Danxi stated: Nangyong (Scrotal Abscess) is caused by Damp-Heat sinking downward. In some cases, pus forms; this indicates that turbid Qi is moving downward and is about to flow into the Shendao (urethral passage). This occurs due to a deficiency in the Yindao (genitourinary tract) and impaired fluid metabolism. Once the pus is discharged, the condition will resolve spontaneously without medication; the key lies in proper nursing care. There are also cases where abdominal swelling gradually extends into the scrotum, causing severe swelling that leads to spontaneous rupture of the scrotum and suspension of the testicles. Apply charcoal powder to the affected area and wrap it externally with Perilla frutescens leaves (Zisuye), keeping the patient in a supine position for recovery. Another theory regarding abscesses entering the scrotum mentions that I have treated several patients whose conditions were entirely due to Damp-Heat invading the Liver Meridian. Treatment involved addressing the Liver Meridian while assisting with Yin-nourishing herbs. Even when pus ruptured and skin sloughed off, resulting in suspended testicles, none of the patients died.
One prescription uses wild Perilla frutescens leaves (Ye Zisuye), characterized by green upper surfaces and red undersides. Dry them by roasting, grind into powder, and apply topically. If the area is dry, mix with sesame oil before application. If the scrotal skin is missing, wrap the area externally with fresh green lotus leaves (Qing Heye); the skin will regenerate naturally.
Another discussion concerns Xuanyong (Hanging Carbuncle): This sore appears between the anus and the external genitalia. Initially, it is very itchy and resembles a pine cone. After forty days, it becomes red and swollen like a pine tree. If treatment is delayed, it will ulcerate, and both urine and feces will discharge from this opening, rendering it incurable.
Guolao Tang (National Elder Decoction): Use 1 liang of large Licorice Root (Gancao) with transverse striations, cut into segments approximately 3 cun long. Take one bowl of running water from a mountain stream; do not use well or river water. Dip the Licorice Root in the water and slowly roast over alternating mild and strong heat. Do not rush; this process must take three shichen (6 hours). Stop when the water has completely evaporated. Split the root to check if it is thoroughly moistened inside. Then, add two bowls of pure wine (Wuhui Jiu) and boil until only one bowl remains. Take warm. Administer once daily until resolved within half a month.
Jiangjun San (General Powder): Rhubarb (Da Huang, roasted), Fritillaria (Beimu), Angelica dahurica (Baizhi), Licorice Root tips (Gancao Jie). Grind all ingredients into powder. Mix with wine and take on an empty stomach. For patients with deficiency, add half the dose of Angelica sinensis (Danggui).
Another discussion concerns Damp-Heat in the Liver Meridian: Sores on the Yujing (penis), or painful swelling from Biandu (perianal abscess) or Xuanyong, or ulceration with pain, obstructed urination, or hanging scrotum/testicles.
Longdan Xiegan Tang (Gentian Liver-Draining Decoction): Gentian Root (Longdan Cao, stir-fried with wine until yellow), Alisma (Zexie, each 1 qian), Plantago Seed (Cheqianzi, stir-fried), Akebia Stem (Mutong), Rehmannia Root (Sheng Dihuang, stir-fried with wine), Angelica sinensis tail (Danggui Wei, stir-fried with wine), Gardenia Fruit (Shanzhi, stir-fried), Scutellaria Root (Huangqin, stir-fried), Licorice Root (Gancao). Chop one dose, decoct in water, and take on an empty stomach.
