Huoluan (Acute Gastroenteritis/Vomiting and Diarrhea Syndrome), what is it? Its etiology involves weak Stomach Qi, internal injury from diet, and external contraction of Cold Pathogen. This syndrome often occurs during late summer and early autumn, when dampness and heat are steaming together. During the day, one accumulates cold and raw food; at night, one contracts Cold Pathogen again. Yin fails to ascend, Yang fails to descend, causing obstruction of Yin and Yang, resulting in this syndrome. If heart pain precedes, vomiting occurs first; if abdominal pain precedes, diarrhea occurs first. Simultaneous pain in the heart and abdomen with concurrent vomiting and diarrhea, and in severe cases, muscle cramps (Zhuanjin), accompanied by sounds and expulsion of matter, constitutes the manifestation of this syndrome, named Huoluan. Another condition presents as simultaneous pain in the heart and abdomen, with sounds but no matter expelled, inability to vomit or defecate, restlessness and confusion, and in severe cases, cramps extending into the abdomen, leading to death within moments. This is called Dry Huoluan (Gan Huoluan), commonly known as "Intestine-Twisting Sha." A surging pulse indicates easy treatment; a faint pulse with cold extremities indicates difficult treatment. If whole-body cramps occur, along with tongue retraction and kidney shrinkage (indicating severe depletion), it is a fatal prognosis. In emergencies, mix half cold water and half boiling water, add a little salt, named "Yin-Yang Decoction," to induce vomiting and expel retained food completely, which is optimal. For severe cases, moxa on the umbilicus for seven moxibustion cones. Cold-natured medicines must strictly be avoided. The treatment principle should guide clear Qi to ascend and turbid Qi to descend, dispel Cold and digest food, and differentiate Yin and Yang. Do not immediately consume rice soup or hot liquids, as doing so may cause death. Wait until the condition stabilizes for half a day before giving cold thin porridge soup. For winter Huoluan, Wuji San (Five Accumulations Powder) is suitable; for summer Huoluan, Huoling Tang is the primary formula. For persistent cramps, men should grasp their scrotum, and women should grasp both breasts; this is an excellent method.
