Pi (abdominal mass) refers to a condition where blood membranes enclose fluid, lodging obliquely beside the hypochondrium, causing intermittent pain. Only Pi can trigger tidal fever and generate alternating chills and fever; thus, patients with malaria often accumulate yellow fluid in their bodies, which, over time, re-condenses into a Pi mass in the epigastrium, resulting in persistent chills and fever. Those suffering from this ailment should refer to this method. Children possess harmonious Zang-Fu organs and smooth Ying-Wei (Nutritive and Defensive Qi), allowing body fluids to flow naturally; even if they consume large amounts of water or liquids, it will not cause disease. However, improper feeding and regulation can lead to obstruction and reversal of the San Jiao (Triple Burner), causing fluid retention and preventing the intestines and stomach from circulating freely. As if attacked by cold qi, these factors condense to form Pi. For mild cases, Jizhi Muxiang Wan (Accumulation Stagnation Costus Pill) is used; for severe cases, Qu Pi Wan (Expel Pi Pill) is employed. Qian Yi (courtesy name Zhongyang) stated: "There is Pi in the abdomen, refusal to eat, but willingness to drink milk." This is the symptom. One should gradually administer Baibing Zi (White Cake Pill) to purge it. If not treated early, the child will be unable to eat, leading to Spleen-Stomach deficiency, inability to move the limbs, generation of various pathogens, emaciation, and eventual development into Gan syndrome (infantile malnutrition).
Muxiang Wan (Costus Pill): Treats vomiting milk and diarrhea of milk, with a sour and foul odor. This results from incessant crying and feeding the infant, causing milk stagnation and failure to digest, known as Milk Accumulation. A hard abdomen, fever, thirst, vomiting, and diarrhea result from unregulated diet and sleeping immediately after overeating, known as Food Accumulation. Abdominal pain with crying and loose stools resembling crab excrement result from violating the Qi, disharmony of Ying-Wei, and prolonged duration, known as Qi Accumulation. After malaria, a Pi mass forms inside the abdomen.
