Slow convulsion (Man Jing Feng) arises from external contraction of wind-cold and internal injury to milk and food, resulting in vomiting and diarrhea; or it occurs after a severe illness, or due to the error of improper emesis or purgation, leading to deficiency of both the spleen and stomach.
The spleen and lung are in a mother-son relationship. When the mother organ (spleen) is deficient, the son organ (lung) is also deficient, generating sticky phlegm. When the stomach qi is deficient, wind is generated. Since wind is active and moving, symptoms include eyes preferring to be open, phlegm stagnating in the throat sounding like a saw being pulled, cold breath from mouth and nose, relaxed lips with a bluish complexion, drooling at the corners of the mouth, and occasional spasms. This constitutes slow convulsion. Treatment should focus on dispelling wind and resolving phlegm, strengthening the spleen and nourishing the stomach. One must not recklessly use drugs such as Borneol (Bingpian), Musk (Shexiang), or Croton seed paste (Bafen). If there are cases with closed eyes and cold limbs, it is named slow spleen wind (Man Pi Feng), which is extremely critical. Immediate use of Yang-restoring medicines is required. Once the hands and feet gradually warm up, Xingpi San (Spleen-Awakening Powder) should be used for regulation. If medication is ineffective but the Taichong pulse remains palpable, moxibustion can be applied to Baihui point (GV20). However, if the face appears dull, spirit is despondent, breathing resembles a fish's mouth opening, voice sounds like a crow, spleen area hurts with rib movement, body is cold with sticky sweat, head shakes with hair standing erect, eyes are fixed or sores appear in the mouth, panting prevents lying flat requiring lateral position, lips are retracted and breathing is coarse, these are untreatable conditions.
Jiawei Hezhong San (Modified Harmonizing Middle Powder): Treats pediatric slow convulsion.
Ren Shen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma, radix removed, each 1 qian), Baifuling (Poria, peel removed), Chenpi (Citri Reticulatae Pericarpium, each 5 fen), Banxia (Pinelliae Tuber, 7 fen), Quanxie (Scorpio, stir-fried, 5 fen), Tianma (Gastrodiae Rhizoma, 7 fen), Xixin (Asari Radix et Rhizoma, 3 fen), Bohe (Menthae Haplocalycis Herba, 3 fen), Gancao (Licorice Root, 2 fen).
Crush into one dose. Decoct with fresh ginger and jujube. The nursing mother should also take this medicine.
Another discussion: When pediatric vomiting and diarrhea persist, evolving into slow convulsion, with spleen fatigue, drowsiness, silence, and anorexia, Xingpi San (Spleen-Awakening Powder) is suitable.
Ren Shen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma, radix removed), Baifuling (Poria, peel removed), Muxiang (Aucklandiae Radix), Quanxie (Scorpio), Jiangcan (Bombyx Batryticatus), Baifuizi (Ligustici Chuanxiong Rhizoma), Tianma (Gastrodiae Rhizoma), Gancao (Licorice Root).
Crush. Decoct with three slices of fresh ginger and one jujube in water.
Another prescription: Remove Tianma (Gastrodiae Rhizoma) and Jiangcan (Bombyx Batryticatus). Add Paonanxing (Arisaematis Cum Bile Rhizoma), Banxiaqu (Pinelliae Fermentatum), and 200 grains of Chencangmi (aged rice). Decoct in water until cooked and administer frequently.
Another discussion: Slow convulsion results from depletion of primordial qi leading to confusion. Emergency moxibustion on Baihui point (GV20) is indicated. If one waits until phlegm has descended without improvement before applying moxibustion, primordial qi will scatter and rescue will be impossible. This is because the transmission of zang-fu pathology has reached its extreme, ultimately returning to deficiency, primarily affecting the spleen. There is no excess wind to drive out, nor excess fright to treat. This is caused by spleen deficiency failing to control saliva, transforming it into phlegm. This formula specifically treats slow convulsion with tidal phlegm and spasms, or vomiting/diarrhea, anorexia, coma, and weak qi. Zijin Dingzi (Purple Gold Pellet) is suitable.
Ren Shen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma, radix removed), Baifushen (Poria cum Pinodia, peel and wood removed), Shanyao (Dioscoreae Rhizoma, stir-fried), Ruxiang (Olibanum), Chensha (Cinnabar, each 2 qian), Chishizhi (Halloysit Rubra, vinegar-fried, 7 fen), Shexiang (Musk, 1 qian), Fuling (Poria, 2 qian).
Grind into fine powder. Use one liang of cake flour to form pills the size of a bullet, coated with gold foil. Take one pill per dose, grind and dissolve in mint tea.
Case 1: A child vomited, refused food, had hand-foot spasms, phlegm obstructing upwards, cold finger tips, black forehead, and blue lips. This indicates kidney water overcoming heart fire. Wuyi Gongsan (Five-Agent Modified Cinnamon Powder) plus Muxiang (Aucklandiae Radix) and Paojiang (Zingiberis Rhizoma Preparatum) was used, bringing immediate relief. Paojiang (Zingiberis Rhizoma Preparatum) was then removed, and after two more doses, the child recovered.
Case 2: A child had tidal fever with spasms, surging phlegm-saliva, cold finger tips. From left cheek to Shen-You hours (3 PM - 7 PM), cyanosis hid white color, with intermittent hand-foot spasms. This is liver channel weakness overcome by lung metal causing tidal spasms, and spleen earth weakness causing cold limbs. Buchong Yiqi Tang (Tonifying the Middle and Augmenting Qi Decoction) was used to tonify spleen and lung, and Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) was used to nourish liver and kidney, resulting in recovery. Generally, when pathological qi is excessive, it should be recognized as insufficiency of primordial qi. Using methods to drain lung metal, attack liver wood, clear heat, or resolve phlegm would be erroneous.
Case 3: A child with slow convulsion slept much and cried upon fright. Cases with yellowish face and thin pulse are difficult to treat. This medicine is administered to the nursing mother.
Nursing Milk Preparation Method:
Muxiang (Aucklandiae Radix), Chenxiang (Aquilariae Lignum Resinatum), Huoxiang (Agastaches Herba), Dingxiang (Caryophylli Flos, halved), Chenpi (Citri Reticulatae Pericarpium), Ren Shen (Ginseng Radix), Shenqu (Massa Medicata Fermentata, stir-fried), Maiya (Hordei Fructus Germinatus, stir-fried).
Crush. Take 4 qian per dose with ten leaves of Zisu (Perillae Folium), ten slices of fresh ginger, and two jujubes. Decoct in water. Have the nursing mother take it after meals, express and discard old milk first, then lie supine for a moment to allow medicinal qi to enter the milk-generating vessels, then let the infant suck. Do not overfeed. This is also a good method.
