In cases of urinary retention where medication has proven ineffective, use 5 qian (approx. 18.75g) of Phytolaccae Radix (Shanglu), ground into powder, mixed with a small amount of Moschus (Shexiang). Cover the umbilicus with an old linen cloth from summer, place the medicine on the cloth. Once the medicinal qi penetrates the abdomen, urination will likely resume within one shichen (approx. 2 hours).
For cases where urination is obstructed for several days, resulting in generalized edema and fullness of the body and extremities, unresponsive to various medications, decoct 1 jin (approx. 500g) of Perillae Folium (Suye) in concentrated water and pour it into a foot basin. Allow the patient to sit over the basin to inhale the steam; add hot water as it cools. Externally, apply hot stir-fried salt to massage the umbilicus and all swollen areas. After some time, urination will resume, edema will subside, and the patient will recover.
Another prescription: Use 1 jin of Scallion White (Congbai) with roots intact, mash it, and stir-fry until hot. Divide into two portions, wrap each in cloth, and alternately apply hot compresses below the umbilicus. Urination will immediately resume. For urinary obstruction unresponsive to internal medicine, take 1 liang (approx. 37.5g) of Sodium Chloride (Yan), dissolved in warm water. After a while, induce vomiting by stimulating the throat with a finger. Vomiting will lead to immediate urination. This is because if the upper orifice is blocked, the lower orifice becomes closed.
Yet another method: Grind Gleditsiae Sinensis Fructus (Zaojiao) into powder and blow a tiny amount into the nose to induce sneezing. This follows the principle of "ascending." Just as blocking the upper opening of a water vessel prevents flow from the bottom, slightly lifting the qi (via sneezing) restores patency immediately.
The Classic states: "When Taiyin governs the year, Yin Bi occurs, characterized by difficulty in defecation, due to the failure of Yin Qi, with the pathology located in the Kidney." It also states: "When Taiyang prevails, there is dysfunction in hidden/obscure matters (referring to urination or reproductive functions) and pulling pain in the medial thigh." Food ingested must eventually be excreted. If defecation is obstructed, the mechanism of intake and output nearly ceases. It is urgent to promote bowel movement to clear old residue so new food can enter. However, constipation can be excess-type (Shi) or deficiency-type (Xu), requiring careful differentiation. If the patient's constitution is robust, Qi is sufficient, pulse is forceful, and appetite is good, indicating a true excess pattern suitable for purgation, use purgatives such as Bazheng San (Eight-Herb Corrective Powder), Chengqi Tang (Order the Qi Decoction), or Muxiang Binglang Wan (Costus and Areca Nut Pill), selecting one appropriately. Stop once the disease is addressed; do not overuse. Conversely, if the patient is physically weak, Qi deficient, pulse is thready/weak, and accompanied by poor appetite, even if signs suggest purgation are indicated, slow rather than rapid action is preferred. Use Baohe Wan (Preserve Harmony Pill) augmented with Zhi Shi (Immature Bitter Orange) to mildly promote bowel movement. For those who habitually have difficulty defecating, this is due to Blood insufficiency; use Run Chang Wan (Moisten Intestines Pill) or Mi Dao Fa (Honey Suppository Method).
