Water Gan (a type of eye growth) suddenly manifests as a single nodule. It may arise within the eyelids or on the cornea (Feng Lun). Sometimes the pain is sharp like needle pricks, radiating to the orbit and head with redness and burning pain. In other cases, there is no pain, and the shape varies in size. If the condition does not resolve, it may develop into Leaking Eye Syndrome (Lou Jing).
This condition mostly occurs between the sclera (Qi Lun) and the eyelids. If it arises on the cornea (Feng Lun), the eye will inevitably be damaged. There are two types: excess and deficiency. The excess type presents as small lesions with severe pain, while the deficiency type presents as larger lesions with mild pain.
The appearance resembles a black soybean, though some are elongated and round. It resembles Wood Gan but differs slightly in location and color. Black corresponds to Water, while green, blue, and azure correspond to Wood. If left untreated for a long time, it transforms into Head Wind (Lou Tou Feng). People often suffer from this. Although Wind belongs to Wood, why does a liver-related pathology belong to Water? This is because when Wind moves, Water moves; this is natural law. Head wind affecting the eyes often damages the pupil spirit (Tong Shen). The essence and fluid of the pupil spirit are attacked by wind, leading to stagnation and excessive Fire over time. The essence and fluid are scattered by the Fire, causing them to stagnate and knot along the damaged meridians, forming Gan. Gan is caused by Fire stagnation; Fire incorporates the transformation of Water. When Water is overcome by pathogenic factors, it loses its clear and moistening properties and instead struggles with Damp-Heat, transforming into Leak (Lou). Therefore, Water Gan is attributed to the Kidney and Gallbladder. The appropriate prescription is Juandu Yin:
Fangfeng (Saposhnikoviae Radix, 1 qian), Chishao (Paeoniae Radix Rubra), Chuanxiong (Ligustici Chuanxiong Rhizoma), Lianqiao (Forsythiae Fructus), Gancao (Glycyrrhizae Radix et Rhizoma), Niubangzi (Arctii Fructus, stir-fried and ground, each 8 fen).
Crush these ingredients into one dose. Add two zhong of plain water, decoct until reduced to eight fen, remove the dregs, and take warm. This formula is for treating the excess type (small and severely painful). For the deficiency type (large and mildly painful), remove Fangfeng, Lianqiao, and Niubangzi, and treat with Siwu Tang (Four Substance Decoction), adding Shudihuang (Rehmanniae Radix Praeparata) and Dangguishen (Angelicae Sinensis Radix) each at 8 fen, decoct and take.
Note: This condition arises from sores forming at the canthus (corner of the eye), discharging pus, or resembling saliva that sticks to the upper and lower eyelids. It is painless and lacks pannus (Yi Mo). This is due to restless Heart Qi causing rebellious pathogenic Heat from the Small Intestine, combined with Wind-Heat lingering in the eyelids. Pus and fluid may discharge from the sore or from the puncta (canalicular openings), often flowing continuously. The verse states: "Originally caused by Wind-Heat lingering in the eyes, condensing like pus and pouring like tears. To expel toxins and dispel wind without other diseases, apply Huanglian Gao (Coptis Ointment) to both eyes."
