distension and pain; or it may be caused by excessive blood loss during delivery, leaving no blood to flow, presenting with a yellowish-white complexion and no abdominal pain, allowing for differentiation based on these signs. For blood coagulation, Shixiao San (Pain-Dispersing Powder) is used to remove stasis and promote flow; for blood deficiency, Shengyu Tang (Sage's Replenishing Decoction) is used to tonify and promote flow.

Lochia that does not cease damages the Ren Mai (Conception Vessel) and Chong Mai (Penetrating Vessel), failing to consolidate and resulting in continuous dripping. This may be due to deficiency and damage where blood cannot be controlled, or due to blood stasis stopping in the abdomen. One must examine whether the blood color is dark or pale, and whether the odor is foul, fishy, or repulsive, clearly distinguishing between deficiency and excess; deficiency requires tonification, while excess requires attack. For deficiency, Shiquan Dabu Tang (Ten Thousand Tonifying Decoction) plus Ejiao (Donkey-Hide Gelatin) and Xu Duan (Radix Dipsaci) is used; for stasis, Foshou San (Buddha's Hand Powder) is used to tonify and promote flow.
[Note] Postpartum lochia is the dirty blood containing the fetus, which should descend with the placenta during delivery. If it continues for a long time without ceasing, dripping continuously, it may be due to deficiency and damage of the Chong and Ren vessels, where blood cannot be retained; or it may be due to incomplete expulsion of stasis remaining in the abdomen, being expelled as it transforms. One must examine the color of the blood: whether it is turbid and unclear, or pale and dull, and whether the smell is offensive, fishy, or foul, to distinguish between excess and deficiency for appropriate treatment with purgative or tonic methods. For deficiency, Shiquan Dabu Tang (Ten Thousand Tonifying Decoction) plus Ejiao (Donkey-Hide Gelatin) and Xu Duan (Radix Dipsaci) is suitable to tonify and consolidate; for stasis, Foshou San (Buddha's Hand Powder) is suitable to tonify and promote flow.

Postpartum headache with a yellowish-white complexion indicates blood-deficiency headache, lacking both exterior and interior syndromes; if lochia does not flow accompanied by abdominal pain, it is necessarily due to blood stasis attacking upward. Xiong Gui Tang (Ligusticum and Angelica Decoction) is most effective for removing stasis; for deficiency, Bazhen Tang (Eight Treasures Decoction) plus Man Jing Zi (Fructus Viticis) is used.
[Note] Postpartum headache, if accompanied by a yellowish-white complexion, without exterior symptoms such as chills, fever, and body ache, nor interior symptoms such as constipation, is caused by excessive blood loss during childbirth, resulting in blood-deficiency headache. If lochia does not flow and is accompanied by abdominal pain, it belongs to pain caused by blood stasis attacking upward. To remove stasis, use Xiong Gui Tang (Ligusticum and Angelica Decoction); to tonify deficiency, use Bazhen Tang (Eight Treasures Decoction) plus Man Jing Zi (Fructus Viticis).
