Sweat is the fluid of the Heart. Only sweating on the head does not require treatment. Children possess a constitution of "Pure Yang" (Chunyang, 纯阳). The head is the convergence point of all Yang meridians. The Heart belongs to Fire. Sweating on the head is an image of fire flaring upwards. Therefore, sweating on the head is an image of Clear Yang Qi dispersing outward and does not require treatment.
Spontaneous sweating (Zihan, 自汗) refers to continuous sweating day and night. This is due to heat in both Blood and Qi, and deficiency of Ying (Nutritive) and Wei (Defensive) Qi. It should be primarily treated with Danggui Liuhuang Tang (Cinnamon Twig and Rehmannia Decoction). In this formula, Huangqi (Astragali Radix) is used to tonify Qi; Danggui (Angelicae Sinensis Radix) and Shengdihuang (Rehmanniae Radix Recens) are used to tonify Ying blood; Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), and Huangbai (Phellodendri Cortex) are used to drain the fire in Blood and Qi; Fuxiaomai (Tritici Levis Fructus) is used as a guiding herb, entering the Lung meridian to drain heat from the skin and hair follicles. This is a miraculous formula for treating various types of sweating.
Night sweating (Daohan, 盗汗) refers to sweating spontaneously during dreams/sleep, which stops upon waking. The pathology lies in the Kidney. It should be primarily treated with Danggui Liuhuang Tang combined with Qian's Zhihan San (Qian's Stop-Sweating Powder).
Qian's Zhihan San: Baipushan (Bai Pusa Fan, dried leaf fan of *Trachycarpus fortunei*), burned until carbonized but retaining its nature, ground into powder, and added to the decoction. This utilizes the principle of "imagery correspondence" (using the object's function to treat the condition).
The son of Jiang Lanfeng from this county, aged seven, had sweat flowing like water from his head and face. He was administered a decoction of Renshen (Ginseng Radix et Rhizoma) and Danggui (Angelicae Sinensis Radix) boiled with Zhu Xin (Porcis Cor) and consumed, resulting in recovery.
When children are ill, there is heat; when there is heat, wind is generated. Regulation is indispensable.
In Liver Heat, the eyes appear greenish-blue, the hands grasp at clothing collars, and the child fidgets with objects aimlessly. Xieqing Wan (Drain the Green Pill) or Danggui Longhui Wan (Angelica and Gardenia Pill) should be used.
If there is body heat, hot breath from the mouth, and symptoms of damp-warmth or wind-warmth, it is termed Wind-Heat, which is also a manifestation of Liver Heat. Shengxi San (Fresh Rhinoceros Horn Powder) or Tuojia San (Shedding Carapace Powder) should be used.
In Heart Heat, the eyes appear red. Observing sleep patterns: if the child sleeps with their face down or on their back, has warm breath, and exhibits convulsions or teeth grinding, Daochi San (Guide Out Redness Powder) or Huanglian Anshen Wan (Coptis Calming Spirit Pill) should be used.
Continuous unceasing heat is also Heart Heat. If severe, it may develop into Jingxian (Convulsive Epilepsy). Huanglian Anshen Wan (Coptis Calming Spirit Pill) should be used.
If there is heat sensation in the eyes, it indicates Heart Deficiency. Qian's Anshen Wan (Qian's Calming Spirit Pill) should be used.
In Spleen Heat, the eyes appear yellowish, and the child plays with their tongue. Xiehuang San (Drain Yellow Powder) or Yinchin Wuling San (Artemisia Capillaris and Five-Ingredient Powder with Poria) should be used.
If there is only warmth without high fever, it is also termed Warm-Thermal disease, representing Spleen Heat. Ren Shen Baihu Tang (Ginseng White Tiger Decoction) should be used.
In Lung Heat, the eyes appear cloudy and white, and the child pinches their eyebrows, eyes, face, and nose. Ganjie Tang (Licorice and Magnolia Bud Decoction) or Mutong San (Akebia Powder) should be used.
Intermittent fever that subsides and returns at the same time the next day is termed Tide Fever (Chaore, 潮热). This is also Lung Heat. Digupi San (Lycium Bark Powder) should be used.
In Kidney Heat, the eyes lack luster, there is photophobia, heaviness in the spine, excessive whiteness in the sclera, and the fontanelle remains open/unhealed. Dihuang Wan (Rehmannia Pill) should be used.
