Postpartum hypochondriac pain is caused by the flow of lochia (residual blood) into the Liver Meridian. The Foot Jueyin Liver Meridian traverses the hypochondriac region, thus causing pain. This condition presents as either deficiency or excess syndromes and requires differentiated treatment to avoid misdiagnosis. If there is distension under the hypochondrium that cannot be touched or pressed, it indicates blood stasis; the appropriate treatment is to remove the stasis using Xiongui Xiegan Decoction (Cnidium and Angelica Decoction to Drain the Liver). If there is pain in the hypochondrium that is relieved by pressure, accompanied by body movements involving the ribs resembling Ben Tun syndrome (a sensation of Qi rushing upward), this results from excessive blood loss leading to Liver deficiency; the appropriate treatment is Danggui Dihuang Decoction (Angelica and Rehmannia Decoction). *Zunsheng* states: Hypochondriac pain belongs to Qi stagnation, treated with Guishao Qinggan Yin (Angelica and White Peony Decoction to Clear the Liver).
Xiongui Xiegan Decoction: Treats postpartum hypochondriac pain due to blood stasis, characterized by intolerance to touch.
Ingredients: Dangguiwei (Tail of Angelica Sinensis, 1 qian), Chuanxiong, Xiangfu (processed with urine of a young boy, each 0.8 fen), Qingpi (Green Tangerina Peel), Zhike (Aurantium Fructus, stir-fried with bran, each 0.6 fen), Taoren (Peach Seed, 8 grains, skin and tip removed), Honghua (Carthamus Flower, 4-5 fen). Decoct with urine of a young boy and wine.
Danggui Dihuang Decoction: Treats postpartum hypochondriac pain due to deficiency resulting from excessive loss of Liver Blood.
Ingredients: Renshen (Ginseng), Danggui (Angelica Sinensis, washed with wine), Baishao (White Peony Root, stir-fried with wine), Shudi (Prepared Rehmannia Root, each 1.5 qian), Chenpi (Aged Tangerina Peel), Zhi Gancao (Honey-fried Licorice Root), Guixin (Cinnamon Twig, each 1 qian). Use fresh ginger and jujube as guides for decoction.
Guishao Qinggan Yin: Treats postpartum hypochondriac pain due to Qi stagnation.
Ingredients: Danggui (1.5 qian), Baishao, Jiegeng (Platycodon Root, each 0.6 fen), Binglang (Areca Seed), Zhike (stir-fried with bran, each 0.3 fen), Guixin, Qingmuxiang (Tasmannia Stem), Chaihu (Bupleurum Root, each 0.25 fen).
Generalized body pain after childbirth occurs because the joints open during delivery, blood vessels disperse, and Qi deficiency fails to fill the meridians. Consequently, blood in the meridians stagnates and does not dissipate, causing tendons and vessels to contract and pull, leading to stiffness in bone joints. Thus, one may experience inability to turn the waist and back, inability to extend or flex limbs, or symptoms such as fever and headache. Qitong San (Qi-Pain Resolving Powder) is the primary formula. If mistakenly treated as Taiyang Shanghan (Cold Damage) with sweating methods, it will cause tendons to tremble, cold extremities, and transform into Jing Zheng (Spasm Syndrome); in such cases, Shiquan Dabu Tang (Ten Significant Tonifying Decoction) should be used. If finger joints are painful, Buyi Zhishen Tang (Tonify Middle and Benefit Kidney Decoction) is indicated. If feet and knees are swollen or painful, Duhuo Jisheng Tang (Angelica Pubescens and Porcinus Decoction) is indicated. *Yitong* states: Postpartum pain in hands, feet, and body is due to Blood deficiency failing to nourish. Wandering pain in hands and feet indicates that Qi and Blood fail to nourish the four extremities, while turbid Qi flows into the limbs causing swelling, and Yin Fire travels around causing pain. The text suggests Yangrong Jia Heijiang Tang (Nourish Ying Decoction plus Blackened Ginger) as the treatment.
Qitong San: Treats generalized body pain after childbirth due to Qi and Blood deficiency with stagnation; do not mistake it for Shanghan.
Ingredients: Danggui (2 qian, soaked in wine), Huangqi (Astragalus Root, 1 qian, honey-fried), Baizhu (Atractylodes Macrocephala Rhizome, soil-fried), Huainiuxi (Achyranthes Bidentata Root, steamed), Duhuo (Angelica Pubescens Root), Rougui (Cinnamon Bark), Jiubai (Leek White, each 0.8 fen), Gancao (Licorice Root, 0.3 fen).
Buyi Zhishen Tang: Specifically treats postpartum consumptive disease with finger joint pain, headache, and sweating.
Ingredients: Renshen, Honey-fried Huangqi, Danshichidou (Light Soybean, each 1 qian), Danggui (2 qian, soaked in wine), Jiubai (0.5 fen), Fresh Ginger (3 slices), Pig Kidney (1 piece).
Preparation: First, boil the pig kidney until cooked, take two cups of its juice, and decoct the herbs in this juice until reduced to 0.8 cup. Take warm.
