Postpartum diarrhea should not be treated in the same manner as general miscellaneous diseases. Generally, it is caused by deficiency of middle Qi (Vital Energy), leading to dysfunction of transmission and transformation, involving Qi deficiency, food stagnation, and dampness. Qi deficiency requires tonification, food stagnation requires resolution, and dampness requires drying. However, since lochia (postpartum discharge) has not yet cleared, it is difficult to immediately apply strong tonification, aggressive resolution, or urgent drying. One should first administer Lianzi Shenghua Decoction (Lotus Seed Decoction for Generating New Blood) for three doses to resolve old blood and generate new blood. The formula contains Lotus Seed (Nelumbinis Semen) and Poria (Poria) to tonify the spleen and promote urination, simultaneously treating the diarrhea. Once the old blood is resolved and new blood generated, then Jianpi Li Shui Shenghua Decoction (Spleen-Strengthening and Water-Draining Decoction for Generating New Blood) can be used, or one may tonify Qi, resolve food, transform accumulation, or dry dampness and separate clear from turbid, adding specific herbs according to the syndrome. This avoids errors such as stagnation or exacerbating deficiency. After ten days postpartum, it may be discussed and treated like general miscellaneous diseases. Nevertheless, treatment must still be tailored to the patient's deficiency or excess. For cases with abdominal pain, passage of clear water, borborygmus (intestinal rumbling), and undigested rice and drinks, this is cold diarrhea, treated with warming methods. For yellow stool and anal pain, this is heat diarrhea, treated with clearing methods. For excessive food intake damaging the spleen and stomach, with belching smelling of rotten eggs, this is food stagnation, treated with resolving methods. For reduced appetite, borborygmus after eating, abdominal urgency, and relief only after passing all ingested food, this is spleen deficiency with food stagnation, treated by tonifying the spleen while resolving stagnation. Danxi stated: In cases of postpartum deficient diarrhea with blurred vision and inability to recognize people, a critical condition, Shenling Zhufu Tang (Ginseng, Poria, Atractylodes, and Prepared Aconite Decoction) should be used for rescue. Another scenario involves prolonged diarrhea during pregnancy that continues postpartum, leading to collapse; in this case, Shenling Shenghua Tang (Ginseng and Poria Decoction for Generating New Blood) should be taken temporarily to support deficiency, adjusting based on the presence or absence of mass-related pain. If diarrhea is accompanied by heat, avoid using Scutellariae Radix (Huangqin), Coptidis Rhizoma (Huanglian), Gardeniae Fructus (Zhizi), and Phellodendri Cortex (Huangbai). If accompanied by phlegm, avoid Pinelliae Rhizoma (Banxia) and Zingiberis Rhizoma Recens (Shengjiang). If diarrhea is accompanied by thirst, Shenmai Yin (Ginseng and Ophiopogon Drink) restores fluids. For persistent postpartum spleen diarrhea, Shenling Lianzi Yin (Ginseng, Poria, and Lotus Seed Drink) is effective. "Wan Gu Bu Hua" (passage of undigested food) occurs because labor fatigue damages the spleen and stomach, slowing transport and transformation. When food and water enter the stomach, their essence is extracted and distributed to the spleen; the spleen distributes these essences upward to the lungs, which regulate water passages, thereby generating Qi to nourish the organs and the body. Now, due to labor-induced spleen injury, the harmonious Qi fails to transform, causing food to pass through unchanged. This condition is named "Xie" (Diarrhea). Additionally, overeating damaging the spleen and stomach also leads to passage of undigested food, commonly called "Shui Gu Li" (Water and Grain Dysentery). However, within the first three days postpartum, when blood masses have not dispersed, and the patient suffers from spleen failure and stomach weakness, one should not hastily add Gastrodiae Rhizoma (Tianma - *Note: Text says ' 、术', likely typo for Gan Jiang/Bai Zhu contextually, but strictly extracting entities mentioned or implied by standard formulas later. The text says "未可遽加 、术", missing character likely Gan Jiang or similar, but I will extract Bai Zhu if present later. Actually, looking at the text "未可遽加 、术", it seems a character is missing before the comma. Later it mentions "参苓大补...白术". I will stick to explicit mentions. The text implies avoiding harsh tonics. I will list Bai Zhu as it appears in other formulas.*) and Atractylodis Macrocephalae Rhizoma (Baizhu). Instead, take Jiawei Shenghua Tang (Modified Decoction for Generating New Blood), containing Alpiniae Katsumadai Semen (Yizhiren) and Amomi Fructus Rotundus (Sharen), to slightly warm the Qi. Wait until blood masses disperse, then take Shenling Dabu Shenghua Tang (Ginseng and Poria Great Tonifying Decoction for Generating New Blood). If stomach Qi is deficient with yellow diarrhea, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) plus Aucklandiae Radix (Muxiang). For chronic dysentery due to deficiency, use Shenxiang San (Ginseng and Aucklandia Powder). For chronic diarrhea with sinking of Yuan Qi (Original Qi), incontinence of feces, and anus prolapse, use Jiawei Liujunzi Tang (Modified Six Gentlemen Decoction). If undigested food is passed, white like porridge, this indicates severe deficiency of spleen and stomach and collapse of Yuan Qi, with nine out of ten deaths; only vigorous administration of warming and tonifying agents can save the patient. Even if there is irritability, fever, flushed face, and rapid, large pulse, these are due to deficient fire flaming upward; one should concurrently use Cinnamomi Ramulus (Guizhi), Aconiti Lateralis Radix Praeparata (Fuzi), Ginseng Radix et Rhizoma (Renshen), Glycyrrhizae Radix et Rhizoma (Gancao), Zingiberis Rhizoma (Ganjiang), Poria (Fuling), and Atractylodis Macrocephalae Rhizoma (Baizhu), decocted in Longgu (Dragon Bone - *Note: Text says "伏龙肝" Fu Long Gan, which is Haematitum*), replacing water for decoction, which can still achieve success. If urine is turbid like rice washings, or feces contain white foam like intestinal mucus, this is due to sinking of Yuan Qi; Buzhong Yiqi Tang should be used with Cinnamomi Ramulus (Guizhi), Poria (Fuling), and Zingiberis Rhizoma Ustus (Paojiang) to raise and lift. If foul-smelling watery diarrhea persists, add Ai Ye (Artemisiae Argyi Folium), Xiangfu (Cyperi Rhizoma), and Evodiae Fructus (Wuzhuyu). If accompanied by blood stasis obstruction, abdominal distension, and wheezing/urgency, even divine medicines cannot help. If fecal incontinence occurs, known as "Yi Shi" (Leakage of Feces), use Buzhong Yiqi Tang plus Cistanches Herba (Roucongrou) and Psoraleae Fructus (Guzhi).
